[Incidence of selected antiphospholipid antibodies in a group of patients with systemic lupus erythematosus].

Z Fojtík, M Beránek, M Klabusay, Z Korístek, A Buliková

Vnitr Lek 2004, 50(4):267-273

Antiphospholipid antibodies (APLA) present very heterogeneous groups of antibodies which can significantly and in various ways influence processes on different levels of coagulation cascade. Their presence can be accompanied with repetitive venous and arterial thromboses, recurrent loses of foetus, and thrombocytopenia. Incidence of these thrombotic disorders was monitored in a group of 46 patients with systemic lupus erythematodes (SLE). Positive lupus anticoagulant (LA), antiphospholipid antibodies complex, and thrombocyte counts were assessed. Thrombotic disorders were assessed in a retrospective analysis. In the LA+ group 62% of patients had history...

Vnitr Lek 2004, 50(4):274-277

INTRODUCTION: Acute upper gastrointestinal tract bleeding is a cause of significant morbidity and mortality and is a reason for urgent endoscopy. Besides an age and associated diseases, prognosis of patients influence also localisation and type of bleeding. The aim of our retrospective analysis was to discover causes of bleeding into upper GI tract and its characteristics over a 4 year period.METHODOLOGY: A survey of urgent upper GI tract endoscopies in the Clinic of Internal Medicine in Motol in Prague because of an acute GI tract bleeding (hematemesis or melena) was done. Found ulcers were assessed using Forrest classification. Moreover, number...

[Interatrial and atrioventricular conduction in dual chamber stimulation].

T Minarík, R Stípal

Vnitr Lek 2004, 50(4):278-281

SAMPLE AND METHODOLOGY: 111 patients (54 men) with atrioventricular stimulation of an average age 56.5 +/- 14.3 were examined. Permanent cardiostimulation was indicated in 56 patients for sick sinus syndrome and in 55 patients for complete AV blockade. Average LV EF was 52.1 +/- 5.7% and LA endsystolic dimension was 39.8 +/- 4.0 mm. All patients were done esophageal electrocardiography under synchronous atrioventricular stimulation regimen. Values of interatrial conduction were compared in relation to placement of stimulation electrode in atrium.RESULTS: Interatrial conduction in the whole sample was 79.8 +/- 19.5 (40-150) ms, for placement of...

[Serum leptin levels in patients with heroin addiction, the effect of one year of methadone substitution].

H Wilczek, M Haluzík, J Svobodová, J Parízková

Vnitr Lek 2004, 50(4):282-285

BACKGROUND AND AIM: The aim of the study was to explore the influence of heroin addiction and one year methadone maintenance treatment respectively on serum leptin levels.METHODS AND RESULTS: 14 heroin addicted subjects (9 men and 5 women) with average age 27.1 years (variation 22-38 years) and 17 control healthy subjects were included into the study. Mean duration of heroin addiction was 9 years. The subjects were treated for one year with mean daily dose of 90 mg of methadone. Serum leptin levels were measured by enzymoimmunoassay, the rest of the biochemical parameters was measured by standard laboratory methods. Body Mass Index (BMI) was calculated...

[Intensive vitamin D supplementation in the treatment of osteoporosis].

K Stefíková, K Chylová, Z Krivosíková, V Spustová, R Dzúrik

Vnitr Lek 2004, 50(4):286-290

Vitamin D deficiency is not possible to correct with the nutritional vitamin D doses in postmenopausal women with decreased bone mineral density. The aim of study was to evaluated the effectivity and safety of 15,000 IU/week vitamin D administrated in 52 postmenopausal women with osteopenia or osteoporosis. Patients were divided into two groups. Treated group was supplemented by calcium 0.5 g/d and 25-hydroxycholecalciferol 15,000 IU/week and control group was supplemented by calcium and placebo for two months. Plasma calcium concentration did not change in the vitamin D treated group while it decreased (p < 0.001) in the control group. Neither...

[Diagnosis of magnesium deficiency in the body, personal experience].

Z Wilhelm, P Hegyi, J Kleinová, R Kalábová, A Pechová, R Urbanová

Vnitr Lek 2004, 50(4):291-299

Magnesium as a component of a range of enzymatic systems is a very important intracellular cation in the organism. Its monitoring is limited in many observations only to determination of its concentration in blood serum. We have done an analysis of serum and erythrocyte magnesium concentrations in 23 healthy women, 70 healthy men and in 184 patients with ulcerative disease of gastroduodenum. It was proved in all the monitored groups that erythrocyte concentrations of magnesium were lower compared to values which were estimated from serum concentrations in 9 from 23 healthy women, in 3 from 60 healthy men, in 10 from 101 ill men with ulcerative gastroduodenal...

[Diurnal rhythm of serum leptin in patients with solid tumors].

A Gaja

Vnitr Lek 2004, 50(4):300-304

Leptin is a protein produced by fat tissue. It has many regulatory effects in the area of energetic metabolism, immunity and haematopoiesis. Its role in tumour diseases especially in states with cachexia is studied. Its physiological diurnal rhythm is very well known. There is lack of information about diurnal rhythm of leptin in tumour diseases except some endocrine tumours. In this study 10 patients with breast and colorectal cancer without marks of kachexia and mostly without pre-existing chemotherapy were examined. The diurnal rhythm of leptin was preserved in cancer patients (morning minimum 10.5 +/- 4.2 ng/ml and nocturnal maximum 17.9 +/- 10.1...

[Endothelial injury and activation of the coagulation cascade during radiofrequency catheter ablation].

A Bulava, L Slavík, M Fiala, P Heinc, L Luběna, J Lukl, V Krcová, K Indrák

Vnitr Lek 2004, 50(4):305-311

GOAL OF STUDY: To identify the extent of systemic activation of the coagulation cascade and to evaluate thrombogenic effect of the radiofrequency catheter ablation.METHODS AND RESULTS: Markers of activation of the coagulation cascade (D-dimers [DD]), markers of activation of the fibrinolytic system (tissue plasminogen activator [t-PA] and its inhibitor [PAI-1]), and markers of endothelial damage (von Willebrand factor [vWf]) were monitored in 50 patients undergoing catheter ablation. Levels of these substances were identified in time T0--at the beginning of the examination, T1--after finishing diagnostic part of the electrophysiological study,...

[The significance of antiphospholipid antibodies].

Z Fojtík

Vnitr Lek 2004, 50(4):312-317

Antiphospholipid antibodies (APLA) present very heterogeneous groups of antibodies which can significantly influence processes on different levels of coagulation cascade depending on effects of phospholipid surfaces on blood coagulation. This usually leads to a particular level of thrombophylia. Clinical syndrome accompanying positive APLA, such as antiphospholipid syndrome, was defined by clinical and laboratory symptoms. This clinical syndrome can be a primary syndrome, if other disorders with ability to induce generation of antibodies can be excluded, or a secondary syndrome. The most often in cases of systemic tissue disease. APLA can be divided...

[An unusual cause of acute anuric renal failure].

Z Ríhová, E Honsová, I Spicka, J Zabka, M Merta, V Tesar

Vnitr Lek 2004, 50(4):318-320

We present a case of a 79-years old woman with acute anuric renal failure due to biopsy confirmed immunotactoid glomerulonephritis. Despite biclonal gammopathy IgG lambda, no hematologic or any other underlying malignancy was found. A possible association with viral hepatitis, cryoglobulinemia or autoimmune disease was also excluded. The patient was treated with pulse i.v. dexamethasone with very good results. Diuresis resumed and renal function gradually normalized. Our patient is one of the oldest patients with immunotactoid glomerulonephritis described. The presentation by acute renal failure with an improvement of renal function after therapy has...

A Bulava, M Skvarilová, D Marek, E Kociánová, J Lukl

Vnitr Lek 2004, 50(4):321-324

The case report describes a rare cause of acute myocardial infarction. 68-years old man with the cancer triplicity (follicular and papillar carcinoma of the thyroid gland, Grawitz tumor, bronchogenic carcinoma with generalization) was admitted to the intensive care unit for the sudden onset of chest pain, positivity of cardioselective enzymes and signs of cardiogenic shock. Echocardiographic examination proved diaphragmatic akinesis with low left ventricular ejection fraction about 30%. The acute coronary angiogram revealed unimportant atherosclerotic narrowing of the left coronary artery and a long significant stenosis in the proximal portion of the...

[The first conferences of dialysis nurses and staff in Czechoslovakia].

K Opatrný

Vnitr Lek 2004, 50(4):325-328

After successful seminars of dialysis nurses from the Clinic of Internal Medicine in Plzen in 1974, 1975 and 1976 the Board of the Czechoslovak Society of Nephrology entrusted the Dialysis Centre from the Clinic to organise national conferences for dialysis staff. The first conference of dialysis staff took place in 1978 in the hotel Solidarita at Spicák. Following conferences were taking place in the manor Zinkovy near Plzen till 1989. To establish independent dialysis conferences was necessary because of a rich content of those conferences and an effort not to limit traditional nephrology at nephrology congresses. The conferences always took from...
