Dyslipidemia in cardio-reno-metabolic syndrome

Cardio-reno-metabolic (CRM) syndrome is a complex disease resulting from the interaction of metabolic disorders, cardiovascular impairment and chronic kidney disease. The basic pathogenetic mechanism is adipose tissue dysfunction leading to insulin resistance, chronic inflammation and vascular dysfunction. A typical laboratory manifestation is atherogenic dyslipidemia characterized by hypertriglyceridemia, reduced HDL-cholesterol and increased small dense LDL particles, often with only slightly increased LDL-cholesterol. Postprandial lipoproteins and residual cholesterol represent a significant cardiovascular risk. The basal therapy of dyslipidemia in CRM syndrome is focused on dietary measures aimed at weight loss and improvement of insulin sensitivity. Incretin analogue therapy plays an important role. If lipid target values are not achieved in this way, hypolipidemic drugs are indicated.
