Introduction:The Atractiv project assessing the efficacy of a complex approach to cardiovascular risk reduction in primary care was conducted by 464 physicians in the entire Czech Republic between 2006 and 2007. Aim:The primary goals of the Atractiv project were description of prevalence of risk factors for cardiovascular disease (CVD) in high-risk patients and attempt to maximize the risk reduction by optimalization of treatment. National working group representing clinicians (hematologists, oncologists, infection diseases and ICU specialists), microbiologists, and different special medical societies and working groups prepared evidence-based guidelines for the treatment established fungal infection - invasive candidiasis in the adult hematology and ICU patients. These guidelines updated those published in the Czech Republic in 2003-2004. Evidence criteria of the Infectious Diseases Society of America (IDSA) were used for assessing the quality of clinical trials, and EORTC/MSG Consensus Group for definitions of invasive fungal disease.

Guidelines for the treatment of invasive aspergillosis

Z. Ráčil, J. Mayer, I. Kocmanová, J. Haber, N. Mallátová, M. Kouba, P. Sedláček, E. Faber, I. Herold, P. Múdry, L. Drgoňa, M. Karas, V. Buchta, J. Vydra, M. Kolář, J. Trupl, V. Marešová, H. Rozsypal, O. Nýč, K. Cwiertka

Vnitr Lek 2008, 54(12):1187-1194

An increasing incidence of invasive aspergillosis is observed in most immunocompromised patients, and especially patients with acute leukemia and after hematopoietic stem cell transplantation. In order to decrease the mortality due to this infection, the clinicians need to optimise their treatment choice. The objective of these guidelines is to summarize the current evidence for treatment of invasive aspergillosis. The recommendations have been developed by an expert panel following an evidence-based search of literature with regard to current recommendation of European Conference in Infections in Leukemia and Infectious Diseases Society of America.

Schnitzler syndrome - report on a fourteen-year course of the disease and an overview of information on the disease

Z. Adam, M. Krejčí, L. Pour, J. Neubauer, J. Prášek, R. Hájek

Vnitr Lek 2008, 54(12):1140-1153

Schnitzler syndrome is a rare disease characterised by chronic urticaria and the presence of monoclonal IgM immunoglobulin, and by other symptoms. We report our experience with 14-year treatment of a patient. The first medical examination in our workplace was at the beginning of 1995 and the patient was diagnosed with the disease in 1996 (at the age of 52). Antihistaminics, the first medication used to relieve the symptoms of urticaria, had no subjective or objective effect. After the detection of osteolytic-osteosclerotic changes in the pelvic region, in areas with intense pain, we started treatment with pamidronate (90 mg at 28-day intervals), and...

Icodextrine peritoneal dialysis solution in clinical practice

S. Opatrná

Vnitr Lek 2008, 54(12):1155-1160

Icodextrin, a glucose polymer, is an alternative osmotic agent to glucose in peritoneal dialysis solutions. Icodextrin generates ultrafiltration through colloid osmosis and is thus effective even during long-term (e. g., nighttime) dwells and in cases of high peritoneal permeability, where it prevents dialysate reabsorption into the systemic circulation. Ultrafiltration is maintained even in the presence of peritonitis. The incidence of bacterial peritonitis is not different when using icodextrin- or glucose-based solutions. Some time ago, icodextrin use was implicated in an increased incidence of sterile peritonitis. This was due to contamination...

New evidence-based criteria for evaluating the appropriateness of drug regimen in seniors. Criteria STOPP (Screening Tool of Older Person's Prescriptions) and START (Screening Tool to Alert Doctors to Right Treatment)

E. Topinková, P. Mádlová, D. Fialová, J. Klán

Vnitr Lek 2008, 54(12):1161-1169

Drug prescribing in the old age is burdened by a significant number of prescribing errors often resulting in high rate of adverse drug events associated with increased morbidity, health care utilization and health costs. The revised 2003 Beers' Criteria represent the most widely used method for identification of high risk ("potentially inappropriate") medication in elderly persons. A standardized method for evaluating omission of potentially beneficial drugs has been lacking. The Beers' Criteria consist of the list of selected drugs with high potential of adverse drug events in old age and clinical conditions with relative contraindications of selected...

Giant cell arteritis manifested by bilateral arteritic Anterior Ischaemic Optic Neuropathy (AION)

P. Němec, T. Jurečka, V. Žampachová, Z. Mašková, M. Souček

Vnitr Lek 2008, 54(12):1195-1205

Giant cell arteritis (GCA) is a systemic vasculitis of unknown etiology affecting medium and large calibre vessels by granulomatous panarteritis with the formation of giant multinucleate cell granulomas. Vision is affected in 25-50% of GCA patients. Affection of vision may be the first GCA symptom or a symptom which occurs weeks or months after the initial symptoms of the disease. Eye symptoms of the disease are mostly a manifestation of occlusion of ocular and orbital blood vessels. Permanent damage to the patient's vision is a serious consequence of visual affection provoked by GCA. Arteritic Anterior Ischaemic Optic Neuropathy (AION) is the most...
