[Statins in the treatment of patients with arterial occlusive disease of the lower extremities].
O Mayer, M Hromádka
Vnitr Lek 2001, 47(10):664-669
In patients with advanced atherosclerosis manifested by arterial occlusion in the lower extremities without a baseline blood lipid disorder statin treatment caused improvement of the efficiency, i.e. prolongation of the claudication interval, while in a comparable control group without statins the efficiency deteriorated further. In patients with arterial occlusion of the lower extremities with baseline dyslipidaemia statin treatment proved protective, i.e. at least it retarded the patients' complaints. The authors recorded improvement of the prooxidation state which followed after the dynamics of improvement of the impaired blood lipid spectrum. The...
[Comparison of two types of devices for long-term Holter monitoring of the ECG in detection of heart arrhythmias].
E Sovová, V Doupal, J Lukl
Vnitr Lek 2001, 47(10):670-673
UNLABELLED: The objective of the work was to compare the use of two types of long-term ECG monitors in examinations of cardiac arrhythmias and to find out whether their yields differed significantly when patients with a history of palpitations and syncope are examined. Two recorders, King of Heart with manual operation of the ECG tracing were used and the R test with continuous recording of the ECG tracing. For statistical processing contingence tables were used. For examination by the King of Heart recorders 111 patients (40 men and 71 women) were indicated, average age 48 years (14-84 years). 108 patients were examined. Yield I (number of patients...

Vnitr Lek 2001, 47(10):678-681
The group of infectious agents causing viral hepatitis was extended by another virus the detection of which was published in 1995. This virus is described as virus of hepatitis G (HGV) or GBV-C virus and is included among RNA viruses family Flaviviridae. Detection of HGV is based on reverse transcription with subsequent amplification of the viral nucleic acid by means of a polymerase chain reaction. The authors examined sera of 82 chronically haemodialyzed patients incl. 22 (26.8%) HGV RNA positive ones, whereby coinfection with HBV was proved in 3 specimens (13%). In 6 HGV RNA positive samples anti-HCV positivity was found (27%), incl. one sample...
[Parameters of power spectral analysis of heart rate variability for use in clinical evaluation of various stages of diabetic cardiovascular autonomic neuropathy].
J Hosová, A Jirkovská, P Boucek, J Pumprla, J Skibová
Vnitr Lek 2001, 47(10):682-688
UNLABELLED: The aim of the study was to select parameters of power spectral analysis appropriate for clinical evaluation of various degrees of diabetic cardiovascular autonomic neuropathy. The diagnosis of autonomic neuropathy was based on Ewing's battery of cardiovascular autonomic function tests and on short-term power spectral analysis of heart rate variability during a modified orthostatic load (in positions supine-standing-supine). 52 diabetic patients and 24 age-matched controls were included. Diabetic patients were divided into two groups according to the total Ewing score. Groups of 14 subjects with early autonomic neuropathy and 38 subjects...
[Long-term follow-up of patients with extensive ulcerous colitis with emphasis on the occurrence of dysplastic changes and colorectal carcinoma].
M Konecný, J Ehrmann, V Procházka, V Kucerová, K Vyslouzil
Vnitr Lek 2001, 47(10):689-693
The chronic course of idiopathic intestinal inflammations is characterized also by a relatively high incidence of complications, local ones affecting the entire large intestine, as well as extraintestinal ones in remote organs. In the submitted paper the authors present the results of a six-year follow-up focused on the incidence of dysplastic changes of the intestinal mucosa and colorectal carcinoma in 46 patients from a group of 220 patients followed up on a long-term basis at the Second Medical Clinic of the Faculty Hospital and Medical Faculty Olomouc because of the extensive type of ulcerative colitis. The incidence of these complications is not...
[Magnetic resonance in examination of the spine in patients with multiple myeloma].
M Vytrasová, V Scudla, J Nekula, J Bucil, V Vavrdová, J Bacovský
Vnitr Lek 2001, 47(10):694-698
Multiple myeloma (MM) is a malignant disease of the haematopoietic system characterized by the formation of osteolytic foci of the skeleton with predilection of the thoracolumbar portion of the spine. The submitted investigation evaluates the importance of examination of the spine by magnetic resonance (MR), as compared with results of conventional radiology (CR). The analyzed group of 75 patients with multiple myeloma was assembled in the course of the previous four years. All patients were examined by conventional radiology and magnetic resonance and the assembled results were mutually compared. On examination by MR a pathological finding was recorded...
[ECG Holter monitoring in the detection of cardiac arrhythmias yesterday, today and tomorrow].
E Sovová, V Doupal, J Lukl
Vnitr Lek 2001, 47(10):699-704
The paper summarizes contemporary knowledge and technical possibilities of the use of Holter systems of 24-hour and long-term ECG monitoring in the diagnosis of cardiac arrhythmias. It evaluates the development of different types of equipment and indications for their use. It summarizes the yield of different types and mentions detailed limitations for their use and deals with the economic evaluation of different methods. Finally it summarizes contemporary technical possibilities and future perspectives such as transmission of data over the internet and combination of ECG monitoring with other non-invasive examinations.
[Respiratory disorders during sleep in patients with diabetes mellitus].
R Olos, J Hanácek, M Mokán
Vnitr Lek 2001, 47(10):705-710
Respiratory disorders during sleep are a serious medical, economic and social problem. In the submitted review the authors discuss the possible relationship between sleep disorders and diabetes. In the introduction they make the reader familiar with basic information on sleep apnoea, incl. the definition, classification and basic pathomechanisms leading to this disorder. In the subsequent part the authors discuss possible relations between the two diseases, the possible participation of diabetic autonomous neuropathy in the pathogenesis of sleep apnoea, the possible influence of hypoglycaemia on sleep quality and the possible influence of sleep apnoea...
[Acute type B aortic dissection treated with endovascular implantation of a stent-graft].
P Tousek, P Gregor, J Krupicka
Vnitr Lek 2001, 47(10):711-714
Type B of the Stanford classification of dissection of the aorta is found in one third of patients with dissection of the aorta. The diagnosis of this disease is not simple. The clinical condition of these patients arouses frequently suspicion of other diseases, some diagnostic methods have a low sensitivity, other are frequently unavailable. Treatment of dissection type B may be either conservative or surgical, whereby better short-term and long-term results are recorded according to older reports by conservative methods. During recent years a new therapeutic method in type B dissection of the aorta is endovascular implantation of a stentgraft at...

[Transcatheter closure of ostium secundum atrial septal defects--initial experience with treatment of adult patients].
M Mates, J Vojácek, J Popelová, J Masura
Vnitr Lek 2001, 47(10):722-724
The authors present their initial experience with the use of Amplatzer septal occluder for transcatheter closure of secundum atrial septal defect. For patients where the secundum atrial septal defect type is haemodynamically significant or when paradoxical embolization via the defect is suspected, the catheterization technique is an alternative for cardiosurgical correction of the defect. The morphology of the defect which is the basic criterium for indication of a transcatheter closure is evaluated on transoesophageal echocardiographic examination. The closure takes place under skiascopic and transoesophageal echocardiographic control. The procedure...
