Vnitr Lek 2017, 63(3):163-169 | DOI: 10.36290/vnl.2017.035
Introduction:Radiofrequency catheter ablation (RFA) has recently become a routine part of atrial fibrillation (AF) treatment. The goal of our study was to determine whether the implementation of RFA of AF under general anesthesia (GA), compared to analgosedation (AS), will affect important characteristics of the ablation procedure, comfort of the patient and whether there is any clinical impact on the complication rate and the overall success of the procedure. Methods:50 patients with AF were randomized in a ratio of 1 : 1 into two groups for RFA in AS and in GA. Procedural characteristics, arrhythmia-free survival for the...

Comments on current guidelines of type 2 diabetes mellitus treatment
Emil Martinka
Vnitr Lek 2017, 63(3):211-217 | DOI: 10.36290/vnl.2017.042
In an effort to facilitate the widest possible application of recent findings in diabetology and the related medical fields, with regard to characteristics of medicines and current possibilities of using modern procedures, but also to their limitations due to the financial capacities of health insurance companies, SDS innovates its therapeutic recommendations for the treatment of diabetes mellitus on a regular basis. The most recent recommendations were issued by SDS in August 2016. The review discusses and describes several factors which the authors considered during their preparation: (1) Compliance with the findings of evidence-based medicine, compliance...

Effectiveness of education in diabetes care management - instructions for educators
Jarmila Jirkovská
Vnitr Lek 2017, 63(3):171-174 | DOI: 10.36290/vnl.2017.036
Continuous process of diabetic patient's education is essential. Educator's task is to select situationally and individually appropriate form of education and educational tools. Than he becomes capable to motivate patients to participate actively and cooperate. Use of practical and visual educational tools increases the memorability. Studies have shown positive effect on improvement of glycemic control for both individual and group education. When properly educated diabetic patient is able to make independent decisions and manage the disease. Achieving good long-term glycemic control with reduced risk of chronic complications in compliant diabetic...
Irregular breathing during the cardiopulmonary exercise test - from mildly irregular breathing pattern to periodic breathing of oscillatory ventilation type

The fluctuating course of tidal volume (VT), breathing frequency (DF) and minute ventilation (VE) during the cardiopulmonary exercise test using a ramp incremental protocol occurs not only in patients, but relatively frequently also in healthy individuals. It can account for a number of irregularities in the course of the curves VO2, VCO2 and in particular of those of ventilatory equivalents for O2 and CO2 (EQO2, EQCO2) as well as curves of partial pressure of end-tidal oxygen and partial pressure of end-tidal carbon dioxide (PETO2, PETCO2), which are also used,...
Contribution of CT colonography to clinical practice
Lukáš Lambert, Lucie Šimáková
Vnitr Lek 2017, 63(3):183-187 | DOI: 10.36290/vnl.2017.038
CT colonography is a CT examination focused on the detection of colonic polyps and masses. Its most common indications include incomplete optical colonoscopy and generally personal preference in symptomatic or frail patients. CT colonography has a diagnostic yield similar to optical colonoscopy. It is less demanding for the patient, has shorter examination times, lower rates of adverse effects (including bowel perforation) and allows limited evaluation of extracolonic structures. In 10-15 % of asymptomatic patients, optical colonoscopy is needed if either a colonic polyp or mass is detected. CT colonography requires bowel prior preparation, including...
Type 2 diabetes mellitus and chronic kidney disease
Peter Ponťuch
Vnitr Lek 2017, 63(3):188-192 | DOI: 10.36290/vnl.2017.039
The number of type 2 diabetic patients is increasing world-wide and a prediction of prevalence of chronic kidney disease up to 2025 in European diabetic population is alarming. Albuminuria and estimated glomerular filtration rate are cardinal biochemical parameters in diagnostics of diabetic nephropathy. Following diagnostic methods are also used: renal ultrasonography, ophthalmoscopy and in not clarified cases renal biopsy. Long-term optimal glycemic control, efficient antihypertensive treatment by angiotensin converting enzyme inhibitor, or angiotensin receptor blocker and recommended protein intake is a cornerstone of therapy. The research is presently...
The role of cardiovascular magnetic resonance imaging in the diagnosis of hypertrophic cardiomyopathy. Part I

Vnitr Lek 2017, 63(3):194-198 | DOI: 10.36290/vnl.2017.040
Hypertrophic cardiomyopathy is the most common genetic cardiomyopathy in our population. The diagnosis of this disease is based on imaging methods, mainly echocardiography. Cardiac magnetic resonance offers more accurate and detailed information about the disease than echocardiography. Development of this method has become a standard part of the diagnostic algorithm. In the first part of review, the authors analyze the potential and significance of cardiac magnetic resonance in the diagnosis of hypertrophic cardiomyopathy.
Cardiovascular complications of cancers and anti-cancer therapy

In our aging population the incidence of cancer is increasing in the elderly. We are thus facing a new challenge especially considering incidence of cardiovascular diseases (CVD) in this patients' population. Overall survival of cancer patients has significantly improved therefore cancer has become in many cases a chronic disease. We are about to be treating patients who either may develop a new CVD or their current CVD may deteriorate. Cancer can cause various cardiovascular conditions locally (pressure in mediastinum, effusions) or systemically (increased risk of pulmonary embolism, arrhythmias, carcinoid heart disease). Medical cancer therapy can...
