[Impact factors and bibliometrics of science. Does pure science really exist?].
J Sochman, A Belán
Vnitr Lek 2003, 49(1):3-9
The impact factor is an artificially created indicator which when applied separately does not achieve the value attributed to it. In its assessment a number of different influences are projected which cause partial mistakes with a cumulative effect. It can have an even worse effect if the impact factor is incorrectly conceived or inadequately handled. Only in a long-term review using specific corrections it can serve to determine really top class periodicals but only within the framework of a single scientific discipline. It is not suitable for comparison of interdisciplinary journals. It should be used very carefully in the evaluation of authors....

[Dynamic changes in the QT interval during the head-up tilt test in patients with vasovagal syncope].
P Mitro, Z Kropuchová, D Pella, D Trejbal
Vnitr Lek 2003, 49(1):27-31
Dynamic changes of the QT and QTc interval as well as QT dispersion and QTc dispersion during the head-up tilt test were investigated in 15 patients (8 men, mean age 32 years) with vasovagal syncope (VVS) and a positive head-up tilt test and in a control group of 15 patients with syncope in the case-history and a negative head-up tilt test (9 men, mean age 33 years). The value at rest of the QT interval did not differ in patients with VVS and controls. In controls at the beginning of HUT shortening of QT occurred (0.447 sec. vs. 0.419 sec. p = 0.0002), subsequently the QT did not change significantly. In patients with VVS during the beginning of the...

Vnitr Lek 2003, 49(1):32-36
One-hundred-ninety-seven osteopenic or osteoporotic patients registered at the University hospital osteology outpatient clinic during 1995-1999 and exposed to glucocorticoids were included in the cohort. Prevalence of glucocorticoid treatment (of all patients) was 12.4%. The patients diagnosed with rheumatoid arthritis showed the greatest reduction in bone density (average T-score = -2.70), fractures were most frequently recorded in patients suffering from asthma or lupus erythematodes.
[Treatment of atrioventricular accessory pathways using catheter ablation: analysis of immediate and long-term results at the end of the learning curve period].
M Fiala, P Heinc, A Bulava, J Lukl
Vnitr Lek 2003, 49(1):37-44
UNLABELLED: Immediate and long-term results of catheter ablation of atrioventricular accessory pathways (AP) are presented.METHOD: One hundred and seventy-one patients aged 41.8 +/- 14.4 years underwent catheter ablation of 179 AP. Right and left AP occurred simultaneously in 3 patients; thus 65 ablation procedures for 57 rights AP in 57 patients were compared with 125 ablation procedures for 122 left AP in 117 patients.RESULTS: Including repeated procedures ablation was successful in 116 (99%) patients with left AP and in 57 (100%) patients with right AP. First catheter ablation failed in 4 (7%) patients with right AP and in 4 (3.4%) patients...
[Tubular function after orthotopic liver transplantation].
H Gottfriedová, O Schück, J Malý, M Stollová, P Trunecka, J Spicák, I Brůzková, J Skibová
Vnitr Lek 2003, 49(1):45-50
Renal function after orthotopic liver transplantation (OTL) is very frequently reduced and its level exerts a significant effect on the morbidity and mortality of these subjects. One of the main factors with a negative impact on renal function after OTL is the nephrotoxic action of cyclosporin A (CsA). Renal function after OTL is usually evaluated on the basis of glomerular filtration (GF). As chronic nephrotoxicity of CsA is manifested in the histological picture by significant tubulointerstitial affection, in 75 subjects after OTL the spontaneous concentrating and acidifying capacity of the kidneys was investigated. The value of urine osmolality...
[Effect of delaying reperfusion therapy with PTCA on long term prognosis in patients with acute myocardial infarct].

Vnitr Lek 2003, 49(1):51-60
BACKGROUND: The benefit of thrombolysis in patients with acute myocardial infarction (AMI) strongly depends on the time from onset of symptoms to the initiation of treatment. For AMI patients treated with PTCA this time seems to be important only to a certain time level. The aim of this study was to assess the influence of time to treatment of AMI with coronary angioplasty on short term and long term prognosis.METHODS: We followed 339 consecutive AMI patients treated with coronary angioplasty from 1995 to 1999 in a cardiac care unit. Patients were divided to five groups according to time to treatment.RESULTS: Time to treatment < 90 min....
[The sympathetic nervous system and insulin resistance].
H Rosolová
Vnitr Lek 2003, 49(1):61-65
The submitted paper deals with the relationship of the sympathetic nervous system, metabolic syndrome of insulin resistance and cardiovascular risk. The author describes the structure and function of the sympathetic nervous system--the central part of the sympathetic nerve is in the hypothalamus and medulla oblongata. The hypothalamus combines vegetative control of the cardiovascular system and processes which influence the intermediary metabolism. This contributes to the explanation of the frequent incidence of metabolic disorders in subjects with arterial hypertension or ischaemic heart disease. The increased activity of the sympathetic nervous system...
[The CD nomenclature of leukocyte antigens].
K Koubek
Vnitr Lek 2003, 49(1):66-72
The author gives the basic information about human leucocyte antigens of haematopoietic cells. At present, the membrane markers (antigens) included into CD nomenclature are gradually detected, classified and ranged, many of them being well defined as to genetic determination, chemical structure and function.
[An unusual manifestation of tuberculosis in a female patient after kidney transplantation. Case report].
S Kukura, O Viklický, J Rudis, E Honsová, J Lácha, V Teplan
Vnitr Lek 2003, 49(1):73-76
Organ transplant recipients are at the increased risk of infection complications. Herein, we present a case of unusual localisation of tuberculosis in renal allograft recipient treated by combination of cyclosporine A, mycophenolate mofetil and steroids. After the non specific prodromes, surgery due to ileus discovered ileocaecal tumour. Microscopically tuberculosis inflammatory process with ulcerations of the intestinal mucosa and classification of the lymphatic nodes, instead of the expected lymphoma was proved to be present. We are discussing the incidence of tuberculosis in relation to immunosuppressive therapy as well as possible pathways of tuberculosis...

The authors describe the disease of a 22-year-old woman treated from the age of 13 years on account of Hodgkin's lymphoma by irradiation and cytostatic treatment. On account of a relapse of lymphoma at the age of 14 years megachemotherapy with subsequent transplantation of autologous bone marrow. In the course of eight years of the follow up gradual development of constrictive pericarditis with exsudate. Concurrently progression of mitral insufficiency based on valvular prolapse resulting from radiation. During the last two years refractory systemic hypertension resistant to treatment. At the peak of the disease development of cardiac tamponade and...
