[Changes of haemostasis in obese subjects during weight reduction].
J Malý, H Malá, M Pecka, Z Zadák, L Sobotka
Vnitr Lek 2001, 47(4):203-209
In a group of 126 obese subjects hospitalized at the obesitological unit of the Gerontological and Metabolic Clinic, Faculty Hospital Hradec Králové the authors confirmed the effect of short-term modified fasting and a low energy diet on the body weight and body mass index They also proved the influence of weight reduction on normalization of cholesterol and LDL-cholesterol and triacylglycerols. They did not prove however the influence of short-term modified fasting and a low energy diet on the concentration of some coagulation pareameters (fibrinogen, D-dimers, antithrombin). In the patients a decline of the fibrinolytic activity was observed and...

Vnitr Lek 2001, 47(4):210-214
One tablet of Sorbifer Durules contains 100 mg Fe2+ and 60 mg vitamin C. The authors examined in a short-term study 24 haemodialyzed patients with chronic renal failure of different etiology. The investigation was divided into three parts. During the first 4 weeks the patients did not receive Fe2+ nor vitamin C. During the subsequent four weeks the patients had Sorbifer Durules, one tablet/24 hours. This period was followed by another four weeks when the patients went again without Fe2+ and vitamin C treatment. At regular intervals, i.e. on days 0, 28, 56 and 84 the authors assessed the packed cell volume, blood haemoglobin and serum iron level, the...
[Hospitalization of discharged patients with acute myocardial infarction in the thrombolytic era].
S Cagán, S Wimmerová, I Besedová, T Trnovec
Vnitr Lek 2001, 47(4):215-222
The objective of the work is to assess in discharged patients with AIM data on the period of hospitalization in the intensive care unit and the total period of hospitalization and how these were influenced by stratification into patients with complicated and uncomplicated AIM and patients with a low, medium and high risk. Data on hospitalization were analyzed in 2,527 discharged patients with AIM. The median of hospitalization at intensive care units was 5 days and the mean period of hospitalization 6.35 days. The significantly longer hospitalization (p < 0.001) in patients with complicated AIM (median 6 days) as compared with patients with uncomplicated...
[Importance of assessment of Helicobacter pylori antigen in faeces].
P Vlcková, A Hep, J Dolina, H Stroblová, P Dítě
Vnitr Lek 2001, 47(4):223-226
Diagnostic methods for the detection of Helicobacter pylori (H.P.) can be divided into invasive and non-invasive ones. Among non-invasive methods we can include now also detection of H.P. antigen in faeces. The objective of our study was to evaluate the sensitivity ans specificity of the test when monitoring the eradication therapy, as compared with another non-invasive test and to test the method in common clincal practice. H.P. positivity was based on the result of the histological examination and CLO test. Repeatedly samples of faeces were collected to detect H.P. antigen in faeces before the onset of and after eradication treatment. For detection...
[Contribution of coloured duplex ultrasonography to preoperative examination of patients with chronic venous insufficiency].
D Musil, J Herman
Vnitr Lek 2001, 47(4):227-231
In a prospective study the authors examined by coloured duplex sonography 76 patients (93 extremities) with chronic venous insufficiency (CVI), before a planned operation of the veins of the lower extremities. Attention was focused on the site of the reflux ( superficial, deep venous system, perforators) and the importance of perforators and congenital venous anomalies in CVI. In the investigated group of patients with CVI a combined reflux was present in several venous systems concurrently, confirmed in 69 extremities (74.2%). In 25 extremities the authors found insufficiency of the saphenofemoral junction along with femoropopliteal reflux. This high...
[HOTEL P--Hypertension Optimal Treatment with Enalapril Lachema-Pliva. Optimal treatment of hypertension with Enalapril Lachema--Pliva].
J Spinar, J Vítovec
Vnitr Lek 2001, 47(4):232-237
The authors investigated in a one-year multicentric open study the effect of enalapril (Enalapril Lachema-Pliva) in 653 patients with mild to moderate hypertension treated in ambulance of 128 general practitioners and specialists in internal medicine. The blood pressure was checked after 6 weeks during the first six months and every 3 months during the second six months. Normal levels of diastolic pressure were achieved in 576 (82%) patients, whereby a combination of antihypertensive drugs had to be used in 383 (59%) patients. Complete renal functions were assessed in 51 patients, microalbuminuria declined from 91.8+/- 37.8 to 72.1 +/- 13.1 mg/l (p...
[Diabetes mellitus and the liver].
M Brodanová
Vnitr Lek 2001, 47(4):238-244
Liver diseases are frequently associated with disorders of the carbohydrate metabolism--impaired glucose tolerance, hyperinsulinaemia, insulin resistance. Impaired glucose tolerance is due in particular to impaired glucose uptake in the splanchnic area and periphery. Glucose production by the liver is normal, similarly as insulin secretion. Its reduced clearance leads to hyperinsulinaemia with subsequent down regulation of insulin receptors and the development of insulin resistance. In diabetic subjects hepatomegaly is frequent, most frequently associated with steatosis of the liver. It correlates with the degree of obesity rather than the type of...
[The importance of leptin in oncology--hypothesis or facts?].
A Gaja, Z Churý
Vnitr Lek 2001, 47(4):245-249
Leptin was described as a protein produced mostly by adipocytes which participates in regulation of caloric intake. This protein has further many physiological regulatory functions especially in hematopoesis and reproduction. The role of leptin in diseases is also considered. Maybe leptin is a part of the pathogenic mechanism or its metabolism can be influenced secondary to disease. Malignant tumours are the important area where leptin can acts. This imagination is supported by recent in vitro studies and some clinical observations. It is consider that some parameters of leptin metabolism can serve as usable marker for diagnosis and monitoring of some...

[Diagnosis of thyreopatic diseases by laboratory examinations--possibility for rational economy].
J Bielik, J Vindis, V Voglová, J Kausitz, J Lepej
Vnitr Lek 2001, 47(4):253-256
This paper is oriented on the possibility of racional economy of laboratory diagnostical process of thyreopathies. The study was based on analysis of 29,683 examinations made at biochemical department in hospital Trencín. The overuse--mostly of antibodies and also, but less of thyreoidal hormones was discovered, mostly in the group of physicians in general practise and less in hospital and internal practise. The absence of initial algoritmus in thyreoidology is documented. The followed approach was formulated as new proposal: the first step is the examination of TSH (by ultrasensitive method) with possibility for all groups of physicians. Clinical...
