Valve-in-valve transkatétrová implantace chlopenní náhrady do mitrální pozice za použití techniky LAMPOON

Za posledních deset let došlo k výraznému vývoji možností katetrizačních intervencí na mitrální chlopni, a to jak na nativní chlopni, tak i na bioprotézách. I když v současnosti stále převažuje chirurgická léčba mitrálních vad, transkatétrová implantace mitrální chlopně nachází uplatnění hlavně u pacientů, kteří jsou kontraindikováni ke kardiochirurgickému výkonu anebo mají vysoké operační riziko. Katetrizační implantace bioprotézy do mitrální pozice má svoje specifika i komplikace. Jejich největší limitací je možnost vzniku významné obstrukce ve výtokovém traktu levé komory. V následujícím článku prezentujeme na kazuistice možnost snížení rizika této komplikace za použití techniky LAMPOON. Technika LAMPOON umožňuje provedení transkatétrové implantace mitrální chlopně i u pacientů, kteří by jinak byli k výkonu kontraindikováni pro vysoké riziko vzniku obstrukce LVOT.

Valve-in-valve transcatheter implantation of a valve replacement in the mitral position using the LAMPOON technique
Over the last ten years, there has been a significant progression in the possibilities of trascatheter intervention on the mitral valve, both on the native valve and on bioprostheses. Although surgical treatment of mitral defects currently prevails, transcatheter mitral valve replacement is mainly used in patients with a high surgical risk or in patients with valve replacement with a bioprosthesis in whom reoperation is no longer possible. Transcatheter implantation of a bioprosthesis into the mitral position has its specifics and complications. One of the possible complications is the formation of a significant obstruction in the outflow tract of the left ventricle. In the following article, we present a case report on the possibility of reducing the risk of this complication using the LAMPOON technique. The LAMPOON technique allows transcatheter mitral valve implantation even in patients who would otherwise be contraindicated for the procedure due to high risk of LVOT obstruction.
