[Ultrasound densitometry of the heel bone in a random sample of the adult Czech population].

To assess the state of the skeleton of 870 subjects selected at random the authors used ultrasound densitometry of the talus. The assessment was made using a Cuba Clinical apparatus (McCue Ultrasonic Ltd.UK). The assessed parameters were BUA in db/MHz (broad band ultrasound attenuation) and VOS in m/s (velocity of sound). From the results it is apparent that the BUA values in men reach their peak at the age of 31-35 years. In women the BUA values are highest between 18 and 20 years, then follows a decline and again a rise between 41 and 45 years. Fibrates are a standard in the treatment impaired lipid metabolism, in particular in combined disorders. The classical Helsinki trial provided evidence of a decreased incidence of IHD in the treated group as compared with the group on placebo. [Intestinal permeability in patients with food allergy].

The authors examined the intestinal permeability using the lactulose mannitol test in 13 women, mean age 45.7 years with a confirmed food allergy before they were on an elimination diet. The intestinal permeability did not differ significantly in these patients as compared with healthy volunteers. The intestinal permeabiity test with lactulose and mannitol thus cannot be recommended on the basis of these results for the diagnosis of food allergy.
[A new method of administering analgesic sedation before gastroscopy--a comparative study of 100 patients].

The authors administered to two groups (56 and 44) patients before gastroscopy analgosedation-5 mg diazepam and local anaesthesia of the throat. To the first group diazepam was administered by the i.v., to the second group by the sublingual route. The objective was to find out whether analgosedation is desirable from the patients aspect and whether the sublingual form of premedication is accepted by the patients. The authors found that 90% of the patients wish to have analgosedation and anaesthesia before the procedure. The sublingual form of administration was accepted by 89% of the patients.

As part of screening focused on the incidence and risk of the diabetic foot syndrome in patients in a model area the authors compared values of different parameters of non-invasive examination methods of angiopathy and neuropathy in patients with diabetic foot and in diabetic patients without diabetic foot. They compared the data with normal clinical diagnosis and used them to assess the degree of risk of diabetic foot. Patients with diabetic foot had when examined for the threshold of vibration perception (VPT) by a biothesiometer, as compared with patients without diabetic foot, significantly higher VPT levels 361 +/- 16 vs. 25 +/- 12 V, p < 0.001....

The authors give an account of a 59-year-old patient with a significant stenosis of the left main coronary artery and occlusion of the right coronary artery, occlusion of the ramus interventricularis anterior after failure of surgical revascularization and with a left ventricular ejection fraction of 20%. The authors performed a successful percutaneous coronary angioplasty of the left main coronary artery with an implantation of a stent. They discuss contemporary possibilities and limitations of catheterization treatment of stenosis of the left main coronary artery.

Assisted reproduction techniques (ART) are modern medical methods which should help infertile couples to resolve their problems. With the introduction of ART new ethical and technical problems arise. The authors wish to demonstrate on the submitted two case-histories the need of interdisciplinary collaboration of the specialist in internal medicine and gynaecologist when dealing with a serious complication of ART--ovarian hyperstimulation syndrome (OHSS).
[Spinocellular carcinoma in a patient with Fanconi's anemia].

The authors describe the case of a 24-year-old female patient with Fanconi's anaemia where they diagnosed spinocellular carcinoma of the oesophagus. At the same time they found skeletal abnormalities, caked kidney with pelvic dystopia and uterus unicornis. In patients with Fanconi's anaemia frequently acute leukaemia, liver tumours and spinocellular carcinoma are described. The knowledge of these facts makes early diagnosis of these associated diseases possible.

Heparin induced thrombocytopenia is encountered in clinical practice as type I and type II. Heparin induced thrombocytopenia type II is one of the most serious complications of heparin treatment. Its incidence is between 0.5-5% of all patients treated with heparins. Contrary to type I which is most probably caused by simple interaction of heparin with platelets, in type II factors of the immune system are involved. The laboratory diagnosis is complicated and negative, in vitro tests do not rule out the diagnosis in vivo. The latter is assessed most frequently per exclusionem when there is a marked drop of platelets associated with the clinical picture...
