Introduction:Metabolic syndrome is a risk factor for cardiovascular diseases. Higher risk of the metabolic syndrome and its components in patients after kidney transplantation is caused by immunosuppressive therapy. The aim of our study was to evaluate the prevalence of the metabolic syndrome and its components in kidney transplant recipients and to analyse their influence on allograft function and albuminuria. Patients, method and results:In the study we monitored 69 patients after cadaveric kidney transplantation. The prevalence of the metabolic syndrome was 61.3 % 3 years after kidney transplantation. The prevalence...

Vnitr Lek 2014, 60(3):205-211
Treatment of pain is one of the main pillars of treatment of pancreatic diseases. Abdominal pain is a common and often debilitating symptom in patients with chronic pancreatitis and pancreatic cancer. Treatment involves abstinence from tobacco, alcohol and analgetics and adjunctive agents. Surgical and endoscopic treatment requires careful patient selection based on a detailed analysis of ductal anatomy. The limited possibilities of this therapy are patients without dilatation of the main pancreatic duct. Results of randomized trials suggest that the effect of surgical treatment is sustained over time and more efficient than the endoscopic treatment....
Methods of economic analysis of screening programs. Situation in Czech Republic.
Jana Bartáková, Jan Jiskra
Vnitr Lek 2014, 60(3):212-215
In the process of reducing health budget the question of the effectiveness of investment in population-based screening programs is very actual. Because of the scope, screening is becoming very expensive and burdens healthy people. An essential part of any screening program is careful consideration, registration and monitoring of the results. Here economic analysis are irreplaceable. They enable an objective assessment of the effectiveness of the use of resources and thus become an important basis for decision-making about the allocation of scarce resources for improving the health status of the population. The present text deals with the basic types...
Pathogenesis of metabolic changes in acute muscular compartment syndrome
Angela Molčányiová, Theodoz Molčányi
Vnitr Lek 2014, 60(3):216-224
Acute compartment syndrome results from a pressure increase within an anatomically defined space delineated by a non-elastic surrounding. Every muscle group, encased by fascia/bone, presents a compartment with a potential to develop increased pressure under certain conditions. Increase in pressure within the compartment causes a decrease in perfusion pressure, leading to ischemia and pathological utilisation of energy substrates on the cellular level. Initially, the malfunction of cellular metabolism is functional and reversible. Later progress of these changes leads into irreversible myonecrosis. Restitution of blood supply reverses the local ischemia,...

Background:Lymphomatoid granulomatosis (LyG) is a rare multisystemic angiocentric and angiodestructive B lymphoproliferative disease that was first described by Liebow in 1972. Disease was then in the "gray zone" between vasculitis and lymphoproliferative disease. LyG is currently categorized as a primary B lymphoproliferative disease associated with Epstein-Barr (EB) virus according to the World Health Organization (WHO) classification of tumours. Epidemiology, clinical course and treatment:Lymphomatoid granulomatosis is a rare disease with unknown prevalence. It occurs more often in males (male : female ratio 2 : 1) between...

Crush syndrome of today - rhabdomyolysis of intoxicateds
Angela Molčányiová, Theodoz Molčányi, Miroslav Mydlík
Vnitr Lek 2014, 60(3):249-254
Crush syndrome is a major issue in war-afflicted countries and in times of peace also in case of a mining accident. This syndrome is characterized by a number of symptoms originating from muscle damage - rhabdomyolysis - resulting from long-lasting entrapment of the body/extremities. Nowadays, crush syndrome seems to be a scarce condition. However, the rare incidence may rather be the result of poor diagnostic recognition. Although the war conflicts and mining accidents became seldom, increased incidence of rhabdomyolysis is progressively associated with world-wide drug consumption. Long-term immobilisation of intoxicated drug addicts frequently leads...
