[Serum cystatin C in haemodialysed patients].
P Pontuch, A Lehotská, V Parrák, L Mar»áková, J Fekete
Vnitr Lek 2001, 47(1):4-9
BACKGROUND: Cystatin C (CyC) is protein (m. w. 13 300), which is produced by nucleated cells, filtered through glomeruli and subsequently reabsorbed and degraded in tubuli.OBJECTIVE: To investigate changes in serum CyC concentration during two low-flux membrane haemodialyses (HD-I, HD-II) and relationship between serum CyC a creatinine concentrations.PATIENTS AND METHODS: Serum CyC was determined in 17 patients on chronic haemodialysis during HD-I and HD-II by immunonephelometric method (Dade Behring, Austria).RESULTS: Significant correlation between serum CyC and creatinine before HD-I and before/after HD-II was found (p < 0.001)....
[Cystatin C in estimates of glomerular filtration in patients with renal disease--initial experience].
M Hertlová, S Surel, M Senkyrík, E Králová, H Novotná
Vnitr Lek 2001, 47(1):10-16
Cystatin is reported in the literature with increasing frequency as a new reliable parameter for estimates of glomerular filtration. The authors examined cystatin C by the PET method of DAKO Co. in 151 patients from the nephrological out-patient clinic. 91 patients had normal renal functions, 60 suffered from renal insufficiency of different severity. Between cystatin C and glomerular filtration assessed by creatinine clearance was a close correlation, R = -0.787 according to Pearson. The authors evaluated separately a group of 36 patients with glomerulonephritis, 34 diabetic patients with diabetic nephropathy, 38 patients with tubulointerstitial nephritis...
[Autoantibodies in relatives of IgA-deficient patients].
J Litzman, D Bartonková, D Stikarovská, Z Pikulová, J Lokaj
Vnitr Lek 2001, 47(1):17-19
Selective IgA deficiency is frequently associated with autoimmune phenomena. We addressed the question, whether autoantibodies are more frequent in relatives of IgA deficient persons. Fifteen first-degree relatives of patients with a familial form of IgA deficiency and 129 first-degree relatives of patients with a sporadic form of IgA deficiency were evaluated. Following autoantibodies were detected: antinuclear, anti-thyreoglobulin, anti-thyroid microsomal, anti-gastric parietal cell, anti-reticulin, anti-smooth muscle, anti-mitochondrial, anti-neutrophil cytoplasmatic, anti-cardiolipin, rheumatoid factor and antibodies against gliadin. We did not...
[QT dispersion intervals in hypertensives with left ventricular hypertrophy].
M A Ghanem Wisam, J Murín, J Jaber, A Radman, J Bulas, K Kozliková
Vnitr Lek 2001, 47(1):20-24
Left ventricular hypertrophy is an important risk factor of cardiovascular complications during the course of hypertension. Increased QT dispersion is associated with sudden cardiac death in congestive heart failure and in other cardiovascular diseases. Our aim was to compare QT dispersion from routine ECG in hypertensive patients with and without left ventricular hypertrophy defined by echocardiography. Authors examined 71 hypertensives treated in our medical department. Left ventricular hypertrophy was defined by echocardiography (Penn convention) as left ventricular mass index > 134 g/m2 in men and > 110 g/m2 in women. QT dispersion was defined...
[Multidrug resistance of leukemia cells to cytostatics--the functional fluorescent dye efflux assay evaluated by flow cytometry].
K Jankovicová, J Krejsek, O Kopecký
Vnitr Lek 2001, 47(1):25-29
The functional fluorescent dye efflux assays are used in the study of the multidrug resistance of the malignant cells to the cytotoxic drugs which is caused by the overexpression of P-glycoprotein (P-gp) or another membrane transport system--a multidrug resistance related protein (MRP). P-glycoprotein and a multidrug resistance related protein are involved in the efflux of cytotoxic drugs out of the cell and are responsible for the resistance. Fluorescent dye efflux mediated by these proteins could be evaluated by the flow cytometry. This test seems to be an optimal approach to study the multidrug resistance. With help of the specific inhibitors such...
[Suggestions for standardized collection of morning urine for assessment of NTx levels in patients with osteoporosis].
J Wendlová
Vnitr Lek 2001, 47(1):30-35
UNLABELLED: The authors examined 79 patients (n = 79) with primary and secondary osteoporosis, incl.31 men (nM = 31) aged 31-61 years and 48 women (nF = 48, aged 33 to 65 years. In these patients they assessed NTx (amino-terminal telopeptide of type I collagen of the Ist organic bone matrix) in 24 (NTx24) and 2-hour morning (NTx2) diuresis (from 6 to 8 a.m.).OBJECTIVE: 1. To assess the relationship between values of NTx in th suggested 2-hour diuresis and NTx values in the standard 24- hour diuresis differentiated by sex. 2. To compare the ratio (index) of widths of 95% confidence intervals (CI) for NTx2 and NTx24 with the ratio (index) of reference...
[Clinical manifestations of AL-amyloidosis and some other types of amyloidosis].
Z Adam, V Scudla
Vnitr Lek 2001, 47(1):36-45
Amyloidosis is the term for specific pathological peptide deposits in various tissues. Amyloid substances may be the manifestation of the following nosological units: AL-amyloidosis, AA-amyloidosis, ATTR-amyloidosis, beta2-microglobulin amyloidosis and possibly othr familiar forms of amyloidosis. The most frequent symptoms of amyloidosis are: 1. proteinuria with nephrotic syndrome, 2. cardiac failure with restrictive cardiomyopathy, 3. unexplicable hepatomegaly, 4. idiopathic peripheral polyneuropathy, 5. haemorrhagic manifestations and symptoms of increased fragility of the capillaries while the number of thrombocytes is normal, 6. periorbital haematomas...
[Treatment of AL-amyloidosis and some other types of amyloidosis].
Z Adam, V Scudla, M Tomíska
Vnitr Lek 2001, 47(1):46-52
Patients with AL-amyloidosis are treated at present by conventional or large-dose chemotherapy with autologous transplantation of haematopoietic stem cells, similarly as patients with multiple myeloma. Only the timing of treatment is different. In myeloma treatment is usually indicated only in clinical stage II or III, in clinical stage I only in case of adverse prognostic factors. In AL-amyloidosis treatment is indicated immediatelyafter establishment of the diagnosis. It was found that high-dose chemotherapy with autologous transpantation of haematopoietic cells is at present the most effective treatment for selected patients with not advanced forms...
[Viral hepatitis of patients in a regular haemodialysis programme].
F Svára, P Urbánek, S Sulková
Vnitr Lek 2001, 47(1):53-59
After cardiovascular diseases and bacterial infections viral hepatitis is the most frequent disease which complicates haemodialyzation treatment of patients with chronic renal failure. Substitution of renal function is for these patients a life saving procedure. It is, however, complicated treatment associated with various risks of acute and chronic complications. The prevalence of parenterally transmitted viral hepatitis in the population of haemodialyzed patients is by far higher than the prevalence of these diseases in the general population. There are several reasons for this condition. In addition to the character of this treatment there is also...
