[The effect of furosemide on urinary excretion of oxalic acid, vitamin C and vitamin B6 in chronic kidney failure].
M Mydlík, K Derzsiová, E Zemberová, A Rajnic
Vnitr Lek 1998, 44(3):127-131
The authors describe the effect of a single i.v. dose of 20 mg furosemide on the excretion of oxalic acid, vitamin C and vitamin B6 in a control group and in patients with chronic renal failure without dialyzation treatment. An increased urinary excretion of oxalic acid, vitamin C and vitamin B6 was found during the first three hours after furosemide administration in both groups. The effect of furosemide persisted for six hours in patients with chronic renal failure without dialyzation treatment. The authors described a new hitherto unknown positive side-effect of furosemide, i.e. enhanced urinary oxalic acid excretion in the control group and in...

Vnitr Lek 1998, 44(3):132-134
The prevalence of Helicobacter pylori is influenced in a significant way by geographical conditions and depends to a certain extent also on the economic standard of different countries. Some published work provides evidence that Helicobacter pylori infections in eastern European countries are in general more frequent than in western European countries. It cannot be ruled out, however, that in different countries there are regional differences as far as Helicobacter pylori is concerned. In a group of 309 subjects, none of those treated previously to eradicate H.pylori, at least three weeks before blood sampling no preparations of the type of H2 blockers,...

R Kizek, M Dastych, V Koutník, J Hebváný, J Simek
Vnitr Lek 1998, 44(3):135-141
The paper deals with the problem of selenium in the food chain of man. The authors investigated the serum selenium concentration of healthy blood donors in four South Moravian regions. Consistent with other authors they found a marked deficiency of this element. While optimal values in analyzed samples were found in 11% in the Trebíc area, 5% in the Vyskov and Brno area, in the Znojmo area no specimens belonging in that group were found. Based on the assembled results the authors consider 22% of the South Moravian population selenium deficient. With regard to this finding it would be useful to modify the therapeutic pattern in some serious conditions....
[Tactics of hypolipidemic therapy after myocardial revascularization].
J Chlumský, P Dostálová, D Holá, P Potuzník
Vnitr Lek 1998, 44(3):142-144
Previous investigations revealed that hypolipidaemic treatment of patients with IHD in the Czech Republic is inadequate. Patients after revascularization operations are discharged from cardiosurgical departments without suitable medication and moreover the values of the lipid spectrum are affected by the surgical intervention. The objective of the investigation was to evaluate the indication of hypolipidaemic treatment during hospitalization or later during check-up examinations by attending physicians. The authors evaluated retrospectively 100 patients after revascularization operations of the heart discharged without hypolipidaemic treatment and...
[The effect of weight reduction on compensation in diabetic obese patients with non-insulin dependent diabetes mellitus].
M Szabó
Vnitr Lek 1998, 44(3):145-150
Non-insulin-dependent diabetes mellitus is associated with obesity in 80 to 90%. The presence of obesity enhances insulin resistance, which increases the demands on insulin secretion and causes deterioration of glucose tolerance. Hyperinsulinism has some metabolic sequelae which increase the risk of the development and progression of atherosclerosis. Reduction of body weight is a basic therapeutic provision in obese diabetics. It results not only in better compensation of diabetes but also other metabolic parameters and blood pressure as it leads to a higher tissue sensitivity for insulin. This can delay the development of long-term complications of...

I Porsová-Dutoit, Z Geisslerová, P Kasal
Vnitr Lek 1998, 44(3):155-157
The authors present an account of all contemporary possibilities to use Internet in medicine. They summarize the advantages and disadvantages of electronic mail (E-mail) and give a list of services available via the World Wide WEb: the possibility to inspect more than seventy medical journals, consult books and specialized databases and acquire further medical training. They demonstrate actual possibilities how to use Internet in everyday medical practice. The authors present also data indicating the general scope of Internet and its application in medical sciences. In the conclusion the authors reflect on the future application of Internet and point...

[Disorders of bone mineralization and their treatment in Crohn's disease].
J Kocián, J Kociánová
Vnitr Lek 1998, 44(3):162-165
Frequent complications of Crohn's disease include disorders of bone mineralization. They are due to a reduced dietary calcium supply in patients with lactose intolerance and a certain degree of malabsorption of calcium as well as vitamin D. The position is made worse by corticoids used in treatment of the basic disease, because they interfere not only with vitamin D conversion into its active (and much more effective) metabolites but also with osteoid formation In the early diagnosis of demineralization a densitometer can be used; markers of bone metabolism are used so far less frequently. As to treatment either blockers of enhanced bone resorption...
[Infections of the diabetic foot--etiopathogenesis and possible preventive strategies].
T Mudríková, I Tkác
Vnitr Lek 1998, 44(3):166-169
The St. Vincent declaration defined the aim to achieve within the next five years a 50% reduction of the number of amputations on account of diabetic gangrene. This can be achieved by systematic application of preventive and therapeutic provisions in practice. The authors mention the most important pathogenetic factors leading to the development of lesions of the lower extremities in diabetics as well as the most frequent microorganisms participating in their infection. They remind also of the most important preventive measures to be taken by patients and physicians.
