What are the risk factors of ischaemic cardiac disease (ICD) knowledge at our physicians? The comparison with the condition in 1991

J. Widimský, T. Pecka

Vnitr Lek 2005, 51(4):397-402

In 1991 we investigated the knowledge about risk factors of ischaemic cardiac disease at our physicians. The objective of study was to find out changes of the last 12 years. Although the cardiovascular mortality has been decreasing within last 10 years, it stays to be the most often reason of dying in Czech Republic. In 2003 we used the questionnaire about risk factors of ICD for 470 physicians who were taking part at the courses of Institute of postgradual studies in medicine (IPVZ), Prague. The physicians of age 31-40 and younger represented majority of the group. If we compare situation in 1991 with our study, there is the noticeable decline in...

The epidemiological analysis of patients with permanent cardiostimulation at the Department of Cardiology in Faculty Hospital Brno-Bohunice

J. Vlašínová, M. Kozák

Vnitr Lek 2005, 51(4):403-408

The objective of the study:To give a summary of patients structure who are cured by the permanent cardiostimulation in the Departement of cardiology within the period of 10 years. The method:We divided the patients who were indicated to the pacemaker implantation in the Departement of cardiology in years 1992-2002 - according to the age, gender and indicatory diagnosis. Results:The analysis of 2 772 patients was completed. In the total sample there were more men than women, in all age groups. In the youngest age group there are even more men than double amount of women. Among the women there is the higher representation...

Clinical importance of moderate elevation of alfa-fetoprotein levels in serum in chronic hepatitis C and B patients

J. Stránský, H. Kopřivová, M. Hořejšová, J. Stříteský

Vnitr Lek 2005, 51(4):409-413

Alfa-fetoprotein (AFP) is a fetal serum glykoprotein, which is mostly found in hepatocelular carcinoma but its moderate elevated level was evidenced also in acute and chronic viral hepatitis. The authors evaluated the clinical importance of alfa-fetoprotein moderate elevated levels in serum of 125 adults with chronic hepatitis B (N = 40) and C (N = 85). Morphological finding in the liver was verified by liver biopsy in 75%. Results:An average score of histological activity index in anti-HCV positive individuals with cirrhosis was higher than in chronic hepatitis (p < 0.05), and correlated with AFP concentration and AST activity in...

V. Chaloupka, L. Elbl, S. Nehyba, I. Tomášková

Vnitr Lek 2005, 51(4):414-420

The goal of the study:The main goal of our work is to appraise the influence of the physiotherapy to the change of the stress tolerance at older patients, to evaluate their total and cardiovascular mortality and the occurrence of heart complications (reinfarction, revascularization) in the long-term monitoring. Patients and methods:There are 582 patients in the sample, 89% men and 11% women, at the average age of 56 ± 11. We divided the sample into the groups of patients >65 years and younger than 65 years. The first group is covered by 145 persons (25%) at the average age of 69 ± 2.7 (65-78). In the group...

The influence of the combined aerobic training to the changes of autonomous modulation at the ill patients after the acute myocardial infarction

L. Elbl, V. Chaloupka, S. Nehyba, I. Tomášková, F. Jedlička, P. Kala, J. Schildbeger

Vnitr Lek 2005, 51(4):421-429

Objective of the study:The autonomous dysfunction is present at the most ill patients after the acute heart attack and is defined by the changes of the sympato-vagální balance that is indicated by the decrease of the variability of the heart frequency (HRV) and the reduced baroreflex sensitivity. The presented work concentrated on the evaluation of the effect of the combined aerobic training and the strength exercise for the changes of the modulation of the autonomous nerve system in the short run by the evaluation of the 8week physiotherapy programme and in the long run of the 12 months after the finished physiotherapy. The patients...

The pleural effusion in general internal practice

M. Teřl, M. Pešek, A. Tauchman

Vnitr Lek 2005, 51(4):430-437

The pleural effusion is a frequent problem of internist's medical practice. It appears with as many as 10% of patients with internal diseases. The objective of this paper is the provision of a survey of a current, clinically significant knowledge concerning pathogenesis of the pleural effusion and its diagnostic possibilities. It resumes the survey of the most important causes and types of pleural effusions and summarizes the existing therapeutical possibilities. Its conclusion is that the base of a rational approach to patients with a pleural effusion is a proper determination of its causes, where a consistent analysis of pleural liquid still remains...

Molecular pathophysiology of late diabetic complications - genetic predisposition to the development of diabetic complications

K. Kaňková

Vnitr Lek 2005, 51(4):438-449

Our contribution deals with a subject of genetic determination of susceptibility to the development of late diabetic complications, ways and means of its study and potential clinical application of this piece of knowledge for assessing of individual's risk. Existence of genetic risk factors for diabetic complications is supported by (i) data regarding the relationship between prevalence and incidence of complications on one hand and diabetes duration and compensation on the other, (ii) frequent proof of familiar aggregation and (iii) results of segregation analyses. Aforementioned facts apply almost exclusively to the diabetic nephropathy and macrovascular...

The transplantation of the stem cells in cardiology

R. Panovský, V. Kincl, J. Meluzín

Vnitr Lek 2005, 51(4):450-456

The transplantation of stem cells for the patients with cardiac diseases is relatively new method. The using of autological mononuclear cells of bone marrow appears as a brightest possibility. After application of the cells into myocardium man can expect an improvement of a left chamber myocardial function and perfusion - by differentiation of implanted cells parts into myocytes and new capillaries making cells. Besides an improvement of eject fraction we can see the positive effect of the bone marrow cells transplantation to the remodelation of left chamber with a thicker side of scar, an improvement of scar elasticity and a prevention of chamber...

Aortic stenosis, sclerosis, and atherosclerosis

K. Linhartová, R. Čerbák, H. Rosolová

Vnitr Lek 2005, 51(4):458-461

Only in the last decade calcific aortic valve disease has been proven to belong to the family of atherosclerosis. Calcific aortic valve disease encompasses a wide range of disease from calcification without flow obstruction, aortic sclerosis, to severe aortic stenosis. Similarity with the atherosclerosis is suggested by histological studies, association with atherosclerosis risk factors and frequent coincidence with coronary artery disease. Possible association with altered calcium metabolism, endothelial dysfunction, inflammatory and infectious etiology as well as genetical factors is being studied. A new insight into the problem of the aortic valve...

Vnitr Lek 2005, 51(4):462-470

After "metabolic prologue" the authors describe 4 cases of the patients at whom the high level of homocystein in serum was detected within hospitalisation at intensive care unit of Hospital Na Homolce. There are the cases of typical homocysteinurie with hereditary deficit of cystathionin-beta-synthase enzyme recognized only after a passed myocardial infarct. The next there were a myocardial infarct within suspect pernicious anaemia, an acute pulmonary embolism with till the time not diagnosed pernicious anaemia and finally a deep flebothrombosis with diagnosed pernicious anaemia but insufficiently treating for allergic reaction. The important homocysteinemie,...

Courses on ethics in the faculty of medicine: A route from descriptive ethics to moral norms

M. Vácha

Vnitr Lek 2005, 51(4):477-478

The paper discusses a topic of lecturing a philosophical discipline to medical students that are oriented towards a science in the broad context of the position of medicine as a borderline discipline between science and arts. Both traditional ethical problems regarding to conscience, liberty and dignity of human beings and new questions asked by contemporary biological medicine are reflected. To solve the latter group of the problems seems to be much more complicated especially because it is not possible to bank on the reflections of philosophers of past decades or centuries. In the final part of the paper there is a brief review of the activity of...
