Anti-platelet therapy reduces postprocedural cardiac events in patients referred to percutaneous coronary intervention. The object of this study was to determine the effect of pre-treatment with ticlopidine plus aspirin compared to aspirin alone. Prospectively collected data on 360 consecutive patients with stable angina pectoris or evidence of inducible myocardial ischemia were analyzed. In a non-randomized trial, three days of pre-treatment with ticlopidine plus aspirin was compared with standard post-procedural ticlopidine therapy started one hour after percutaneous intervention. The primary end point was the incidence of pathological troponin I...

Study objective:Sympathetic-adrenal activity plays important role in multifactorial pathogenesis of essential hypertension. Tissue effect of catecholamines is mediated by adrenergic receptors. Vasodilatation action of β2-adrenergic receptors participates in BP regulation. Variations in their structure may lead to the limitation of vasodilatation efficiency and thereby to genesis of hypertension. Consequently polymorphism of β2-adrenergic receptor gene seems to be a promising candidate gene for an analysis of its association with essential hypertension. Patients' population and methods:In this...
Relevance of left ventricular contractile reserve evaluated by dobutamine echocardiography in patients with advanced chronic heart failure
P. Lupínek, P. Frídl, I. Málek, T. Marek, J. Kautzner
Vnitr Lek 2005, 51(9):950-956
Background:Several publications have suggested that the examination of left ventricular contractile reserve by dobutamine echocardiography may be useful in the prognostic evaluation of patients with heart failure. Aim:To investigate whether the change of left ventricular volumes, ejection fraction (LVEF) and Doppler Tei index after dobutamine challenge is able to predict further evolution of heart failure. Methods:66 heart transplant candidates (average age 51.5 ± 9.2 years; men/women 55/11, dilated cardiomyopathy/ischemic heart disease 32/34) with chronic systolic heart failure underwent dobutamine echocardiography...

Research objective:Research objective was to assess, if controlled rehabilitation programme can influence an extent of stress-induced myocardial ischaemia, clinical status and prognosis of patients with chronic ischaemic heart disease with significant coronary stenosis on 1 or more arteries and with manifestations of myocardial ischaemia, which was indicated to conservative treatment. Patients' population:69 patients with chronic ischaemic heart disease (IHD) (14 women/55 men) in the age of 62 ± 8 (62) years were enrolled in the study. In 23 patients 8-week ambulatory controlled rehabilitation programme (RHB) with aerobic...

Vnitr Lek 2005, 51(9):965-970
Objective:Big endothelin (BE) and interleukin 6 (IL 6) belong to important active substances, which are related to severity of heart failure. We compared patients with high and low levels of BE and IL 6 in relation to haemodynamics, left (LV) and right ventricle (RV) functions. Population:155 patients with chronic heart failure, left ventricular ejection fraction (LVEF) below 40 %, NYHA II-IV, mean age 51.8 ± 8.8 years, 129 men, 26 women. 86 patients were diagnosed with ischaemic heart disease (IHD), 69 patients had dilatation cardiomyopathy (DCMP). Methods:Echocardiography with LV sizes, LV volumes and...
Treatment of symptomatic intermitent atrial fibrillation with catheter ablation in the left atrium: Immediate and long-term results in 150 patients

Vnitr Lek 2005, 51(9):971-983
Catheter ablation has been establishing as a routine curative method of atrial fibrillation. Immediate and long-term results of catheter ablation employing method of pulmonary vein isolation using conventional or electroanatomic mapping in patients with intermitent atrial firillation are presented. Patients and methods: Ablation was performed in 150 patients (26 females), aged 52.4 ± 10.7 years, in 173 procedures. Results: During the initial ablation procedure in the first 110 patients, full pulmonary vein isolation was achieved in 358 (96%) of 372 target pulmonary veins. In 286 (77%) out of these 372 pulmonary veins, investigated angiografically...
Migraeflux in acute treatment of migraine
J. Mastík, I. Novotná
Vnitr Lek 2005, 51(9):984-987
Relatively large portion of migraine patients with and without aura may be classified according to MIDAS score into the groups I and II. In accordance with the method of stratified treatment it is recommended to use nonspecific combined antimigrainous agents during paroxysms in groups I and II. Authors demonstrated good tolerability and satisfactory efficacy of Migraeflux preparation (paracetamol with dimenhydrinate) both in the alleviation of headache intensity and in the action on accompanying symptoms (nausea, vomiting, photophobia and phonophobia). They noted also relatively low incidence of recurrence of migrainous paroxysms and adverse effects,...
The effect of removable total contact cast therapy on healing of patients with diabetic foot ulcers, acute Charcot osteoarthropathy and neuropathic fractures.

Vnitr Lek 2005, 51(9):988-994
Removable total contact cast (TCC) is one of the appropriate methods used for lower limb off-loading in patients with the diabetic foot. The aim of our study:To assess the effect of TCC therapy on healing of patients with neuropathic foot ulcers, acute Charcot osteoarthropathy and neuropathic fractures. Methods:Removable TCC were applied for different indications during the observed period of 20 month: for chronic neuropathic ulcers in 27 patients (group 1; mean age 53.5 ± 8 years, mean diabetes duration 14.8 ± 7 years, mean HbA1c 8.2 ± 2%), for acute Charcot osteoarthropathy in 35 patients...

Disorders of iron metabolism. Part 2
J. Novotný
Vnitr Lek 2005, 51(9):995-1006
Disorders of iron metabolism are very frequently seen in clinical medicine. Up to 60% of women in gestational age suffer from some degree of sideropenia. Sideropenia occurs in situations with the negative bilance of this vital element - i.e. in patients with chronic blood loss, in vegetarians or in malaabsorptive syndromes and in states with increasing utilization of iron in gravidity, lactation or in pubescents. The iron overload represents the opposite situation and is found mainly in transfusion dependent patients, in some hemolytic states with inefective erythropoesis, in severe hepatopathias and in patients with congenitaly incerased absorption...

Noncomplicated course of hypertension, paroxysmal atrial fibrillation and essential thrombocytaemia
J. Pella, P. Kalist, J. Lazúr
Vnitr Lek 2005, 51(9):1007-1010
In the case report we describe more than 17-year long monitoring and treatment of 79-year-old woman patient. In spite of progressive increase of number of diseases (altogether 6), each being associated with increased risk of thromboembolic (alternatively haemorrhagic) events, the history was without complications up to now. Hypertension, diagnosed in the year of 1987, was the first disease we monitored and treated with calcium channel blocker (CCB - verapamil). Patient experienced the first paroxysm of atrial fibrillation (PAF) in the year of 1997. This paroxysm was managed by application of propaphenone in combination with acetylsalicylic acid in...
Carcinoid
V. Cempírková, P. Havránek
Vnitr Lek 2005, 51(9):1011-1018
The authors are reporting on two cases of carcinoid tumours, which represent very well the great extent of problems with these malignomas and are discussing diagnostic signs and some puzzles. The first case is an unusual one, with the thrombosis of lienal and portal vein, displaying the difficult and tedious investigation of both etiology and character of the growth. The second case represents the carcinoid syndrom as the cardiologist's primarily problem. The authors are discussing the possibilities of the treatment in the case of generalised disease.
