[The information potential of the names of drugs].

V Springer, J Kolár, M Chalabala

Vnitr Lek 2004, 50(2):91-97

Authors pay their attention to the issue of names of pharmaceuticals and drugs and their relationship to their therapeutic use. They specify way of making names of pharmaceuticals and structure of those names (chemical name, trivial name, generic name, unprotected name--INN, name in dispensatory) and way of making names of drugs (protected titles of original drugs, names of generic drugs). One part of the work comprises a chapter applying to subsidiary pharmaceutical agents and their E-code.

[Distribution of HBV genotypes in the Czech population--pilot study].

L Krekulová, V Rehák, P Killoran, M Zavoral, L W Riley

Vnitr Lek 2004, 50(2):106-111

A compact structure of the hepatitis B virus genome does not provide much space for development of mutations capable of further replication. In spite of that, 7 genotypes have been described so far. Discovery of the hepatitis B virus (HBV) genome diversity led to a range of further questions. Individual genotypes are subjects of investigation these days as for the pattern of disease transmission, course of disease, and consequences of an infection. The aim of this pilot study was to describe genotype distribution in the infected population of the Czech Republic. Moreover, epidemiological and clinical characteristics were monitored in our sample. We...

[Effect of beta radiation brachytherapy in the treatment of restenosis of coronary stents].

J Kovác, J Lukl, A Gershlick

Vnitr Lek 2004, 50(2):112-117

Brachytherapy currently represents the only treatment method for in-stent restenosis (ISR) confirmed in randomised clinical studies. In a group of 46 patients, selected for brachytheraphy on the bases of clinical signs of restenosis accompanied with development or deterioration of angina pectoris and with angiography results showing significant in-stent restenosis, we used a beta radiation system BetaCath NOVOSTE. This therapy was safe. Results of acute angiography were positive in 91% of patients. Clinical restenoses within 6 month of clinical monitoring occurred in 8.6% of patients. Due to impairment of more arteries in the majority of patients from...

OBJECTIVES: To investigate feasibility and safety of primary PCI in diabetic patients.BACKGROUND: Diabetic patients with acute myocardial infarction (AMI) have been shown to be at high risk for adverse clinical outcomes. Limited data is available on long term prognosis of diabetics treated with primary PCI.METHODS: Retrospective analysis of consecutive 67 diabetic patients and 211 non diabetic patients treated with primary PCI from 1/1995 to 12/1999, follow up for 38 +/- 12 months.RESULTS: The baseline characteristics were comparable in both groups. The mean age was 62 years in diabetic patients and 59 years in non diabetic patients. Hypertension...

[Occurrence of carcinoid syndrome in patients with carcinoid tumors].

S Kinová, E Kovácová, J Bulas, J Murín, M Belovicová, I Jurkovicová, I Duris

Vnitr Lek 2004, 50(2):126-133

BACKGROUND: Carcinoid syndrome occurs in about 10% of patients with carcinoid tumors.OBJECTIVES: The main objective of the present study was to determine the presence of the carcinoid syndrome in the group of patients with the carcinoid tumor.SUBJECTS AND METHODS: 43 pts. (22 w/21 m) with carcinoid tumors (M group--26 pts. with metastatic disease, B group--17 pts. after resection of the primary tumor without known metastases at the time of the investigation) were included in the study. We estimated the presence of the carcinoid syndrome in all patients. The urinary excretion of 5-HIAA was performed and echocardiography were done.RESULTS:...

[Skeletal involvement in Hodgkin's lymphoma--personal experience].

D Feltl, J Marková, T Kozák

Vnitr Lek 2004, 50(2):134-138

PURPOSE: To assess the incidence and prognostic significance of osseous Hodgkin's lymphoma.METHODS AND MATERIALS: The authors retrospectively analyzed 85 patients treated at their institutions between 1995-2001. They assessed the incidence of bone involvement, involved localities and basic characteristics of the subgroup (age, gender, stage, histology). All patients were treated according to the protocols of the German Hodgkin Study Group (GHSG). Treatment response, disease free- and overall survival were evaluated with special attention to predictive and prognostic significance of bone involvement.RESULTS: From the total of 85 eligible patients...

[Relation between Bcl-2 protein expression and results of therapy in patients with acute myeloblastic leukemia].

E Tóthová, N Stecová, A Kafková, M Fricová, T Guman, A Elbertová

Vnitr Lek 2004, 50(2):139-142

Flow cytometric expression of Bcl-2 protein was analyzed in 67 newly diagnosed acute myeloblastic leukemia (AML) patients using an anti-Bcl-2 monoclonal antibody by direct immunofluorescence technique and result were correlated with FAB subtype, CD34 expression and clinical outcome. The number of Bcl-2+ cells in each sample was heterogenous (range, 19% to 96%), with mean of 81%. The percentage of Bcl-2+ cells was higher in M0 and M1 types according French-American-British classification. The mean fluorescence index (MFI), expressed as the ratio of sample channel: control mean channel was significantly higher (p < 0.01) in M0 (19.0) and M1 (17.6)...

[Endoscopic control of hemorrhage in the upper part of the digestive tract using hemostatic clips].

J S»ovícek, R Keil, M Buresová, J Golánová, L Hrdlicka

Vnitr Lek 2004, 50(2):143-146

One of the endoscopy methods to stop upper gastrointestinal tract (GIT) bleeding in the limelight these days is hemostasis using metal clips. This method is indicated especially in patients with bleeding from an eroded artery and in patients where other hemostatic methods proved to be unsuccessful. Goal of our work was to assess successfulness of hemostasis, percentage of early rebleedings, and lethality of patients managed with this method. At the same time we present indications and locations of bleedings in these patients. We have retrospectively evaluated a group of 75 patients indicated for urgent endoscopy for GIT bleeding in a period from August...

[Significance of plant sterols in diabetes].

A Smahelová, Z Zadák, R Hyspler, T Haas

Vnitr Lek 2004, 50(2):147-152

Plant sterols occur naturally in plants and vegetable oils. Sitosterol and campesterol are markers of cholesterol absorption. The ratio of cholesterol endogenous synthesis to its absorption may be assessed by sitosterol, campesterol and other non-cholesterol sterols (lathosterol and squalen) serum concentration measurements. In 38 Type 2 diabetics (59.9 years, BMI 29.8 kg/m2, HbA1c 7.6%, C-peptid 0.82 nmol/l) and in 40 non-diabetics (37.2 years, BMI 25.4 kg/m2, HbA1c 5.2%, C-peptid 0.85 nmol/l) plant sterols serum concentration were measured: lathosterol (diabetics 10.64, non-diabetics 6.04 mumol/l, p = 0.09), squalen (diabetics 3.42, non-diabetics...

[Procalcitonin as an indicator of infection in patients with liver cirrhosis].

L Husová, P Husa, M Senkyrík, J Lata

Vnitr Lek 2004, 50(2):153-156

Incidence of bacterial infections in hospitalised patients with liver disease is high. Due to a liver dysfunction immune reactivity is significantly impaired and bacterial infections are more frequent. Also incidence of nosocomial infections is higher in patients with liver disease compared to patients hospitalised for other conditions. To make a differential diagnosis of infectious and non-infectious aetiology of an inflammation is very difficult. Characteristic laboratory tests for bacterial infection include test of a number of leucocytes in peripheral blood, differential count of leucocytes, erythrocyte sedimentation, procalcitonin, C-reactive...

[Nimesulide: a lower risk treatment in rheumatic diseases].

K Trnavský

Vnitr Lek 2004, 50(2):157-162

This review paper presents some basic information of a "rediscovered" nonsteroidal anti-inflammatory drug Nimesulid which is one of the first selective inhibitors of cyclooxygenase-2, has a history of good clinical results, and is relatively well tolerated with patients. Nimesulid enhances possible ways of analgesia treatment of rheumatic diseases.

[Clinical conditions imitating pheochromocytoma in diagnostic practice].

I Balazovjech

Vnitr Lek 2004, 50(2):163-166

Serious predominantly diastolic treatment-resistant hypertension with unstable blood pressure values draws first contact physician's attention to a possibility of pheochromocytoma, although in some periods of disease blood pressure values of a patient can be normal or only slightly elevated. Clinical picture is sometimes atypical, can be overlooked or lead to a wrong diagnosis. The author brings forward atypical cases where pheochromocytoma emulates different disease or diseases with clinical pictures similar to that of pheochromocytoma.
