Chronic thromboembolic pulmonary hypertension in local hospital
M. Belicová
Vnitr Lek 2011, 57(2):140-146
Background:Chronic thromboembolic pulmonary hypertension is a serious consequence of pulmonary embolism associated with considerable morbidity and mortality. It develops in about 0.1-3.8% among patients who survive pulmonary embolism despite adecvate treatment. Objective:Retrospectively evaluate incidence, clinical picture and difficulties in diagnostics and treatement of patients, in which the diagnosis was confirmed. Patients and methods:In years 1996-2007 there were 33,108 patients hospitalized at 1. Internal Clinic of University hospital in Martin, 267 patients (134 women) had pulmonary embolism, 5 (2 women)...
Endocarditis 2007: Results of a multicentric study on occurence and characteristics of infective endocarditis
J. Beneš, R. Baloun, O. Džupová
Vnitr Lek 2011, 57(2):147-154
Aim:To recognize incidence and epidemiological characteristics of infective endocarditis (IE) in the Czech Republic. Methods:A prospective multicentre observational study on occurence and characteristics of IE in 29 hospitals within one year. A cooperative physician in every hospital reported all cases of IE that occured in pre-defined hospital attraction area in 12 consecutive months. A unified anonymous form was used for reporting. Only patients fulfilling the modified Duke criteria were enrolled. Results:One hundred and thirty-four cases of IE were reported. Thus, crude incidence of IE appeared 3.4 cases/100,000/year....
Relationship of the CDKAL1 and KCNQ1 gene polymorphisms to the age at diagnosis of type 2 diabetes in the Slovakian population

Vnitr Lek 2011, 57(2):155-158
Background/aims:The association of CDKAL1 and KCNQ1 genes with type 2 diabetes mellitus (DM2T) was confirmed by several genome-wide association studies in both Caucasian and Asian populations. For both genes, it is supposed that the risk of DM2T development is related to impaired insulin secretion. Based on assumption that the presence of risk allele might predispose to an earlier onset of DM2T, the aim of the present study was to assess the frequency of risk alleles of CDKAL1 rs7756992 and KCNQ1 rs163184 polymorphisms and to analyze their association with the age at DM2T diagnosis in the Slovakian population. Methods:CDKAL1...
Pancreatic cancer - association with diabetes mellitus and smoking

Introduction:Pancreatic cancer is a disease with rather poor prognosis. This can be explained, among other reasons, by unusually aggressive course of the tumour growth and, in the majority of cases, late, and thus further treatment limiting, diagnosis. In addition, no effective screening programme for pancreatic cancer is available and thus identification of risk factors associated with the development of pancreatic cancer represents a possible approach to diagnosing early stages of the disease. Smoking represents a general and diabetes mellitus a specific risk factor for pancreatic cancer. The aim of our prospective study in...

Alcoholic cardiomyopathy - still a topical diagnosis
P. Kyčina, J. Murín
Vnitr Lek 2011, 57(2):163-169
Even though extensive experimental as well as clinical evidence exists for adverse cardiac effects of alcohol, pathogenesis and incidence of alcoholic cardiomyopathy is not fully understood. From the genetic level and understanding of the pathogenesis of the toxic effects of alcohol on the heart we come to discuss the diagnosis and treatment of the disease that, in the context of co-morbidities and complications (hypertension, diabetes, arrhythmias, heart failure), continues to represent an significant clinical entity with an important impact on patient prognosis.
Evaluating of quality of life and functional status in patients with rheumatoid arthritis
A. Čalfová, I. Lazúrová, J. Benka, Ž. Macejová
Vnitr Lek 2011, 57(2):170-175
There is a growing worldwide interest for more information regarding morbidity and especially chronic diseases as well as the economical burden which it creates for the disabled individual and the society as a whole. Recently, increased attention has been paid to the scientific assessment of quality of life in chronically ill. Measures of physical and other abilities have shown a great prognostic value regarding disease outcomes. Rheumatoid arthritis (RA) is a chronic, progressive, systemic inflammatory disease which negatively influences quality of life of patients. In rheumatology quality of life is assessed with two types of instruments: generic...
Bicuspid aortic valve - ethiopathogenesis and natural history
M. Brtko, J. Vojáček, M. Tuna, P. Telekes
Vnitr Lek 2011, 57(2):176-182
Bicuspid aortic valve is a disease with autosomal dominant inheritance with small penetration. The population of patients with bicuspid aortic valve is heterogenous. In the absence of dilatation of ascending aorta, valvular lesion or degenerative changes of valve leaflets patients are considered to belong to the low risk population. On the other hand patients with the above mentioned characteristics compose a high risk group. Dilatation of the ascending aorta in patients with bicuspid aortic valve has a progressive feature and continues even after replacement of the aortic valve. With progression of aortic dilatation the risk of aortic dissection and...
The importance of and options available for screening of celiac disease
P. Makovický, K. Rimárová
Vnitr Lek 2011, 57(2):183-187
Even though guidelines for diagnosing celiac disease have been compiled and accepted by a number of expert associations, this disease continues to be under-diagnosed in children and, particularly, in adults. This is even though numerous scientific papers, short case studies and review papers have been published highlighting this issue and consensually supporting screening methods. In addition, research results by reputable Czech and Slovak authors suggest that, at present, only a small proportion of all cases of celiac disease are correctly diagnosed. Considering this, methods to diagnose the disease in its initial stage are continuously being sought....

Vnitr Lek 2011, 57(2):189-213
In 2009, the recommendations of the Czech Collaborative Group for Ph- Myeloproliferative Diseases (CZEMP) for diagnosis and treatment of BCR/ABL-negative myeloproliferative diseases (MPD), i.e. essential thrombocythemia (ET), polycythaemia vera (PV) and primary myelofibrosis (PMF) were updated and extended. The present article gives the rationale of the recommendations in full detail. The CZEMP diagnostic criteria for ET and PMF are based on histopathological (HP) findings, which must unconditionally be in line with the given clinical and laboratory characteristics of ET or of a certain stage of PMF, respectively. The platelet count is not decisive...

We describe a case of an untreated female patient monitored over 8 years for chronic B-lymphocytic leukaemia (B-CLL). Over the 8 years, the patient has gradually developed severe kidney failure, even though the criteria for B-CLL treatment had not been fulfilled. Kidney biopsy revealed renal damage due to λ free light chains cast nephropathy as well as an infiltration of renal parenchyma with B-CLL cells. It was not before this biopsy that the presence of monoclonal immunoglobulins has been investigated. Immunofixation identified free monoclonal λ light chains in the serum and urine. Their serum concentration, quantified by densitometry,...
