Background:Congestive heart failure with reduced ejection fraction is a common clinical condition with a serious prognosis. Non-celiac gluten/wheat sensitivity (NCG/WS) is a syndrome characterized by intestinal and extra-intestinal symptoms elicited by gluten/wheat ingestion in the persons, who are not affected by celiac disease and wheat allergy. Due to the lack of established biomarkers, the diagnosis of NCG/WS is based on the clinical picture, and it is necessary to validate it in well-defined double-blind, placebo-controlled challenge (The Salerno Experts' Criteria). In the article is discussed a current knowledge of NCG/WS in terms of pathogenesis, diagnosis and treatment.
Diagnostic pitfalls of celiac disease

Celiac disease is a very common autoimmune disorder caused by the ingestion of dietary gluten products in genetically susceptible persons. Its global prevalence is estimated around 1 %. However, the most cases are not diagnosed. Clinical presentation is widely variable with the involvement of various human systems. Besides a clinical picture (that is often non characteristic), the diagnosis is based on positivity of serological testing (tissue transglutaminase autoantibodies) and histological evaluation of small intestinal mucosa. The article presents a rational diagnostic approach to celiac disease.

A man aged 60 years was examined for intense inflammatory response, night sweats, subfebrile and later febrile temperatures and a weight loss of 18 kg in 7 months. CRP was 270 mg/l, i.e. more than 20 times the upper limit of the physiological range. Reactive leukocytosis (10 × 109/l), thrombocytosis (530 × 109/l), increased fibrinogen (greater than 7 g/l), and anemia with hemoglobin of 80 g/l were present. Relation between oncological diseases and venous thrombo-embolism (VTE) is well known for almost 2 centuries. In 1823 Bouillaud assumed by three patients with tumor and recent deep vein thrombosis (DVT), that peripheral edema of lower limbs emerges as a result of "obturation" of veins by "fibrinous coagulum" (caillot fibrineux), which was induced by oncological disease. French physician Armand Trousseau wrote about this relation in his book "Phlegmasia alba dolens" again in the year 1865. Acromegaly is a rare disorder usually caused by a benign tumour of the pituitary gland. Long-term presence of elevated growth hormone (GH) and insulin like growth factor 1 (IGF1) levels accompanying this disease is associated with complications such as cardiomyopathy, diabetes mellitus, sleep apnoea and arthropathy.
