[2-dimensional echocardiography in the evaluation of right ventricular volume and function. Comparison with echocardiography].

P Niederle, V Jezek, J Jezková, A Michaljanic

Vnitr Lek 1991, 37(4):313-322

Using two-dimensional echocardiography (ECHO), the authors examined within one week preceding right-sided catheterization of the right ventricle (RV) 44 patients. Of three tested ECHO techniques for calculation of right ventricular volumes and/or function the best correlations were obtained by the method of evaluation of area-length of the right ventricle from two orthogonal projections (apical 4-cavity and subcostal with visualization of the outflow portion of the RV). The following correlations were assessed: telediastolic volume--r = 0.831, telesystolic volume--r = 0.815 stroke volume--r = 0.810 and ejection fraction (EF)--r = 0.752 (p less than...

[Echocardiographic monitoring of treatment of mild hypertension using nifedipine].

I Oral, H Vizinová

Vnitr Lek 1991, 37(4):323-329

Forty men with hypertension and a BPd lower than 14.66 kPa (110 mmHg) were treated for six months with nifedipine, 30 x 10 mg, with echocardiographically controlled changes of the minute output of the heart and the peripheral vascular resistance at rest and during an isometric load. In the group a wide range of values of cardiac output and peripheral vascular resistance was recorded. In patients with a high peripheral vascular resistance (group B, n = 11) a substantially better therapeutic effect was proved than in patients with a low initial vascular resistance (group A, n = 20). Nifedipine is indicated in patient with a slow heart rate at rest which...

[The significance of myoglobin determination in the differential diagnosis of chest pain syndrome].

O Topolcan, A Polívková, V Karlícek, M Zikmund, Z Kratochvíl

Vnitr Lek 1991, 37(4):330-335

In 1984-88 the authors examined in 813 subjects with the chest pain syndrome of varying aetiology (acute myocardial infarction, myocarditis, pericarditis, vertebrogenic algic syndrome, embolism of the pulmonary artery, patients lacking detectable organic causes of pain) the trend of myoglobin serum levels. They found significantly elevated values only in patients with myocardial infarction and myocarditis whereby the two diseases differ in particular as regards the shape of the curve of myoglobin values. In chest pain with another aetiology the myoglobin levels rose only rarely or not at all. From the differential diagnostic aspect it is particularly...

[Problems in indications for vascular reconstructive surgery].

J Petrík, P Roubal, V Korístek, Z Gregor, V Krejcí, I Dohnalová

Vnitr Lek 1991, 37(4):336-341

The authors discuss examinations made before a planned vascular reconstruction operation. As the number of vascular reconstruction operations is steadily increasing, and the operations affect the entire organism, and the operations are associated with a relatively high mortality rate, the authors try to reduce the latter by the correct classification into a risk group, select the correct order of surgical operations, and last not least, on the basis of these conclusions they try to define also the surgical procedure proper during reconstruction. As atherosclerotic affections can be isolated or combined, the authors divide the patients into three groups....

[Serum beta 2-microglobulin in multiple myeloma. I. Relation to selected indicators, clinical stage and disease prognosis].

V Scudla, M Budíková, E Fischerová, I Raska, V Vavrdová

Vnitr Lek 1991, 37(4):342-351

The authors evaluated in a group of 89 patients with monoclonal gammapathy (18 patients with monoclonal gammapathy of undermined significance, 34 patients examined at the time of diagnosis of multiple myeloma (MM) and in a group of 71 patients with MM examined in different stages of the disease) the serum beta 2-microglobulin. It was revealed that the mentioned indicator is of no differential diagnostic value, it is not related to sex nor to the immunochemical type of monoclonal immunoglobulin. A relationship of serum beta 2-microglobulin to age, serum urea and serum creatinine, to the severity of anaemia, serum albumin, sedimentation rate of red cells,...

[Use of nuclear magnetic resonance in the study of hemolytic anemia. Preliminary report].

V Fábryová, T Hammerová, L Zalibera, I Goljer, A Sakalová

Vnitr Lek 1991, 37(4):352-358

Nuclear magnetic resonance is a method which contributes nowadays to further investigations of the phosphate metabolism in red blood cells. By means of 31P nuclear magnetic resonance it is possible to record in intact red cells signals of phosphate compounds as well as changes in their concentrations. In the submitted work the authors followed up the important phosphate compounds in red cells--adenosine triphosphate, adenosine diphosphate and inorganic phosphate. They assessed and evaluated by statistical methods spectra of 10 healthy blood donors of different age and sex. The same procedure was used in 6 patients with hereditary spherocytosis, in...

[The effect of thrombocytapheresis on hematologic, biochemical and immunologic indicators].

J Nepomucká, A Vítek, J Sejkorová, A Motejlová, J Horejsí

Vnitr Lek 1991, 37(4):359-365

The authors investigated in 33 separate donors the influence of thrombocytapheresis on haematological, biochemical and some immunological parameters. The paired t-test revealed a statistically significant increase of values of total proteins, aspartate aminotransferase, immunoglobulin A, alpha 1 antitrypsin, transferrin and the C4 component of complement seven days after apheresis and a significant increase of thrombocyte, total protein, alkaline phosphatase, immunoglobulin A, orosomucoid, and transferrin levels two weeks after apheresis. A significant decrease of haemoglobin values was recorded 7 and 14 days after apheresis. As the revealed changes...

[Plasma iron levels in patients with rheumatoid arthritis].

M Kucera, H Brabcová

Vnitr Lek 1991, 37(4):366-368

In 50 patients with confirmed or classical rheumatoid arthritis the serum iron level (FeS) was examined (in some repeatedly), the total number of examinations being 82. In 51.1% reduced values were recorded. In 27 subjects of the group the total iron binding capacity was assessed. In none of the patients reduced values were found, in 24 the values were at the upper borderline of normal values and in one instance the value was elevated. In 13 patients with a low FeS with a normal or elevated total iron binding capacity Ferronat Retard was administered. The FeS level reached relatively rapidly normal values which in the author's opinion is rather a manifestation...

[The iron reserve status in patients on regular dialysis treatment].

J Zadrazil, T Papajík, V Scudla, J Jezáková, M Budíková, J Slavícek

Vnitr Lek 1991, 37(4):369-375

The authors examined the iron level, total plasma iron binding capacity, serum ferritin, the red cell haemogram and iron reserves in bone marrow of 31 patients with chronic renal failure who had regular dialyzation treatment. In 24 patients (77%) they did not detect any iron reserves in bone marrow, 22 of these patients had serum ferritin levels lower than 100 micrograms/l. The authors found a significant correlation between serum ferritin and the iron reserves in bone marrow and confirmed that assessment of serum ferritin is the most reliable non-invasive method for assessment of iron deficiency of the organism. At the same time they draw attention...

[Vitamin A and metabolic disorders of calcium and phosphorus in patients on a long-term dialysis program].

M Mydlík, K Derzsiová, I Machánová, M Brátová

Vnitr Lek 1991, 37(4):376-382

The aim of the present work was to reveal the relationship between vitamin A serum levels and some indicators of metabolic calcium and phosphorus disorders in patients in a long-term dialyzation programme. Thirty-six patients in a long-term dialyzation programme were divided into three groups. Group A comprised 11 patients who were treated for 3-9 months but without specific treatment, group B was formed by 10 patients treated for 2-5 years who were given calcium carbonate, 3 g/24 h, by the oral route for a period of six months and group C which comprised 15 patients treated for 3-10 years who were given 1 alpha, 25-dihydroxycholecalciferol (Rocaltrol),...

[Experience with the Czechoslovak-made analysis system for diabetes self-monitoring].

J Voznícek, V Chromý, E Hrbotická

Vnitr Lek 1991, 37(4):383-389

The authors tested the Czechoslovak analytical system for the self-control of diabetics involving diagnostic strips of melliPHAN reflo and the compensating glucometer Remat GU. For testing diagnostic strips and the glucometer from the first regular batch distributed to health institutions were used. The Czechoslovak system was compared with a similar system of Ames Co. and the enzymatic photometric method for glucose estimation. The Czechoslovak system is suitable for self-control of diabetics and provides satisfactory results in the range of blood sugar levels between 10 and 20 mmol/l. It overestimates results in the range of the reference interval...

[The autonomic nervous system and disorders of heart rhythm. I].

O Jurkovicová, S Cagán

Vnitr Lek 1991, 37(4):390-396

The authors give an account of recent findings on the modulating action of the autonomous nervous system on electrophysiological properties of the heart and on mechanisms by which the vegetative nervous system participates in the development of disorders of the cardiac rhythm. In the development of arrhythmias, in particular the importance of the sympathetic nerve is emphasized. The relationship of stimulation of the sympathetic nerve and the development of arrhythmias may be conditioned: by its great activity, uneven distribution of sympathetic innervation and abnormal reaction of the damaged tissue to stimulation of the sympathetic nerve. Mechanisms...

[Hemophilia].

M Matýsková, M Penka

Vnitr Lek 1991, 37(4):397-403

Haemophilia is one of the longest known diseases. Despite its relatively low prevalence it remains a serious medical and social problem. The authors tried to summarize in the submitted review contemporary knowledge of this hereditary disease.

[Use of nonparametric methods in medicine. VIII. Conclusion].

J Holcík, A Gerylovová

Vnitr Lek 1991, 37(4):404-409

The submitted paper terminates the series of eight articles on the application of non-parametric methods in medicine, which were published in this journal in 1990-1991. Attention is drawn, on the one hand, to the urgent need of knowledge of mathematical and statistical methods, on the other hand, to the fact that proper processing of experimental data is not guaranteed by the use of computer technique alone, the complexity and numerical correctness of calculations but by interpretation of results and their plausibility which depends above all on the correctness of the reflections of the research worker. On a general level attention is paid to the description,...
