[Comparison of dynamic and isometric loading in echocardiographic examinations in patients with ischemic heart disease].

[Dobutamine stress echocardiography in patients with acute myocardial infarct].

The objective of the present work was to evaluate dobutamine echocardiography in patients after myocardial infarction. The group of patients comprises 39 subjects, age 54 +/- 10 years with a first non-complicated myocardial infarction treated by thrombolysis. The patients were examined on the 5th-7th day with small amounts of dobutamine (5 and 10 micrograms/kg/min.) and a maximal test after two weeks and three months after the myocardial infarction. [Changes in platelet activity in invasive cardiologic procedures].

The authors revealed some changes in the platelet activity in patients with invasive cardiological procedures. Changes of the platelet function were manifested by an enhanced aggregation of platelets in vivo, an increased secretion from alpha granules and increased release of prostaglandin metabolites from platelets and from the vascular wall. The authors assessed in blood some indicators of the reaction of the organism to the formation and action of free radicals in patients with rheumatoid arthritis, with other inflammatory articular diseases selected at random and with degenerative diseases of the joints and spine. The presence of hepatitis C virus (HCV) was investigated in 20 agammaglobulinemic patients using polymerase chain reaction (PCR) and by specific anti-HCV antibodies detection. The authors present their results assembled in patients with chronic myeloid leukaemia (CML) to whom they administered interferon alpha (IFN) in the chronic stage of the disease. From the literature ensues that in 1995 we cannot expect that in the Czech Republic the ratio of smoking in the mortality from lung cancer, chronic obstructive pulmonary diseases and cardiovascular diseases will decline, as compared with 1990. The authors describe an uncommon and severe intoxication with the mushroom Lepiota Helveolla Bres. which manifested itself by fulminant hepatic failure. The possibility of urgent transplantation of the liver was considered. After conservative treatment the patient recovered, however, completely. In the conclusion the authors' view is discussed as regards indications of urgent transplantation of the liver in intoxications with mushrooms which may cause fulminant hepatic failure.

A new era of multiple myeloma management includes new approaches such as genetic marking and gene therapy, immunotherapy, stimulation of graft versus myeloma effect, etc. Intensive clinical research is also focused on the detection of minimal residual disease and bone marrow purging. Clinical trials have begun and the first results are promising, though preliminary. The combinations of new types of therapeutical approaches with the most efficient regimens used until now could form a new type of MM management strategy that better involves minimal residual disease.

Erythropoietin can be successfully used in the treatment of anaemia induced by chemotherapy and radiotherapy as well as for the treatment of anaemia induced by malignant disease without previous chemotherapy of radiotherapy. Erythropoietin administered during chemotherapy is effective in 50-70% patients, it has a more marked effect as a supplement to chemotherapy with cisplatinum and carboplatinum than non-platinum regimens. Diabetes mellitus type 1 (DM type 1) is a chronic, organ specific autoimmune disease. Autoantibodies against the islets of Langerhans (ICA) are found in 71%, against insulin (IA) in 65% against glutamate decarboxylase (GAD) in 67-77% of children with DM type 1; in their close relatives and in patients with a more prolonged persistence of DM type 1 these antibodies and manifestations of cell-mediated autoreactivity are found less frequently also as a sign of the autoimmune character of DM type 1.
