Vnitr Lek 2007, 53(1):9-17
Endocrinal products of adipocytes (PPARγ, A-FABP, E-FABP, leptin, adiponectin and others) modulate insulin tissue sensitivity enabling them to participate in the ethiopathogenesis of diabetes mellitus type 2 (DM2T). Persons with DM2T are characterised by typical changes in lipid spectrum (lower HDL-cholesterol and higher TAG) and in the endocrinal function of subcutaneous adipose tissue; adipocytes produce more PPARγ, A-FABP and E-FABP and less adiponectin. Aims of the study:To measure chosen markers of metabolic syndrome (MS) in serum and in abdominal subcutaneous adipose tissue in healthy persons and patients with DMT2,...
Disorder of glucose metabolism regulation in patients with multiple myeloma treated with high doses of corticosteroids at our clinic in 2004

Vnitr Lek 2007, 53(1):18-23
A deterioration of glucose tolerance is one of the most important side-effects of glucocorticoid therapy. Disorders of glucose metabolism are present in almost all patients treated with glucocorticoids and 25 % of these develop manifest diabetes mellitus. Glucocorticoids increase gluconeogenesis in hepatis and decrease insulinosensitivity in peripheral tissues and probably also decrease the release of insulin from pancreatic β cells. The deterioration of glucose tolerance leads to worsening of morbidity and mortality of seriously ill patients. In glucocorticoid-induced diabetes mellitus the highest levels of glucose are seen in the afternoon,...
The smoothness index of betaxolol hydrochloride in patients with newly diagnosed hypertension
I. Řiháček, M. Souček, P. Fráňa
Vnitr Lek 2007, 53(1):25-30
Introduction:24-hour ambulatory blood pressure monitoring (ABPM) has a higher predictive value for cardiovascular diseases than occasional blood pressure (BP) measurement with sphygmomanometer. ABPM allows for the assessment of 24-hour effect of drugs administered once a day using the smoothness index (SI) method. Objective of work:Find out the 24-hour effect of betaxolol hydrochloride administered once a day by determining the smoothness index. Cohort and methodology:Examination of 30 newly diagnosed hypertonics prior to and after 3-month treatment with betaxolol hydrochloride at an average dose of 15 mg once...
Chemotherapy of BOVAPEC in the primary treatment of Hodgkin's lymphoma intermediate stages
L. Raida, T. Papajík, K. Indrák, M. Heřman, B. Pauček, J. Zapletalová
Vnitr Lek 2007, 53(1):31-37
Design:Chemotherapy of BOVAPEC is the modification of temporary intensified Stanford V protocol, an effective primary treatment of advanced Hodgkin's lymphoma (HL) in spite of limited toxicity. Nitrogen mustard was substituted by less myelotoxic cyclophosphamide and the protocol has been used in the treatment of patients with an intermediate stage of HL. Methods:The primary treatment with BOVAPEC was started in 62 patients. Complete chemotherapy schedule was administered to 60 patients (97 %) and the median of its overall duration was 13 (12-18) weeks. 31 patients (50 %) underwent adjuvant "involved field" radiotherapy (RT)....
The importance of determination of Nt-proBNP and big endothelin in diagnosing chronic heart failure in patients on regular haemodialysis treatment

We have followed 99 patients with end stage renal failure, treated by regular haemodialysis. Chronic renal failure is frequently accompanied by chronic heart failure (over 50 %), especially by heart failure with preserved ejection fraction. Patients treated by regular haemodialysis had a tendency to cardiomegaly (51 %), mild systolic dysfunction of the left ventricle (mean LVEF 53 %) and diastolic dysfunction (88 %) of the hypertrophic left ventricle. They had also activated endothelin and neurohumoral system. Only 3 % of the patients had normal values of Nt-proBNP and big endothelin. The plasma level of Nt-proBNP in haemodialysed patients correlated...

Epilepsy and disorders of reproduction

Vnitr Lek 2007, 53(1):46-53
Epilepsy in women is relatively often linked with reproductive disorders which include polycystic ovarian syndrome, hypothalamic amenorrhea and functional hyperprolactinaemia. These disorders have a significant share in a high incidence of infertility and premature menopause while the polycystic ovarian syndrome, also manifested by the metabolic syndrome, places the affected patients at risk of later consequences such as type 2 diabetes mellitus, cardiovascular diseases including arterial hypertension, gynaecological neoplasias (the breast and the endometrium), and in the case of pregnancy, a higher incidence of pregnancy induced hypertension. Apart...
Aortic allograft (homograft) for the treatment of valvular disorders in adults with focus on treatment of infective aortic valve endocarditis

Vnitr Lek 2007, 53(1):54-62
Still after 40 years of heart valve prostheses intensive development ideal valve substitute still does not exist. Aortic allograft represents one alternative which could be used for aortic and/or pulmonary valve replacement. This type of biological heart valve prosthesis is being currently discussed from the point of view of Tissue Banking, as well as from clinical aspects - e.g. surgical implantation technique and long term results. Live issue remains particularly the aortic allograft implantation into the aortic position. The authors discuss the aortic allograft role in the aortic valve infectious endocarditis treatment, which was widespread worldwide...
The FEVER (Felodipine EVEnt Reduction) trial; a randomised, double-blind, placebo-controlled trial in Chinese hypertensive patients
M. Souček, M. Plachý
Vnitr Lek 2007, 53(1):63-70
The FEVER trial observed the difference between intense and less intense treatment of hypertension by comparing combination therapy consisting of a low diuretic dose (12.5 mg of hydrochlorothiazide) and a low calcium antagonist dose (5 mg of felodipine) with monotherapy based on a low diuretic dose (12.5 mg of hydrochlorothiazide) in Chinese hypertensive patients. The trial enrolled 9,711 hypertonics (of which 4,841 on hydrochlorothiazide + felodipine combination and 4,870 on hydrochlorothiazide + placebo combination). Blood pressure decreased from 158.7/92.4 to 138.1/82.3 mmHg and from 158.0/92.7 to 141.6/83.9 mmHg in the combination therapy group...

Vnitr Lek 2007, 53(1):71-77
Takayasu arteritis (TA) is a rare chronic vasculitis primarily involving the aorta and its main branches. The disease affects women much more frequently than men, the ratio being 8: 1. The onset occurs in the teenage years, always before age of 40. Aortic regurgitation is rare. The pulmonary artery stenosis may also rarely occur. We report the case of a 49-year old female patient with Takayasu arteritis who presented with severe left pulmonary trunk stenosis resulting in pulmonary hypertension, severe aortic regurgitation due to the dilatation of the ascending aorta, mitral insufficiency, critical left renal artery stenosis, and probably with left...
