[Renal excretion of amino acids in patients on a regular dialysis program].

In 10 patients with chronic renal failure and a polyfructosan S clearance (CPFS): 1.91-12.70 ml/min (mean 5.68 ml/min) with a preserved residual diuresis (more than 1000 ml/24 hours) renal excretion of 22 amino acids by residual nephrons was investigated before and 12 hours after haemodialysis. It was revealed that 12 hours after haemodialysis a significant drop of the filtered amount of all investigated amino acids occurred. [Protective effect of carvedilol on renal circulation and tissue injury in reperfusion of ischemic kidneys in rats].

In one-kidney rats the resting blood flow in the renal artery was recorded for 10 minutes then the artery was clipped for one hour and the following reperfusion blood flow was registered again for 10 minutes. The animals were divided into three groups: Series A, untreated control animals. Series B, two weeks before renal ischemia the rats were fed a diet containing 0.1% of BM-14.190, a derivate of carvedilol. Series C the rats received a diet with 0.1% of carvedilol. [Success of pharmacologic cardioversion and electric cardioversion in patients with atrial fibrillation or flutter in a 5-year retrospective study].

The authors evaluated in a five-year retrospective study the success of pharmacological and electric cardioversion in general in 353 patients with atrial fibrillation and in 117 patients with atrial flutter. The success of electric cardioversion in the group with fibrillation arrhythmia was 72%, in the group with atrial flutter 92%. The success of pharmacological cardioversion in the group with fibrillation arrhythmia was 96.2%, in the group with atrial flutter 79.8%. In pharmacological cardioversion in both groups the combination quinidine-isoptin was best.

In four unrelated families of Czech and Slovak origin two nonsense dominant beta-thalassaemic alleles (CD 121 (G-T); CD 112 (T-A)) and in one family simple substitution in codon 115 (GCC-GAC) or alpha 2 beta 2 115 (G17) Ala-Asp HB-Hradec Králové were identified. [Extrapulmonary tuberculosis with involvement of the reticuloendothelial system and secondary lupus vulgaris].

The authors describe a case of extrapulmonary tuberculosis with affection of the reticuloendothelial system in a 59-year-old female patient. After successful treatment the findings in the spleen, bone marrow and the laboratory results had normal levels. The authors draw attention to the diagnostic difficulties. They were able to confirm the diagnosis only by histological examination of biopsy from the pelvic bone. An interesting finding was eruption of lupus vulgaris on the legs in the course of treatment. The authors discuss some aspects of the disease and its differential diagnosis.

At the First Medical Clinic of the Faculty Hospital Prague Motol the authors detected during a routine gastroscopic examination of a 71-year-old female patient the presence of a gastric bezoar. Diabetic patients have a 2 to 4 times higher risk of development of atherosclerosis than non-diabetic subjects. One of the risk factors of atherosclerosis is an impaired lipid and lipoprotein metabolism which is influenced by the type of diabetes, the degree of its metabolic compensation, character of treatment and other concurrently present metabolic abnormalities. In metabolically balanced type 1 diabetes the levels of commonly assessed lipoproteins do not differ from those in non-diabetic subjects, the HDL-cholesterol level can be even higher. In follicular lymphomas with a low malignancy combined treatment (interferon alpha and chemotherapy) achieves the same number of remissions as chemotherapy alone, remissions after combined treatment are longer. Maintenance treatment with interferon alpha prolongs the remissions period. Maintenance treatment with interferon alpha can be recommended as standard treatment. For combined initial treatment it is important to define the optimal time schedule and dosage of interferon. Interferon alpha holds its place in the treatment of skin lymphomas where it leads in a great proportion of lymphomas to remission, and in angioimmunoblastic lymphadenopathy....
