A present view on ivabradine in the therapy of cardiovascular diseases

Jiří Slíva

Vnitr Lek 2017, 63(5):307-310 | DOI: 10.36290/vnl.2017.063

Cardiovascular illnesses belong to diseases with the highest prevalence. They are also the most predominant reason of death in the vast majority of European countries. Hence, the offer of effective treatment of these dis-eases is absolutely crucial. This text focuses on the current therapeutical use of ivabradine in its approved indications - chronic stable angina pectoris and chronic heart failure - in the context of so far published relevant clinical trials.

Osteoprotective therapy with bisphosphonates or denosumab in patients with multiple myeloma: benefit and risks

Zdeněk Adam, Jan Straub, Marta Krejčí, Luděk Pour, Dagmar Brančíková, Lenka Ostřížková, Viera Sandecká, Martin Štork

Vnitr Lek 2017, 63(5):311-321 | DOI: 10.36290/vnl.2017.064

Bisphosphonates have been used during the complete treatment of multiple myeloma for more than twenty years. They slow osteolysis and thereby contribute to the improvement of quality of life. Their long-term use, however, is related to 2 serious, usually later appearing complications: osteonecrosis of the jaw, occurring in 6-9 % of patients, and rarer atypical bone fractures. Both these complications are very difficult to heal, and all the more emphasis is therefore laid on prevention. This first of all includes discussion about the risk with the patient, followed by a dental checkup before the commencement of therapy and then repeated during its course,...

Thoughts about valvular abnormalities yesterday and today

Pavel Gregor, Hana Línková

Vnitr Lek 2017, 63(5):322-327 | DOI: 10.36290/vnl.2017.065

The article deals with the problems of acquired valvular disorders. It mainly focuses on the new findings regarding ethiopathogenesis, diagnostics, evaluation and treatment, especially catheterization. In advanced countries, post-rheumatic abnormalities have almost disappeared, their current etiology has substantially changed (mitral stenosis). The earlier relatively rare degenerative changes now predominate, mainly affecting older patients (aortic stenosis). Also the treatment methods for acquired abnormalities have substantially changed - currently catheterization approaches are used more frequently in their correction.

What has the GLAGOV clinical study shown?

Ján Murín

Vnitr Lek 2017, 63(5):329-332 | DOI: 10.36290/vnl.2017.066

If we want to favorably influence the cardiovascular atherosclerotic disease, it is necessary to significantly lower LDL-C blood levels. PCSK9 inhibitors can significantly reduce LDL-cholesterol levels, being administered together with statins. However we focus on establishing whether this plays a role in influencing coronary atherosclerosis. The GLAGOV study is a multicentric, double-blinded and placebo-controlled study which during a 76-week period followed an impact of the treatment with evolocumab (PCSK9i) together with statin on the status of coronary atherosclerosis in patients with ischemic heart disease, and in comparison with the statin treatment....

News in the treatment of hypertension and dyslipidemia

Jan Václavík

Vnitr Lek 2017, 63(5):333-337 | DOI: 10.36290/vnl.2017.067

These review article presents the results of major recent studies in cardiovascular prevention. HOPE-3 study showed that people with medium cardiovascular risk who have not experienced a cardiovascular event, and have normal blood pressure, benefit from deploying a statin, and in the presence of hypertension benefit from deploying a combination of a statin and antihypertensive drugs. Studies evaluating genetic predisposition brought new evidence that both LDL cholesterol and systolic blood pressure are independent and multiplicative causal risk factors for cardiovascular events. A mild long-term reduction in blood pressure and LDL cholesterol has the...

Gastric antral vascular ectasia and solitary rectal ulcer syndrome - two rare diagnoses as the cause of anemia in a single patient: case report

Lumir Kunovsky, Milan Dastych, Radek Kroupa, Beata Hemmelova, Katarina Muckova, Miroslava Chovancova, Lenka Kucerova, Jiri Dolina

Vnitr Lek 2017, 63(5):339-342 | DOI: 10.36290/vnl.2017.068

Gastric antral vascular ectasia (GAVE) and solitary rectal ulcer syndrome (SRUS) are both mentioned in the literature as rare causes of iron deficiency anemia and gastrointestinal (GI) bleeding. GAVE accounts for up to 4 % of upper non-variceal GI bleeding; SRUS is a rare benign disorder that presents with rectal bleeding. We present the case of a 75-year-old patient who was admitted to our facility with anemia. In the same patient, we encountered chronic bleeding from GAVE and SRUS. Both diagnoses were treated endoscopically: GAVE by argon plasma coagulation and a subsequent treatment with proton pump inhibitors and SRUS by adrenaline injection and...

Štěpán Hrabovský, František Folber, Barbora Jurová, Zdeněk Řehák, Michael Doubek

Vnitr Lek 2017, 63(5):348-353 | DOI: 10.36290/vnl.2017.070

Though acute lymphoblastic leukemia (ALL) is the most common malignancy of childhood age, it is a rare diagnosis in adults. This disease often manifests with common and nonspecific symptoms, so it can easily escape an early diagnostics without a proper blood count examination. We present a case of an adult ALL patient suffering only from severe hips and thighs pain, without any significant blood count abnormities leading to the diagnostics. In the second part of the article, we summarize current highlights regarding this disease.

Kristián Brat, Zdeněk Merta, Ivan Čundrle

Vnitr Lek 2017, 63(5):354-360 | DOI: 10.36290/vnl.2017.071

This case report focuses on an immigrant admitted to the Department of Respiratory Diseases, University Hospital Brno due to suspicion of relapsing intestinal tuberculosis. The patient presented with fever, night sweat, weight loss, diarrhea, and a history of several tuberculosis attacks in the last few years. None of the examinations confirmed the presence of active tuberculosis but raised suspicion of hematological malignancy. Pancytopenia was present in the peripheral blood. However, bone marrow examination and flowcytometry excluded the presence of a hematological malignancy. The results pointed to the possibility of vitamin B12 or folate...
