Set and methodics:During 2002-2004 years, within the scope of preventive examination, there was made targeted prevention of risk factors of cardiovascular diseases (CVD) at the big hospital employees at the Centre of preventive care of employees and in the department of preventive cardiology of Ist Internal Clinic of LF UP and FN Olomouc. According to the effective tables, there was stated a CVD risk of these employees. The employees received a personal individual letter which informed them of the level of CVD risk and proposed...

Resting energy expenditure during hemodialysis

M. Fořtová, S. Dusilová Sulková, J. Horáček, F. Lopot, T. Zima, J. Bláha, V. Bednářová

Vnitr Lek 2006, 52(1):26-33

Very few studies have so far reported about resting energy expenditure (REE) in chronic renal failure and there is no information available on REE during hemodialysis (HD). Hypothetically, we can expect an increase in REE during HD procedure (due to the inflammatory response to extracorporeal blood circuit). However, such increase in REE could be modified by thermal balance of the procedure. In our study, REE was measured by indirect calorimetry (Deltatrac Datex) in a group of 13 HD patients (7 males and 6 females, mean age 59.8 ± 13.5 years). In each patient, REE was assessed during two HD sessions: one isothermic and one thermoneutral. All...

Objective of the work is to determine the relation of G8002 polymorhism in endothelin 1 gene to the incidence of diabetes mellitus (DM), ischemic disease of lower limbs (ID LL) and myocardial infarction (MI) at the patients with heart failure. Methodics:There were observed 224 patients, 176 males, 48 females, average age 55 ± 12 years, NYHA II/III/IV 82/131/11, average EF LK 25 ± 7 %, diagnosis ischemic heart disease (IHD) 133, dilatation cardiomyopathy (DCMP) 91. Results:Patients with IHD had higher incidence of hypertension (p < 0.0007), diabetes mellitus (p < 0.00007) and hyperlipoproteinemy (p <...

Is the left ventricle enddiastolic pressure pathological in the older population?

M. Vytiska, P. Kala, M. Peciar, O. Boček, P. Jeřábek, P. Neugebauer, M. Poloczek, J. Pařenica, J. Špinar

Vnitr Lek 2006, 52(1):40-43

Introduction:The percentage of older population has significantly increased in the recent decades. Morphologic and functional changes of the cardiovascular system go together with ageing. The aim of the study should show the correlation between the age and left ventricular enddiastolic pressure (LVDEP) value. Methods:106 patients of the age from 23 to79 years without an organic heart disease and the history of hypertension underwent elective coronary angiography including left ventricle angiography between 1999 and 2002. LVEDP was obtained as an average value from 8 consecutive beats without extrasystoles. According...

Vnitr Lek 2006, 52(1):44-50

The objective of the study was to evaluate the physiological effectiveness and the influence of two modifications of aerobic training (interval and continuous) on the physical performance in the patients with coronary heart disease. 38 males with coronary heart disease (age 60 ± 10.2 years) passed three months training programme of 60 min 3 times a week (10 min of warm up phase, 25 min of aerobic phase, 15 min of resistance training, 10 min of relaxing phase). Patients with coronarographically verified stenosis > 50 % luminal diameter and/or left ventricular ejection fraction lower than 40 % (n = 22) had in terms of aerobic phase interval...

Vnitr Lek 2006, 52(1):51-56

Objectives:We assessed the rate and identified risk factors for postprocedure vascular complications following direct percutaneous intervention (dPCI). Methods:Data were collected on 881 consecutive patients who underwent dPCI in our cath-lab from January 2002 to December 2003. Multivariate regression was used to identify characteristics associated with vascular complications. Results:Out of 881 patients, hematoma was found in 148 (16.8 %) cases. Pseudoaneurysm was detected by ultrasound in 40 (4.5 %) patients, 5 (0.6 %) patients underwent surgery. Variables associated with increased risk included age, female...

R. Brůha, P. Drastich, P. Hůlek, J. Lata, J. Petrtýl, V. Procházka, J. Špičák, T. Vaňásek, M. Volfová, P. Zdeněk

Vnitr Lek 2006, 52(1):85-86

Hepatic encephalopathy (HE) is a set of reversible neuropsychic features which occur in connection with hepatic cirrhosis or acute hepatic failure. We distinguish manifest HE (with clinical symptoms) and minimal FE (normal clinical finding, abnormal psychometric or neurophysiologic exam). The diagnosis is clinical or laboratory one. From the auxiliary examinations in common practice the number connection test is sufficient. Therapy: Presence of hepatic encephalopathy should lead to the consideration of the possibility to solve basic disease by hepatic transplantation. Conservative therapy lies in 1. Basic disease elimination, 2. Measures lowering the...

AIP - Atherogenic index of plasma like significant predictor of cardiovascular risk: from research to practice

M. Dobiášová

Vnitr Lek 2006, 52(1):64-71

Background:Various indices have been used for the diagnosis and prognosis of cardiovascular disease (CVD). Atherogenic index of plasma (AIP) is a logarithmically transformed ratio of molar concentrations of triglycerides to HDL-cholesterol. The strong correlation of AIP with lipoprotein particle size may explain its high predictive value. Here we summarize data on AIP calculated in 8394 subjects from 6 population and clinical studies. Results:AIP values increase with increasing CV risk. Thus umbilical cord, young children, healthy women have values below 0.1 while men and subjects with CV risk factors such as hypertension,...

Non fluoroscopic map techniques in contemporary catheter ablation therapy of dysrhythmias

P. Olexa, B. Stančák

Vnitr Lek 2006, 52(1):72-79

Rapid development and clinical introduction of 3D techniques of intracardial mapping significantly extended the possibilities of catheter ablation therapy of dysrhythmias. Contemporary imaging techniques allow precise determination of the source and the way of propagation of dysrhythmia in myocardium, which is an evitable condition of a successful ablation therapy. Although the fluoroscopic image during electrophysiologic examination allows continuous check of the position of catheters in the heart, due to its low resolution it remains just rough estimation of their real position. The objective of the authors of this lucid work was to give a brief...

"Unknown" cause of the onset and recurrence of venous thromboembolism

P. Dulíček, L. Vodičková, J. Malý, M. Pecka, M. Beránek

Vnitr Lek 2006, 52(1):87-88

We reported the case of 61 years old man with the first episode of massive pulmonary embolism at the age of 54 without the proper explanation of the cause. Thrombocythaemia was manifested after the dismissal from the hospital but the origin of this condition was not clarified. Essential thrombocythaemia was diagnosed after the recurrence of venous thromboembolism and that is the explanation for thrombophilic condition in this patient. The patient is without problems and signs of recurrent venous thromboembolism after the achievement of the complete remission of myeloproliferative disease and well - managed of anticoagulation therapy.

Antiphospholipid syndrome - the description of two cases

L. Podrazilová, V. Peterová, M. Olejárová, Z. Seidl, C. Dostál

Vnitr Lek 2006, 52(1):89-94

Antiphospholipid syndrome (APS) often occurs in young people, it is defined by the presence of venous or arterial thromboses, repeated miscarriages, thrombocytopenias and increased levels of antiphospholipid antibodies. Clinical symptoms are different, there is often experienced the phlebothrombosis of lower limbs, miscarriages or neurological symptoms characterized by transient ischemic attacks (TIA). If APS is associated with other system disease, most often with systemic lupus erythematosus (SLE), it is called secondary APS. We present two cases of secondary APS in the work. In first case we describe synchronous occurrence of SLE with secondary...
