Vnitr Lek 2007, 53(3):231-241
Objective:The aim of the article is to present the method and results of catheter ablation for chronic atrial fibrillation. Method:Catheter ablation for chronic atrial fibrillation was performed in 82 patients (18 females, aged 54 ± 10 years), in 112 ablation procedures. Mean duration of the chronic phase of atrial fibrillation was 28 ± 28 months. Before ablation, amiodarone was administered without effect to 74 (90 %) patients, and was counter-indicated in 8 (10 %) patients. Ablation strategy consisted of circumferential lesions around the pulmonary veins and of complex linear lesions in the left atrium. Full...

Vnitr Lek 2007, 53(3):242-245
Introduciton:Impairment of cognitive functions is one of the neuropsychiatric symptoms of systemic lupus erythemathosus (SLE), the association of which with secondary antiphospholipid syndrome is often mentioned in the literature on the subject. Ethiology has not yet been fully clarified. Patients and methods:The aim of the study was to compare the "mini-mental state examination" (MMSE) outcomes in SLE patients with and without antiphospholipid antibodies (APA). Results:Probands producing APA presented with significantly worse results in MMSE when compared to patients without these antibodies. There was no difference...

Vnitr Lek 2007, 53(3):246-252
Acute humoral rejection (AHR) is a rare complication which often results in the loss of kidney graft. The objective of this retrospective monocentric study was to evaluate two different approaches to AHR. Documentation of 730 patients was analysed, who underwent renal transplantation between 2002 and 2005. From 2002 to 2003, patients with AHR were treated with 5 plasmaphereses (PF group, n = 13), and from 2004 to 2005 with a combination of 5 PF and intravenous immunoglobulins (PF + IVIG, 0.5 g/kg, n = 8). Data for the period of one year post-transplant was analysed. AHR occurred in 21 out of 730 patients (2.9 %). Survival of grafts in the 6th month...

Timely diagnosis of malignant diseases largely depends on attention being given to early symptoms and on timely start of an extensive diagnostic process. Only this way can a tumour be diagnosed in its initial stage, and better effect of therapy can be achieved. The following overview provides a list of systemic (paraneoplastic - distant) manifestations of a tumour, and of symptoms related to local tumour expansion. The objective of the overview is to draw attention to all early symptoms of malignant diseases in patients, and to contribute to timely diagnosis and treatment.
Infection and atherosclerosis

Vnitr Lek 2007, 53(3):286-291
Atherosclerosis is a chronic inflammation and the cause of most cardiovascular diseases. It is the main cause of death in the Western world today, even though growing incidence of atherosclerosis-related diseases has been recently observed in developing countries, too. In many patients with atherosclerosis, however, traditional risk factors for atherosclerosis are not identifiable. This has renewed the interest, in recent years, in the links between atherosclerosis and environmental exposures, including infectious agents. Infection was identified as as risk factor for atherosclerosis in the first half of the 20th century. Experimental and clinical...
Effectiveness of anti-resorption therapies of postmenopausal osteoporosis
J. Štěpán
Vnitr Lek 2007, 53(3):293-299
The aim of this article is to justify a need for evaluation of the absolute risk of fractures, to compare efficacy of anti-resorptive therapies as indicated by large clinical trials, and to review currently available methods to assess efficacy of therapies in individual patients. It is emphasized that adherence to therapies is influenced not only by the individual bone efficacy and safety of the drug but also by beneficial impacts of the treatment on tissues other than bone.

Hashimoto's encepahlopthy - a rare and unusual syndrome
J. Payer, L. Lisý, L. Baqi, T. Petrovič, P. Langer
Vnitr Lek 2007, 53(3):300-306
Hashimoto's encephalopathy (HE) is a brain disease associated with autoimmune thyroid disease. Over 100 articles have been published on the topic, especially in connection with hypothyroidism. In addition to an overview of the relevant literature, we also report a case of a female patient with a rare association of HE with thyreotoxicosis. The patient presented with varied clinical-neurological symptoms: she had convulsions, psychotic symptoms, altered consciousness Examinations (laboratory tests, CT, MRI, EEG, CSL exams) detected increased level of proteins in the liquor, episodes of rhythmic δ activity on EEG, increased antithyreoidal antibody...
Detection of ectopic gastric mucosa in Meckel's diverticulum by means of scintigraphy with 99mTc-pertechnetate in a 24-year old man
J. Doležal
Vnitr Lek 2007, 53(3):307-310
Aim:The article presents a case of a 24-year old man with ectopic gastric mucosa in Meckel's diverticulum. Ectopic gastric mucosa may be present in Meckel's diverticulum and may be associated with peptic ulcer disease and lower gastrointestinal (GI) bleeding. Diagnosing ectopic gastric mucosa can largely benefit from 99mTc-pertechnetate scintigraphy. Material and method:A 24-year old man underwent Meckel's diverticulum scintigraphy for timely anamnesis of GI bleeding from uncertain source (enterorrhagia, haemorrhagic anaemia). Upper tract and large bowel endoscopy, enteroclysis, ultrasound, CT and radioactive labelled...
