Hormonal therapy in transsexual patients (TS) includes sexagens administration: androgens in female-to-male transsexual patients (FtM) and oestrogens and antiandrogens in male-to-female transsexual patients (MtF). Duration of hormonal therapy should continue at least 1 year before gender reassignment surgery. Hormonal therapy supresses former gender and induces partially new gender changes. Hormonal therapy continues subsequently after surgery during life. Hormonal therapy in MtF TS includes oestrogens and antiandrogens application. Introduction:The effect of obesity on chronic venous disease has been known. The effect of body height has not been studied until now, although venous pressure is clearly influenced by hydrostatic pressure in the vertical position. Biosimilar drugs or drugs which are similar to reference biological drugs represent a new treatment option within the biological treatment. They are already the standard biotherapeutics option in some fields of medicine, in others as in diabetology, remain to be evaluated. The article presents the definition of biosimilar drugs and summarizes the therapeutic benefits and attitudes of agencies for drug control in the United States and Europe.

Hyponatraemia is the most common electrolyte disbalance in clinical practice, which is associated with increased patients morbidity and mortality. At present the pathophysiology of hyponatraemia is explored in more details, antidiuretic hormone and osmoregulation play the major roles. This article informs about relatively new classification of hyponatraemia for clinical practice based on the severity of clinical symptoms and based on the effective serum osmolality. It also offers diagnostic and treatment guidelines of hyponatraemia, which are based on current recommendations of the world experts and on the evidence based medicine.

We describe a case of multicentric Castleman disease with generalized lymphadenopathy and splenomegaly, accompanied by typical B symptoms - loss of 15 kg, fever of non-infectious origin, night sweats, symptoms of anemia. Histological examination of the nodes with the highest accumulation of fluorodeoxyglucose, taken from mediastinum by thoracoscopy, revealed plasmocellular type of Castleman disease. Tests for HIV and human herpesvirus 8 (HHV-8) were negative. Three recurrences of herpes zoster indicating an alteration of immunity preceded the diagnosis of disease. Our report describes the case of patient with hypersensitive reaction regularly arising early after initiation of haemodialysis. This characteristic reaction with pletoric face coloration, bronchospasm, increase of blood pressure, anxiety and decrease of blood oxygen saturation at the consequence and central cyanosis was regularly present without dependence on type of dialysis membrane, drug premedication or prophylactic flushing haemodialysis system by isotonic natrium chloride solution.
