[Immunophenotyping in chronic B-cell lymphatic leukemia].

The authors present the results of immunophenotyping analysis of cells from 67 patients with the diagnosis of B-CLL from group of 393 adults examined by immunophenotyping because of a haematological malignity. Characteristic for B-CLL is the finding of surface molecules HLA DR, B lymphocytic superficial molecules and CD5. Aberrant membrane equipment of B-CLL cells is suggested by the rare absence of some pan B lymphocytic sign. Marked reduction of T lymphocyte ratios, their sub-populations and NK cells is associated with immunodeficiency manifestations which are common in patients with B-CLL.

In a group of 74 patients with multiple myeloma the authors revealed elevated values of serum thymidine kinase (REA kit ADICO Praha, range of normal values 0-5 U/l) in 40% of the patients-incl. a group of 22 subjects examined at the time of diagnosis of the disease in 50%, and a group of 52 subjects examined in different stages of the disease in 36% of the patients. In a group of 74 patients with multiple myeloma examined in different developmental stages of the disease the authors evaluated the clinical impact of assessment of the serum thymidine kinase (S-TK) activity. The authors describe data on the course of pregnancy, delivery and subsequent development of 20 children born to parents treated for Hodgkin's disease. Thirteen women in the clinical stage II and III were delivered of 16 infants (10 daughters and 6 sons) and three men (II A and III A) had four daughters. Thrombocytes participate in the pathogenesis of myocardial infarction. Larger platelets are more active than small ones. In patients after an acute coronary a larger number of platelets was found, a medium volume of půatelets and a similar thrombocytocrite as compared with the healthy population of comparable structure and age. The MPV in patients of advanced age is statistically significantly larger. There are no significant correlations between MPV and the risk factors investigated by the authors.

We have previously shown that angiotensin converting enzyme inhibitor enalapril causes a potent decrease in portal pressure gradient, but only in about one half of patients with portal hypertension and an episode of bleeding esophageal varices in patient's history. Twenty-one consecutive patients after first episode of bleeding from esophageal varices were enrolled in the trial. Patients were treated by sclerotherapy in combination with enalapril. Nine patients with anemia of chronic renal failure were treated with recombinant human erythropoietin (rHuEPO) in dose 50-150 IU/kg/week. After 8 weeks the treatment was maintained with 30-50 IU/kg/week for one year. A significant increase of hemoglobin (Hb) level and red blood cell (RBC) count was observed in all patients. The authors examined, using bronchoalveolar lavage, 70 immunocompromised patients with pulmonary complications. The bronchoalveolar fluid was subjected to cytological and microbiological examination. The examination was not associated with any serious undesirable side effects, not even in patients who were in a serious general condition. The risk of development of ventricular arrhythmias can be evaluated at present also by examination of late ventricular potentials. Late ventricular potentials are high-frequency low-amplitude signals which develop in the terminal part of the QRS complex and are the manifestation of retarded excitation in the myocardial area where islets of vital muscle alternate with islets of fibrosis and/or necrosis. [Ebstein's anomaly in a 66-year-old woman].

The authors describe a case of Ebstein's anomaly in a 66-year-old woman. The first, and almost only clinical manifestation of the disease was atrial flutter at the age of 56 years with rapid restoration of the sinus rhythm after pharmaceutical treatment. The authors describe a 50-year-old man who was hospitalized on account of one-week increasing epigastric pain with marked regression several hours after admission. Due to its physiological and pharmacological action ANF could be an ideal diuretic and vasorelaxation product in the treatment of oedema and essential hypertension. Experimental and clinical investigations in oedematous conditions revealed a very slight diuretic and natriuretic effect of ANF, as compared with healthy subjects. [Interferon alfa in the treatment of hairy cell leukemia].

For patients with hairy cell leukemia, with regard to the cost of interferon treatment, splenectomy is the treatment of choice. In patients who do not respond to splenectomy or have relapses, the drug of choice is interferon alpha, 3 x 10(6) i.u.s.c. daily till remission occurs and then three times per week for a period of 18-24 months. After this period treatment can be terminated, it is however better to proceed permanently with lower maintenance doses. During a relapse interferon alpha is again administered, and if the disease does not respond, it is recommended to test a different interferon preparation or deoxycoformycin or chlorodeoxyadenosine.
