[Precordial S-T stress mapping in the evaluation of the effects of percutaneous transluminal coronary angioplasty].

The morphological impact of percutaneous transluminal angioplasty (PTCA) on the treated artery is closely checked by coronary arteriography. Indirect non-invasive evaluations are made possible by electrocardiography, isotope methods and echocardiography which are the commonest visualization methods. The authors used a precordial S-T load map. After complete revascularization following PTCA there was a significant drop of the sum of S-T depressions and in some members of the group it reached normal values. The S-T load map is a suitable non-invasive procedure for monitoring the effect of PTCA.
[Contralateral asynergy after myocardial infarct].

The purpose of the work was to assess whether during several years of echocardiographic follow-up of patients after myocardial infarction later contralateral asynergy develops (i.e. asynergy of another coronary vessel than that which supplies the area of the infarction) and what are the clinical symptoms associated with its development. [Cor pulmonale in patients with pneumoconiosis].

Of 577 patients with pneumoconiosis who died in 1964-1988 cor pulmonale was revealed on necropsy in 334, i.e. in 57.8%. In 120 it was the cause of death, i.e. in 20.7% (and it remains the most frequent cause of death). The mean age of the patients who died from cor pulmonale does not differ significantly from the rest of the group. The prevalence of cor pulmonale in different forms of pneumoconiosis did not differ significantly. Emphysema of the lungs was revealed in 71.2% of the patients who died from cor pulmonale. The submitted work was concerned with two problems: a) investigation of the dynamics of changes of the fibrinogen and fibrin breakdown products after a single dose of streptokinase, b) evaluation of the effectiveness of thrombolysis from changes in the fibrinogen concentration. Two groups of patients were examined with the Q form of acute myocardial infarction who were treated at an intensive care unit. At a cardiosurgical department in Brno in 1978-1987 244 patients above 15 years of age were operated for the first ++time on account of inborn heart disease. This number comprised 21 patients (8.6%) with incomplete defects of the atrioventricular septum. The new calcium antagonist, the dihydropyridine derivative isradipine (Lomir, Sandoz) is used above all in the treatment of hypertension. During oesophageal stimulation the authors proved its very favourable effect also in patients with coronary insufficiency during exertion. A single oral dose of 7.5 mg of isradipine led in all 14 examined patients to increased tolerance of exertion; in 36% of the patients after administration of the drug coronary insufficiency was no longer found. Atenolol, a selective beta-blocker, slightly potentiates the stimulated aggregation of platelets against dose-dependence. The drug potentiates similarly stimulated release of arachidonic acid, it does not influence the formation of malondialdehyde and the thromboxane B2 production in stimulated platelets. We reviewed smears of 102 bone marrow aspirates and 3 spleen punctures of 30 patients with hairy cell leukaemia (HCL) to find the occurrence of hairy cells with ring-shaped nuclei. No such cells were found in 18 bone marrow aspirates designated as non-evaluable, because of the poor smears quality or low numbers of hairy cells (under 500). Hairy cells with ring-shaped nuclei were found in 4 bone marrow aspirates of two patients with a frequency of 1.5% and 1.0% of hairy cells in the first patient and 0.3% and 0.1% in the second one. The author describes his own modification of a two-colour fluorescent technique for typing of HLA-DR antigens. Concurrent examination of HLA-DR antigens in 34 subjects by this method and the monocytotoxic or B-lymphocytotoxic test revealed a greater sensitivity of the fluorescent technique; there was however also a tendency of non-specific positivity of results. More reliable results were obtained when the fluorescent technique was performed on Hammat plates, as compared with examinations using Terasaki plates. To eliminate erroneous results, the author recommends to use for HLA-DR typing two methods concurrently.
