B Bendlová, I Mazura, J Vcelák, T Pelikánová, M Kunesová, V Hainer, J Obenberger, D Palyzová

Vnitr Lek 1999, 45(5):267-272

UNLABELLED: A missence mutation Trp64Arg in the beta3-adrenergic receptor gene is associated with obesity, insulin resistance, a lower metabolic rate and the earlier onset of NIDDM but the published results are controversial. We investigated the effect of this mutation on insulin resistance (euglycemic hyperinsulinemic clamp), on fat mass and fat distribution (anthropometry, bioimpedance, CT) and resting metabolic rate (indirect calorimetry), lipid spectrum and other metabolic disturbances in Czech juveniles recruited from juvenile hypertensives (H, n = 68) and controls (C, n = 81). The frequency of this mutation (determined by digestion of 210 bp...

[Prevention of venous thrombosis and pulmonary embolism. Scoring system of the risk of TEN development].

M Pacejka, A Adamíková

Vnitr Lek 1999, 45(5):273-276

The authors summarize hitherto used patterns of thromboprophylaxis. They attempted to quantify the risk of development of thromboembolism by means of a "Scoring system of the risk of development of TEN" which facilitates the decision on the intensity of prophylaxis of thromboembolic disease.

[Endosonographic picture of the esophagus and paraesophageal lymph nodes in subjects with oesophagitis].

I Novotný, H Münzová, P Dítě

Vnitr Lek 1999, 45(5):277-280

UNLABELLED: Echocardiographic examination (EUS) gives a good overview of the structure of the whole oesophagus and the paraoesophageal space. To the EUS picture of reflux oesophagitis and normal EUS picture of paraoesophageal lymph nodes relatively few publications are devoted. The objective of the present work is to describe ultrasonographic (USG) signs of endoscopically and histologically confirmed oesophagitis (grade III and IV) and compare them with a control group, describe the USG picture of paraoesophageal lymph nodes in subjects with oesophagitis and in controls. The authors examined, using an Olympus GIF-UM20 instrument with a radial probe...

[Vitamin D deficiency as one of the causes of bone changes in chronic pancreatitis].

J Payer, Z Killinger, S Aleryany, M Kratochvíµová, P Ondrejka

Vnitr Lek 1999, 45(5):281-283

Chronic pancreatitis is a longlasting inflammatory disease manifested clinically in the advanced stage by malabsorption syndrome. Its manifestations include also changes in the calcium metabolism and the occurrence of osteoporosis and osteomalacia or their combination. The objective of the study was to assess the vitamin D3 blood concentration in patients with chronic pancreatitis. The group comprised 15 patients (8 men and 7 women), median age 45.0 years. The authors found a significantly reduced serum concentration of vitamin D3 (p < 0.01) in patients with chronic pancreatitis. They assume that vitamin D deficiency is one of the decisive causes...

[Radiosurgery with the Leksell gamma knife in the treatment of solitary brain metastasis--5-year results].

G Simonová, R Liscák, J Novotný, J Novotný

Vnitr Lek 1999, 45(5):284-290

During five years (1992-1997) 215 patients with solitary brain metastases were treated in the Department of Stereotactic and Radiation Neurosurgery (117 men and 98 women). The most frequent first neurological symptoms were: headache, hemiparesis, seizure, mental and behavioural disturbances. Location of lesion was supratentorial in 163 patients (75.8%) and infratentorial in 52 cases (24.2%). Treatment results: complete and partial regression in 175 patients (81.4%), stabilisation in 30 of patients (14.0%), local progression in 10 patients (4.6%). Local recurrence after initially effective treatment was observed in 11 pts (5.1%). The severity of neurological...

[Latent tetany in patients with a negative coronarographic finding].

E Sovová, M Skvarilová, J Bartousek, V Doupal, J Lukl, M Urbánek

Vnitr Lek 1999, 45(5):291-294

UNLABELLED: Chest pain, constriction of the throat and palpitations are symptoms common to tetany and ischaemic heart disease. The objective of the work was to assess the latent prevalence of tetany in patients with typical chest pain and a negative outcome of coronarographic examination. The group comprised 20 patients (5 men and 15 women), mean age 54 years (28-74) with a history of chest pain, who had selective coronarography with a negative or minimal (up to 30% stenosis) outcome. In this group EMG, the ischaemic and hyperventilation test for latent tetany was made. A metabolic cause of tetany was ruled out. The results of the EMG test were positive...

[Evaluation of nutritional state in hospitalized patients with cerebrovascular stroke].

J Charvát, H Kubátová, J Vecer, M Kvapil

Vnitr Lek 1999, 45(5):295-297

The cerebrovascular stroke is serious disease with the high mortality and resulting in invalidity. The result of its treatment is not yet encouraging. The stress situation which is characteristic for the acute stage of disease leads to catabolic status. This condition can be worsen if nutritional parameters are not properly monitored and the nutrition is not sufficient. We examined 30 patients admitted to our medical ward with the diagnosis of ischemic cerebrovascular stroke. We have not applied any nutritional intervention. We measured some anthropometric, biochemical and imunological parameters of nutrition at the time of admission to ward and after...

[Agrobacterium tumefaciens (Radiobacter) as an infectious agent in an oncological patient].

H Franková, Z Churý, V Gaislerová, J Jílková, N Hejlová, J Mach

Vnitr Lek 1999, 45(5):298-300

The authors submit the description of a 62-year-old patient with multiple myeloma where the causal agent of pyretic reactions was Agrobacterium tumefaciens (radiobacter). It was a patient with an implanted venous port which was colonized by the above bacterium. This finding most probably has not been described so far in the Czech literature. In the English literature the authors found 36 cases. The authors draw attention to the possible higher incidence of future infections caused by organisms hitherto considered non-pathogenic for man, in particular in immunocompromised patients.

[Thyrotoxicosis due to long-term therapy with interferon alpha in the patient with mixed chronic active hepatitis B and C].

J Vortel, J Bures, S Rejchrt, P Zivný, V Palicka

Vnitr Lek 1999, 45(5):301-304

The authors describe a case report of 48-year-old woman treated with Interferon alpha for the chronic active hepatitis B and C. During this therapy thyrotoxicosis occurred. It disappeared after the withdrawal of Interferon alpha and administration of strumigens. Immediately after stopping of the administration of Interferon alpha and three month later antibodies against the thyroid epithelium were found.

[Esophageal reflux disease--comments on confusion in terminology, diagnosis and therapy].

K Lukás, A Hep

Vnitr Lek 1999, 45(5):305-309

Oesophageal reflux disease is a serious condition with an impact on the entire population. The provoking factor of the disease is gastroesophageal reflux which itself is not a disease but a normal physiological process. Reflux is described as pathological it is damages the oesophagus and respiratory tract. Oesophageal reflux disease develpomeps when antiferlux mechanisms fail, it is the consequence of impaired motility where the crucial role is played by dysfunction of the lower oesophageal sphincter. The most frequent consequence and manifestation of gastrooesophageal reflux is reflux oesophagitis which may be macroscopically obvious (endoscopically...

[Involvement of the cardiac muscle in anemia].

L Elbl, M Penka, V Chaloupka, I Vásová, I Kiss, J Jancík, J Vorlícek

Vnitr Lek 1999, 45(5):310-314

Anemia is probably the most widespread syndrome associated with a very wide spectrum of pathological conditions with a different genesis. In malignities and their treatment it is one of the very serious complications the impact of which is manifested in particular in the state of the circulation. Thus anemia can cause deterioration of primary cardiac and cardiovascular complications which develop either as a result of the malignant disease or as a result of its treatment by cardiotoxic drugs. Thus risk conditions with the functional consequence of cardiovascular impairement are potentiated. This can complicate the disease or limit therapeutic possibilities....

[Drugs and polypharmacy].

Vnitr Lek 1999, 45(5):315-318

The author deals with the causes of increasing polypharmacy in the Czech Republic. He discusses all problems concerned with treatment and analyzes attitudes of drug manufacturers, physicians, patients, the state and its health policy. He mentions also possible ways how to reduce polypharmacy. Attention is drawn to the role of the pharmacist in conjunction with extension of the spectrtum of drugs which can be purchased without restriction.

[Free radicals and extracorporeal renal replacement therapy].

J Eiselt, J Racek, K Opatrný

Vnitr Lek 1999, 45(5):319-324

Free radicals (FR), highly reactive substances with an unpaired electron in the outer orbital attack lipids, proteins and nucleic acids and alter the structure and function of these macromolecules. Against the negative effects of FR during evolution various defense mechanisms developed described comprehensively as antioxidant defense. Under physiological conditions in the organism equilibrium is established between free radical production and antioxidant defense factors. Extracorporeal renal replacement mechanisms can interfere in a negative way with this equilibrium. They provoke the formation of FR and at the same time they weaken the antioxidant...
