Introduction:The incidence of well-differentiated low-risk thyroid cancer have increased globally over the last three decades. Thyroid cancer treatment relates to a suitable surgical procedure and the use of adjuvant radioiodine therapy in selected patients. Evaluation of prognostic factors and risk stratification are critical for determining appropriate treatment. Survival of patients with low-risk thyroid cancer is excellent. Appropriate choice of medical treatment resulted in full recovery in most patients. Relapse risk increases with the size of the primary tumor, along with the findings of the risk factors in men. Methods and...

Vnitr Lek 2015, 61(9):778-784
Introduction:Systemic lupus erythematosus (SLE) is a chronic autoimmune multisystem disease. The aim of our study was to clarify the frequency of decreased serum immunoglobulin levels in SLE patients. There were evaluated 799 results of serum immunoglobulin levels gained from 157 patients fulfilling revised ACR criteria in the retrospective study. Results:The immunoglobulin levels under the normal range were found in 29/157 (18.5 %) patients. The most frequent was isolated reduction of IgG 12/157 (7.6 %), two persons fulfilled criteria for selective IgA deficiency, and one case possible diagnosis of common variable...

Vnitr Lek 2015, 61(9):785-791
Introduction:Treatment of chronic myeloid leukemia (CML) with tyrosine kinase inhibitors (TKI) has significantly changed the survival of patients of all age categories. In our work, we have tried retrospectively to analyze a therapeutic response and toxicity of imatinib according to age of patients (≤ 60 years vs > 60 years). Patients have started treatment with imatinib in 2001-2013 in our department. The duration of treatment was at least 18 months. Patients and methods:The whole group contains 103 patients (52 women, 51 men) with a median age at diagnosis of CML 55 years (range 19-88 years). The first group includes...

Goal:Metabolic syndrome and its components play an important part in the development of not only cardiovascular conditions, but also digestive and pancreaticobiliary system diseases. The aim of our study is to present a comprehensive overview of the diseases where metabolic syndrome is an inducing risk factor, or where it affects the course of the disease. Results:Metabolic syndrome is a significant risk factor of induction of gastroesophageal reflux and its complication, which is Barrett's esophagus. Metabolic syndrome was described as the disease closely linked to idiopathic intestinal inflammations, diseases of the biliary...
Usefulness of vena cava filters from clinicians view
Otto Mayer, Zdenka Poklopová
Vnitr Lek 2015, 61(9):799-804
The indication for vena cava filters (VCF) in treatment of venous thromboembolism is still controversial. The presented overview should support the practical decisions. Beside of large volume of observational date there is only one study - PREPIC 1 - fulfilling requirements of prospective randomized design. During 8 years of follow up, pulmonary embolism (PE) was less frequent in the group with VCF than in the group without VCF, but at the cost of more frequent deep vein thrombosis (DVT). Other, but observational studies, showed similar results. Since last 10 years retrievable VCF are available. PREPIC 2 study was settled to prove, if use of retrievable...
Therapeutic potential of micronized purified flavonoid fraction (MPFF) of diosmin and hesperidin in treatment chronic venous disorder
Lukáš Hnátek
Vnitr Lek 2015, 61(9):807-814
Chronic venous disorder (CVD) is a common illness with high incidence existing especially in Europe and North America. The main goal of micronized purified flavonoid fraction (MPFF) of diosmin and hesperidin is to eliminate the symptoms of CVD (venous pain, fatigue, etc). But MPFF of diosmin and hesperidin has good effectiveness for treatment of venous oedema and venous ulcer too. There are many papers that prove its effectiveness in the experiment and in the microcirculation too. The other indications for MPFF of diosmin hesperidin is hemorrhoidal disease and the accessory treatment of lymphedema. It is proved that this substance could be used as...
Dietary changes in relationship to risk factors and coronary heart disease mortality
Jiří Brát, Michal Vrablík, Otto Herber
Vnitr Lek 2015, 61(9):815-820
Europe is a region, which is most severely affected by non-communicable diseases. Cardiovascular disease is the leading cause of disability and death. Complex interactions between diet, lifestyle, and lipoprotein metabolism significantly contribute to the development of atherosclerosis and its complications. Role of diet in prevention of coronary heart disease becomes sometime underestimated in comparison with pharmacological treatment. Coronary heart disease (CHD) mortality has declined substantially in the Czech Republic in the last decade of the 20th century. Significant and positive dietary changes, mainly reduction of saturated fatty...

In rare cases, monoclonal immunoglobulins bind to intrinsic antigens whereby causing autoimmune damage to the body. The text describes Waldenström's macroglobulinemia which was manifested by muscle pain and anemia. The female patient had suffered from back pain for about 3 years before she came to our clinic. In the last year pain in the muscles of the upper and lower extremities developed in addition to back pain. This led to the suspicion of polymyositis. However this was not confirmed by a special examination. The patient was diagnosed with clearly established infiltration of lymphoplasmacytic lymphoma and 10.8 g/l of type IgM monoclonal immunoglobulin...
