M Pleskot, P Parízek, M Hodac, L Haman, J Sťásek, P Cervinka, M Tauchman

Vnitr Lek 1999, 45(2):75-80

The objective of the investigation was to evaluate in patients with chronic ischaemic heart disease (IHD) and malignant ventricular tachyarrhythmia the asset of myocardial revascularization for improvemet of the electric instability of the ventricular myocardium and a subsequent outline of the tactics of antiarrhythmic treatment. The authors included in the group a total of 35 patients (30 men and 5 women), age 34-78 years (mean 61 +/- 11) with IHD (according to selective coronarography) with spontaneous ventricular fibrillation (18 sebjects) or persistent (above 30s) marked symptomatic ventricular tachycardia (17 subjects), after ruling out acute...

[Myocardial perfusion tomography imaging in patients without prior myocardial infarction--comparison between 201Tl, 99mTc-tetrofosmin and dual-isotope protocol (rest 201Tl/stress 99mTc-tetrofosmin)].

M Kamínek, M Myslivecek, M Skvarilová, V Husák, O Lang

Vnitr Lek 1999, 45(2):81-84

The authors compared retrospectively the diagnostic accuracy of three protocols--201Tl stress-redistribution, 2-day 99mTc-tetrofosmin and the dual-isotope protocol (rest 201Tl/stress99m Tc-tetrofosmin)--for the detection of ischaemic heart disease (IHD) in a total of 115 patients without previous myocardial infarction (IM). 201Tl protocol (group A) was used to examine 43 patients, incl. 15 women, mean age 51 years (38-69 years). The 2-day 99m Tc-tetrofosmin protocol was used to examine 39 patients incl. 12 women, mean age 49 years (30-71 years). The dual-isotope protocol was used in 33 patients, incl. 8 women, mean age 52 years (31-69 years). In all...

[Is gastric carditis associated with antral gastritis in patients with non-ulcer dyspesia and Helicobatcer pylori infection?].

J Kyzeková

Vnitr Lek 1999, 45(2):85-89

UNLABELLED: Helicobacter pylori is the most frequent cause of antral gastritis type B. Little is known about the affection of the gastric cardia by inflammation associated with H. pylori infection.OBJECTIVE: Assess the status of morphological changes of the cardia in the presence of H. pylori infection in patients with non-ulcer dyspesia.METHODS AND PATIENTS: 251 consecutive dyspeptic patients with H.pylori infection were subjected to endoscopic examination of the upper part of the gastrointestinal tract. Six biopsies were taken (2 antrum, 2, corpus, 2 cardia) for histological (HaE) and microscopic (Giemsa) status, scored according to the Sydney...

[Effect of eradication treatment on histological changes of the gastric mucosa in patients with non-ulcer dyspesia and Helicobacter pylori infection. Randomized prospective study].

J Kyzeková

Vnitr Lek 1999, 45(2):90-96

UNLABELLED: Eradication treatment leads to improvement of gastritis. There is little information of its effect on changes in the topographic distribution and regression of actual mucosal alterations in dyspeptic patients. In a previous study we provided evidence of a complex of histopathological changes in the cardia, corpus and antrum in patients with H. pylori infection and non-ulcer dyspesia.OBJECTIVE: To assess the effect of eradication treatment on the development of histological changes in the lower oesophagus, in the gastric cardia, corpus and antrum in patients with non-ulcer dyspesia three and six months after treatment.METHODS AND...

[Latent autoimmune (Type-1) diabetes mellitus in adults. Part. I. Serologic markers of autoimmune involvement of pancreatic beta-cells: GADA, ICA, IA-2 a IA-A].

E Martinka, A Ocenásová, J Straková, M Mokán

Vnitr Lek 1999, 45(2):97-102

AIM OF STUDY: To assess the prevalence of markers of autoimmune destruction of pancreatic beta-cells (AIDbeta) in patients classified initially as Type-2 diabetes mellitus (Type-2 DM).SUBJECTS: 250 patients subdivided according to the: 1. BMI and C-peptide, 2. type of treatment. Measured parameters: age, BMI, C-peptide, autoantibodies directed against: glutamic acid decarboxylase (GADA), islet cells (ICA), thyrosinphosphatase (IA-2) and insulin (IA-A).RESULTS: GADA (and other AIDbeta markers) positivity varied from < 5% in patients with overweight/obesity (> 27 kg.m(-2)) and normal/increased C-peptide (> 0.32 nmol/l) to > 30% in...

[Latent autoimmune (type I) diabetes mellitus in adults. Part. II. Association of HLA antigens, status of cellular immunity and occurrence of other autoimmune diseases].

E Martinka, I Shawkatová, J Straková, M Buc, M Mokán

Vnitr Lek 1999, 45(2):103-109

AIM OF STUDY: To assess some immunological and immunogenetic aspects in patients with latent autoimmune (Type-1) diabetes mellitus (DM) of adults (LADA).SUBJECTS: 24 patients with LADA, 11 patients with Type-2 DM and 20 healthy volunteers (Pilot study). PARAMETERS TESTED: HLA-DRB1* and HLA-DQB1* alleles, parameters of cellular immunity (CD4+, CD8+, CD3/HLA-DR+, CD8/HLA-DR+, CD45RA+[CD4], CD16+CD56), CD19+, IL-4, INF-gamma and organ specific (OSA) autoantibodies (against thyroid gland, gastric parietal cells, tubuli, basal membranes of glomerulus, AMA and ABBA).RESULTS AND DISCUSSION: Type-1 DM HLA-DRB1* and HLA-DQB1* risk alleles have been...

[Use of proinsulin analysis for estimates of insulin resistance].

D Stejskal, R Pastorková, J Bartek, V Růzicka, D Horalík

Vnitr Lek 1999, 45(2):110-114

INTRODUCTION: According to contemporary estimates diabetes is present in 120 million subjects. This disease is associated with the incidence of a number of very serious organ complications and very frequently is diagnosed late (several years after its development). Because despite increased diagnostic and therapeutic efforts the number of diabetic patients is increasing, new diagnostic and therapeutic means are sought. Evidence was provided that some complications of diabetes develop not only in case of poor compensation but also in hyperinsulinaemia (hypertension, ischaemic heart and coronary artery disease etc.). In clinical practice it is however...

[The POEMS syndrome: a case report].

M Doubek, A Krivanová, Z Adam, I Niedermayerová

Vnitr Lek 1999, 45(2):115-117

The POEMS syndrome is a synopsis of different symptoms such as polyneuropathy, organomegaly, endocrine disturbances, M-protein and skin changes. POEMS syndrome has a complete and incomplete form. The leading symptoms are neuropathy and the monoclonal gammopathy. The authors report about a 55 year old lady who fell ill with this disease.

[Solitary cardiac metastasis from carcinoma of the cervix].

J Ziembová, J Schildberger, R Dvorák, I Pohanka, B Semrád

Vnitr Lek 1999, 45(2):118-121

Authors report the case of 37 years old women after radiotherapy for spinocelular cervical carcinoma which was presented by a solitary metastasis in the right ventricle causing right ventricle output tract obstruction. Cardiosurgical intervention as well as chemotherapy were not indicated for the great extension of cardiac involvement. The patient died from a massive pulmonary embolism. Echocardiography, although limited by the discrimination ability, seems to be the best method for the cardiac metastasis detection.

[Anti-angina treatment in stable forms of angina pectoris with emphasis on nitrates].

J Murín

Vnitr Lek 1999, 45(2):122-126

Current evidence strongly suggests that coronary atherosclerosis is a common denominator in patients with stable effort angina pectoris. The concept of pathophysiology of coronary atherosclerosis is presented--angiographic and pathologic evidence now suggest presence of eccentric and irregular atherosclerotic lesions (sometimes associated with plaque rupture) and simultaneously present endothelial dysfunction increases sensitivity of vascular smooth muscles to physical and biochemical stimuli with propensity to spasm. Ischemia is due to an increased myocardial oxygen demand (increased heart rate or blood pressure) that cannot be met because of fixed...

[New perspectives of treatment of osteoporosis with thiazides].

K Stefiková, V Spustová, R Dzúrik

Vnitr Lek 1999, 45(2):127-130

Thiazides inhibit calcium and magnesium excretion, the basic substrates of bone mineralization. Clinical investigations pointed already previously to their effectiveness in the prevention and treatment of osteoporosis despite a relatively small effect on the balance. This aroused some distrust and prevented their acceptance among preparations with this indication. They were indicated mainly in patients with hypercalciuria. The results of recent research indicate however their direct inhibition of osteoclasts and in particular stimulation of osteoblasts. This confirms results of clinical investigations which could not be explained previously. It may...
