The pleural effusion is a frequent problem of internist's medical practice. It appears with as many as 10% of patients with internal diseases. The objective of this paper is the provision of a survey of a current, clinically significant knowledge concerning pathogenesis of the pleural effusion and its diagnostic possibilities. It resumes the survey of the most important causes and types of pleural effusions and summarizes the existing therapeutical possibilities. Its conclusion is that the base of a rational approach to patients with a pleural effusion is a proper determination of its causes, where a consistent analysis of pleural liquid still remains the essential instrument. In case that the focused treatment of the cause(s) of pleural effusions fails, nowadays, there are available, besides the classical surgical treatment, possibilities of a local mini-invasive treatment with the help of fibrinolysis and pleurodesis.
