Atrial macroreentry tachycardias (AMRT) independent of the conduction across the subeustachian isthmus represent a relatively rare group of different reentry circuits developing from individual arrhytmogenic substrates. The purpose of the study is to present causes, mapping facilities, and the results of catheter ablation of these arrhythmias. Patients and methods:Forty-two patients (11 females), aged 57.2 ± 12.8 years, presenting with clinically significant AMRT, were referred to mapping and catheter ablation. Eighteen patients had known structural heart disease, in other 24 patients, no significant structural heart...

Objective:The objective of the study is to assess the practical benefit of the determination of serum levels of free immunoglobulin chains in the serum (S-FCH) in patients with different types of monoclonal gamapathies (MG), in particular with regard to multiple myelome (MM). Patient set and method:The analysed set of 196 patients contained 119 patients with MM (27 patients examined for diagnosis and 92 "under treatment" assessed in different phases of MM), 52 patients with monoclonal gamapathy of uncertain significance (MGNV), 8 patients with solitary plasmocytoma, 9 patients with AL-amyloidosis, and 8 patients with primary...
The relation between prolactin values and the degree of functional disability assessed by a HAQ questionnaire in patients with rheumatoid arthritis

Rheumatoid arthritis (RA) is a chronic inflammatory disease of joints the ethiopathogenesis of which is still uncertain. The finding that prolactin (PRL) has an influence on immunity has lead to assumptions that it may interfere with RA ethiopathogenesis. The consequences of RA consist in the destruction of joint cartilage and the adjacent bone, which causes pain and restricts the function of the affected joint. The objective of the study was to examine basally stimulated prolactin values in patients with RA in order to identify the relation between prolactin levels, inflammation markers and the patient's functional condition. Set and methods:The...

Objective:The effectivity of ICD therapy is usually evaluated according to the total mortality of ICD patients. The aim of this study is to analyse the total mortality of long-term followed ICD patients and describe potential markers increasing mortality. Patients and methods:We have observed 138 consecutive patients in mean age of 62.0 ± 12.2 year (108 M, 30 F) with mean LV EF 0.38 ± 0.14, who had ICD implanted for malignant ventricular arrhythmias from X/95 to XII/02. The mean follow-up was 47.35 months. 99 patients had CAD (coronary artery disease), 16 CMP (dilated cardiomyopathy), 5 ARVC (right ventricle...
The influence of the passive phase shortening on the diagnostic yield of nitroglycerine stimulated head-up tilt test

Vnitr Lek 2005, 51(11):1272-1276
The aim of the study was to compare the diagnostic yield of three different head-up tilt test protocols. A total of 438 patients with syncope of unexplained etiology were examined. Protocol I was represented by Westminster protocol (passive test for 45 minutes, 60) followed, if negative, by nitroglycerine (NTG) stimulated test for another 15 minutes. Protocol I was performed in 236 patients (99 men, 137 women, mean age 37.7 ± 18.9 years). In protocol II, passive phase was shortened to 30 minutes followed by 15 minutes NTG stimulated test. It was performed in 116 patients (38 men, 78 women, mean age 35.7 ± 18.9 years). Protocol III - Italian...

The prevalence of hepatitis C virus infection (HCV) in the Czech Republic is 0.2 %. On the world scale, between 180 and 200 million of persons are infected. The standard therapeutic procedure for HCV infection indicated for antiviral therapy is the combination of α pegylated interferon (PEG-IFN) and the ribavirin virostatic (RBV). Treatment is indicated for a period between 24 and 48 weeks, depending on the input parameters before the start of the treatment (especially the HCV genotype and viremia). The above virological parameters and the extent of liver damage are the most important factors of prognosis for therapy. Constant virological response...

Regression equations for QT and QTc intervals of the electrocardiogram
©. Kujaník
Vnitr Lek 2005, 51(11):1277-1288
One of the parameters of the electrical stability of the heart is also the QT interval (electrical systole) duration. It is dependent on the heart rate, during tachycardia is shortening, during bradycardia prolonging. However, its borders on the ECG are not very precisely defined, especially the end of T wave. For QT interval duration the ventricular cells of type M are mainly important and they have the very long action potentials. Since 1920, when the first regression equation of QT interval was published, there were plenty of equations linear or nonlinear (parabolic, cubic, hyperbolic, exponential, logarithmic) proposed, the most frequently used...
New possibilities in the treatment of chronic thromboembolic pulmonary hypertension

Pulmonary arterial hypertension (PAH) has a multifactorial pathobiology. Vasoconstriction, remodeling of the pulmonary vessel wall, and thrombosis contribute to increased pulmonary vascular resistance in PAH. Prostanoids have played a prominent role in the treatment of PAH. They improves exercise capacity, hemodynamics, and survival in patients with idiopathic pulmonary arterial hypertension (IPAH). Several interventional and surgical approaches are available for treatment of PAH - atrial septostomy, lung or heart-lung transplantation. Pulmonary endarterectomy is the accepted method of treatment for patients with chronic thromboembolic pulmonary hypertension...
Type 1A diabetes mellitus and autoimmunity

Vnitr Lek 2005, 51(11):1297-1302
Type 1A diabetes mellitus presents the most frequent part of type 1 DM. This disease is caused by an autoimmune destruction of pancreatic β-cells. Genetic, autoimmune and environmental factors are responsible for the development of type 1 diabetes. The most important genes that determinate the development of the disease are genes of Major Histocompatibility Complex (MHC). There is evidence that viral infection acts as a triggering event of autoimmune reaction. The immune response to a microbial determinant would cross-react with host tissues and results in an activation and proliferation of T-lymphocytes that cause destruction of pancreatic β-cells...

First experiences with treatment of chronic hepatitis C with Pegintron and Rebetol in the Czech Republic
Jan ©perl
Vnitr Lek 2005, 51(11):1314-1318
Based on retrospectively collected data, the team evaluated results for treatment of the first hundred of patients treated for chronic hepatitis C with PegIntron and Rebetol in the Czech Republic between 2003 and 2005. The treatment resulted in sustained virological response (SVR) in 53 out of 100 patients (53.0%). The effect of treatment was highest for patients without previous treatment, with SVR ratio 34 versus 53 (62.2%). The effect of treatment was lower in patients who relapsed after previous treatment with conventional interferon-α or showed no response to treatment at all, with SVR ratio 14 versus 27 (51.9%), and 5 versus 20 (25.0%),...
