Gastroesophageal reflux disease (GERD) is one of the most common diseases affecting upper gastrointestinal tract. It is a chronic disease, whith stadily growing incidence and prevalence in west countries during last 30 years. GERD is caused by pathologic gastroesophageal reflux (GER). GERD includes endoscopically positive, endoscopically negative and extraesophageal reflux disease. Extraesophageal symptoms of GERD have been of a growing attention and discussion during last few years. The most discussed topics are the relation of GERD and bronchial asthma (BA), chronic cough and symptomatology from ear, nose and throught (ENT) regions, but also non...

Probiotics are live cultures of mikroorganisms which are good for human health and are used in human medicine as a treatment. Advantageous effect on a human health was proved in many studies in Europe and in the USA. On the other hand, many studies described lack of information about products containing probiotics - in particular the real bacterial strain or quantity of bacteria. We tried to evaluate thirteen products containing probiotics available in the Czech Republic. In all cases the leaflets contained information about the species and amount of bacteria (from seven products containing more than one species of bacteria were only in two cases declared...

Procalcitonin (PCT) is a highly selective and specific marker for early diagnosis of sepsis. There is enough information confirming that procalcitonin should be considered as an important mediator in the pathophysiology of sepsis. The aim of our work was to evaluate the effect of recombinant human procalcitonin (rhPCT) on phagocytic and candidacidal activity of polymorphonuclear leukocytes (PMNL) in blood, as well as on killing mechanisms of fresh serum and blood against Escherichia coli (E. coli) and Staphylococcus aureus (S. aureus). Our results show that rhPCT dose dependently decreased both phagocytic and candidacidal...

Possibilities of ultrasonographic differentiation of neck and axillary lymphadenopathy
I. Hrazdira, M. Krupová, H. Kyselová
Vnitr Lek 2005, 51(12):1371-1374
Technical development of ultrasonography connected with improvement of resolving power has enlarged possibilities in detection and differentiation of lymph nodes. In the current clinical practice ultrasonic examination is most frequently requested for assessment of lymph nodes in neck and axillary regions in connection with inflammatory or tumour diseases. Although the final diagnosis must be confirmed histopathologically, there exist some echographic criteria enabling with a great probability to appoint the malignant character of lymph nodes: round shape expressed by the shape index near to 1, sharp edges, heterogeneous echostructure and mainly loss...
Idiopathic pulmonary fibrosis

Vnitr Lek 2005, 51(12):1375-1384
Idiopathic pulmonary fibrosis (IPF), known in Europe as cryptogenic fibrosing alveolitis, is a rare, progressive and usually fatal form of idiopathic interstitial pneumonia. IPF is characterized by failure of alveolar re-epithelization, persistence of fibroblasts, deposition of extracellular matrix, and distortion of lung architecture which ultimately results in respiratory failure. Current consensus statements reserve the term IPF to refer to a specific clinical entity associated with the histopatological pattern of usual interstitial pneumonia (UIP). UIP is characterized by temporal heterogeneity, with alternating areas of interstitial fibrosis,...
The idiopathic hypereosinophilic syndrome and chronic eosinophilic leukemia
L. Chrobák, J. Voglová
Vnitr Lek 2005, 51(12):1385-1393
Idiopathic hypereosinophilic syndrome is a heterogenous group of hematological disorders characterized by eosinophilia (> 1.5 x 109/l) persistent for more than 6 months, exclusion of reactive eosinophilia from other causes, such as parasitic infections or allergy, and evidence of end-organ damage [22]. According to World Health Organization the exclusion includes all neoplastic disorders in which eosinophils are part of the neoplastic clone. Excluded should be also T cell population with aberant phenotype and abnormal cytokine production [7], recently considert also as "lymphocytic" variants of the HES [42]. HES has to be reclassified...
