Vnitr Lek 1999, 45(3):139-147
INTRODUCTION: Leptin is a protein, determined by the ob-gene which influences in a fundamental way the energy metabolism of the organism. A significant effect of leptin on the haematopoietic, immunological and endocrinological system cannot be ruled out nor its effect on the course of pregnancy and maturation of the organism. In humans leptinaemia correlates with the amount of subcutaneous fat which is due to a receptor or post-receptor disorder, obese subjects suffer frequently from hyperleptinaemia.OBJECTIVE: To assess the mean leptinaemia of the general population, seek relations between leptinaemia and anthropological indicators and age.METHODS:...

The authors present a group of their first 110 patients subjected to selective coronarography during an average 7-hours hospitalization period. In selection of patients they respect the criteria ruling out the following approach: age above 70 years, distance of the patients domicile above 80 km or inaccessibility of catheterization laboratory within 45 minutes, serious aortic valve disease, unstable coronary syndromes, insulin treated diabetes mellitus, renal insufficiency, cardiac insufficiency, grade III or IV, severe left ventricular dysfunction, history of malignant arrhythmia, acute myocardial infarction or cerebrovascular attack during month...
[New beta0-thalassaemic insertion mutation (CD 7/8, +G) in a Slovak family, associated with the Mediterranean haplotype IX].
E Kynclová, V Divoký, L Kovaríková, R Melichárková, J Indráková, M Divoká, T Hammerová, A Sakalová, J Hudecek, K Indrák
Vnitr Lek 1999, 45(3):151-154
The authors describe a newly identified beta0-thalassaemic mutation found in two subjects from two generations of a Slovak family. The beta0-thalassaemic allele developed by insertion of one nucleotide (+G, CD 7/8) into the first exon of the beta-globin gene. The mutation causes a shift of the open globin reading frame which leads to the development of a terminal codon in codon 22. The thalassaemic allele is associated with the mediterranean haplotype IX. The mutation has in both heterozygotes the phenotype of beta0-thalassaemia minor with a slightly elevated level of HbF.
[Chlamydia pneumoniae and the risk of ischaemic heart disease. Prevalence in a group of hospitalized elderly men].
J Icsó, D Sabaková, C Cajka, M Halyáková
Vnitr Lek 1999, 45(3):155-158
Chlamydia pneumoniae (CP) causes frequently nosocomial pneumonia and other inflammations of the upper and lower airways. Initially reports on the association between infection with CP and ischaemic heart disease (IHD) were published in 1988 by Saikku et al. who found a higher antibody titre against CP in acute myocardial infarction and sudden cardiac death as compared with a control group. The mechanism of this phenomenon is explained by the action of Chlamydiae on LDL-cholesterol, cytokines, the tumour necrotizing factor with a subsequent effect on the vascular wall. The authors assessed IgG and IgM antibodies in 39 elderly men using the immunofluorescent...
[Linear regression analysis of values of the calciuria index, N-telopetide and bone mineral density in patients with osteoporosis].
J Wendlová, M Mikulecký, K Rácová
Vnitr Lek 1999, 45(3):159-162
UNLABELLED: The authors examined 49 patients (n=49) with primary and secndary osteoporosis incl. 37 women aged 41-58 years and 12 men aged 38-56 years. In all they assessed the 24-hour calciuria index (Ca/creatinine), the aminoterminal N-telopetide of collagen I of the organic matrix (NTx24) and the bone mineral density (BMD) in the first three lumbar vertebrae using quantitative computed tomography (QC).OBJECTIVE: To find an answer to the questions:1. Is the calciuria index in the 24-hour diuresis a parameter which provides information on osteresorption? 2. Is there a significant negative correlation between values NTx24 and BMD? 3. Is NTx a sensitive...
[Portosystemic anastomosis at the time of transplantation and TIPS].
J Klein, V Král, R Aujeský, Z Frysák
Vnitr Lek 1999, 45(3):163-166
Sclerotherapy has become in the treatment of acute haemorrhage of oesophageal varices the method of first choice. The relapse of haemorrhage within a five-year interval is however too frequent after this treatment. TIPS is a new method which can resolve profuse haemorrhage from varices in patients with a markedly reduced hepatic reserve. The high incidence of stenosis or occlusion (50% during the first six months) however foresees the use this method rather for bridging the interval before definite therapy i.e. transplantation of the liver. Patients with a sufficient functional hepatic reserve and those who are candidates for transplantation can be...
[Conversion from Consupren sol. to Consupren S capsules in kidney transplant patients].
I Matl, S Vítko, P Bubenícek, M Horcicková, J Lácha, V Lánská, O Marecková, L Nosková, R Petrásek, I Reneltová, P Táborský
Vnitr Lek 1999, 45(3):167-169
In a group of patients after transplantation of the kidney with stabilized graft function treated by Consupren sol. combined with prednisone and azathioprin in 20 patients (group A) Consupren sol. was replaced by Consupren S capsules, in 17 patients (group B) Consupren sol. therapy proceeded without any change. To maintain the cyclosporin blood concentration within the therapeutic range it was necessary after the change of drug form in group A to adjust the dosage of the drug in 12 patients of group A while in group B only in one patient (p

[In towns there is a higher prevalence of asthmatic symptoms in children than in rural areas].
V Vondra, M Reisová, M Branis, M Malý, F Kotěsovec, N Vitnerová, J Skorkovský, J Nozicka, J Kolácný
Vnitr Lek 1999, 45(3):173-179
The authors assessed the prevalence of symptoms of bronchial asthma by means of a standardized questionnaire used in the international survey PEACE (Pollution Effect on Asthmatic Children in Europe). The questions about complaints were addressed to children aged 6-13 years (the questionnaires were completed with the parents assistance). In urban areas 5669 children participated from Prague 5, i.e. 35% of all elementary school children, in Teplice 2489 (21% children), in rural areas: in the Benesov district 5619, i.e. 61% children, in the Prachatice district 1983, i.e. 37% children. The response rate of questionnaires in the urban areas was 86-88%,...

It discusses the treatment of hypercalcaemia and symptoms of gastroenteropancreatic tumours.
[Bioptic typing and classification of Hodgkin's lymphomas--status in 1998].
L Plank, F Fakan
Vnitr Lek 1999, 45(3):186-192
In the submitted review the authors give an account on the development of views regarding bioptic typing and classification of Hodgkin's lymphomas, incl. the REAL classification and the classification prepared at present by WHO. Hodgkin's lymphomas are divided into the nodular type with a predominance of lymphocytes--nodular paragranuloma and classical Hodgkin's lymphomas. In addition to a brief morphological description, the authors give also an account on immunohistochemical features of different types, short comments on the biology and evolution of tumourous diseases as well as basic data on the differential diagnosis of Hodgkin's lymphomas.
[Prevention and treatment of mushroom poisoning].
P Gavorník
Vnitr Lek 1999, 45(3):193-196
In the submitted review the author gives an account on basic aspects of primary, secondary and tertiary prevention of mushroom poisoning. Mushroom poisoning is not caused only by toxic mushrooms (true intoxications) but under certain conditions also by edible mushrooms (false intoxications and pseudointoxications). The basis of primary prevention is not to pick and eat mushrooms which could damage health, and knowledge of basic facts, and types of mushroom poisoning. The main objectives of secondary prevention of mushroom poisoning are to prevent absorption of toxins from the digestive tract into the human organism and their effective elimination from...
