[Treatment of significant coronary stenoses using stents without preliminary balloon angioplasty: present status, results, perspectives].

The authors present an account on a group of 57 patients where they tried to treat 58 significant coronary stenoses by means of direct stenting without predilatation. In two patients they were able to implant the stent only after previous dilatation, in one instance postdilatation was necessary because of a residual stenosis of more than 20% and in one instance it was necessary to implant another stent on account of distal dissection. Direct stenting was successful in 93%. Treatment of relapses of multiple myeloma is a problem which was not resolved quite satisfactorily so far. One of the possible therapeutic approaches is administration of major doses of chemotherapeutic agents with the support of autologous transplantation. Treatment of dyslipidemia and its frequently associated complications (manifest atherosclerosis) is very pretentious from the economic aspect. The authors describe two cases suffering from Caplan s syndrome, i.e. a combination of pneumoconiosis and rheumatoid arthritis, in miners of deep coal mines in southern Moravia. The finding on the lungs preceded the manifestation of articular symptoms. The disease progressed even after the risk of fibrogenic dust was eliminated. In both patients occupational diseases were involved.

Hypofibrinolysis caused by increased PAI-1 levels in patients with insulin resistance (IR) is one of the most common acquired prothrombotic states with higher risk of arterial thrombosis associated with atherosclerosis. Atrial flutter is a common cardiac dysrhythmia which responds for half of the supraventricular tachycardias with exception of atrial fibrillation.
