[Glanzmann thrombasthenia--a defect in the surface membrane of platelets].

P Cieslar, J Dyr, J Suttnar, O Matousová, B Friedmann

Vnitr Lek 1990, 36(8):729-737

The authors present a clinical description, detailed platelet function analysis, and certain biochemical parameters in two siblings with Glanzmann thrombasthenia (G. t.). The isolated occurrence of this disorder in the family corresponds with its autosomal recessive inheritance. In both cases blood platelets completely failed to aggregate. In contrast, the platelet interaction with ristocetin, reflecting their ability to adhere to the subendothelium, the so-called "shape change", the storage granule contents and their release and arachidonic acid metabolism were unaffected. Further, the aggregation abnormality was accompanied by marked procoagulant...

[Monocytosis and myelomonocytic dysplasia in myelodysplastic syndrome].

R Neuwirtová, V Malasková, M Steruská, K Indrák, V Vozobulová, J Haber, A Smolíková, J Musilová, J Jelínek

Vnitr Lek 1990, 36(8):738-745

Myelodysplastic syndrome (MDS) is frequently associated with monocytosis in the blood and myelomonocytic dysplasia in the bone marrow. In two groups of patients with MDS, all subtypes excluding CMML, the authors demonstrated that monocytosis was present in 15% of the patients in group I according to the haemogram at the time when the diagnosis was established and in 19% in group II where it was required that at least in half the haemograms throughout the course of the disease there were more than 10% monocytes. No difference was found in the prognosis of patients with monocytosis, as compared with patients with monocytopenia as regards the life span...

[Immunologic classification of lymphoid cells based on the detection of membrane phenotype using monoclonal antibodies in patients with T-lymphoproliferative diseases].

K Koubek

Vnitr Lek 1990, 36(8):746-752

A series of (CD1-CD10) mouse monoclonal antibodies reactive with human T lymphoid cells, in different stages of differentiation, was used for immunological classification of leukaemic cells of 25 patients with T cell lymphoproliferative disorders, using a fluorescence assay. The majority of patients with T-ALL had cells of an immature or early thymic phenotype and patients with T lymphoblastic lymphoma had phenotypes corresponding to different levels of more mature stages of T cell differentiation. Three patients with T-CLL and one patient with mycosis fungoides had mature T cell phenotypes CD3+, CD4-, CD8+, corresponding to cytotoxic/suppressor cell...

[Does the left atrial dimension predict the short-term effect of electric cardioversion?].

B Stancák, J Pella, J Resetár, M Dubíková, M Palinský, J Bodnár

Vnitr Lek 1990, 36(8):753-758

The authors evaluate in their retrospective investigation the influence of the left atrium, assessed by echocardiography, on the short-term effect of electric cardioversion in auricular fibrillation. The group comprises 44 patients divided into four groups: ischaemic heart disease, cardiomyopathy and specific myocardial affection, mitral stenosis and idiopathic fibrillation. The authors did not find a significant difference between the left atrial diameter in patients with successful electric cardioversion (ECV) (44.4 +/- 8.2 mm) and patients with not successful ECV (43.4 +/- 6.7 mm). The authors did not prove the influence of age and aetiology of...

[Morphologic changes in chronic active superficial Campylobacter pylori-positive antrum gastritis after treatment with bismuth].

M Sorf, V Krislo, M Gogora, O Masek

Vnitr Lek 1990, 36(8):759-762

The authors investigated the effect of one month bismuth treatment (Bismuthi citrici 120 mg per capsule), 4 x 120 mg/day, on morphological changes of the gastric mucosa along with the effect on Campylobacter pylori (CP) in a group of 23 probands with histologically verified active superficial CP positive antrum gastritis. The probands suffered only from functional dyspepsia, to eliminate the action of other disease of the digestive tract on chronic gastritis. Complete eradication of CP occurred in 65.2% and disappearance of granulocytic infiltration as a manifestation of activity in 73.9%. The disappearance of activity correlated with the eradication...

[Uric acid--a risk factor or atherosclerosis marker in type 2 diabetes?].

I Tkác, J Bomba, A Cvanigová

Vnitr Lek 1990, 36(8):763-768

The incidence of some risk factors of ischaemic heart disease (IHD) was investigated in a group of 91 type 2 diabetics. A group of 57 patients who had a myocardial infarction was compared with a control group of 34 diabetics without clinical and electrocardiographic signs of IHD. In the group of diabetics with IHD there was a significantly higher proportion of hypertonic patients (70%), as compared with the control group (47%). The diabetics with an infraction in the case-history had a significantly higher mean age and a longer mean duration of diabetes. There was not a significant difference between the two groups as regards mean values of cholesterol,...

[Diagnosis of diabetic macroangiopathy of the lower extremities using rheography].

A Lacko, A Kreze, G Fodor, J Velemínský

Vnitr Lek 1990, 36(8):769-773

Rheography (impedance plethysmography) makes it possible to differentiate the normal condition of arteries of the lower extremity from functional and organic affections. The examination of a group of 325 patients with diabetes mellitus revealed that without the use of rheography, i.e. when relying only on examination of the lower extremities by palpation, the presence of macroangiopathy would escape attention in 46% of the affected subjects. Macroangiopathy of an organic character was found significantly more frequently in type 2 (NIDDM) (35%) than in type 1 (IDDM) (9%; p less than 0.03). Its incidence increased in both types of diabetes with the duration...

[Biochemical values in erythrocytes before freezing and after freezing at low temperatures].

L Buchtová, M Láznicka, I Bilýk, Z Hrkal

Vnitr Lek 1990, 36(8):774-778

The authors tested three ways of freezing red cell concentrates with glycerin. At -30 degrees C in a deep freezer; on a Planer at the rate of 1 degree C per minute and by immersion into liquid nitrogen to -196 degrees C, stored in cryo-containers in liquid nitrogen. In samples processed in all three ways, incl. the follow-up of glycolysis in red cell concentrates in acid-citrate-dextrose solution, the levels of the following enzymes were assessed: glucose-6-phosphate dehydrogenase, glutathione reductase, pyruvate kinase. In addition also the levels of sodium, potassium, pH and glucose were assessed. The enzyme estimations of all three mentioned enzymes...

[Total lipid levels in erythrocyte concentrates before and after freezing].

L Buchtová, M Láznicka

Vnitr Lek 1990, 36(8):779-782

The authors investigated changes of total lipids, cholesterol, triacyglycerols in red cell concentrates collected into acid-citrate-dextrose solution before freezing with glycerin and after freezing with glycerin to -30 degrees C in a deep freezer and to -196 degrees C by immersion into liquid nitrogen. It was revealed that the triacylglycerol levels after deglycerolization increased approximately three times. In transfusions of major amounts of thus processed blood the possibly impaired tissue saturation with oxygen will have to be taken into account. It may be concluded that lipids released from red cells impaired as a result of freezing may contribute...

[Benign lactic acidosis in 2 female diabetics treated with insulin].

V Rosival, L Belková, J Martinková, A Takác

Vnitr Lek 1990, 36(8):783-785

In two young female diabetics treated with insulin lactic acidosis developed. After normalization of the low pH of blood by NaHCO3 infusion the subjective complaints disappeared and eventually the blood lactate level reached normal values.

[Crohn's disease of the stomach].

M Sorf, V Krislo, V Kratina, M Kascák, O Masek, O Hloza, M Gogora, L Plank, A Beseda

Vnitr Lek 1990, 36(8):786-790

The authors describe the incidence of Crohn's disease at an uncommon site the stomach--in a 40-year-old man. With regard to the steadily increasing incidence of the disease, it is important to consider its presence also in uncommon sites in the gastrointestinal tract. The establishment of the diagnosis must be comprehensive, as even histological examination of endobioptic specimens or resected portions need not be unequivocal from the diagnostic aspect.

[Electrocardiographic diagnosis and classification of acute coronary attacks].

S Cagán, L Kuchárová

Vnitr Lek 1990, 36(8):791-798

In clinical practice the use of terms transmural and non-transmural infarction, based on the evaluation of the electrocardiogram is abandoned, as the correlation of an abnormal Q wave and transmural infarction and its absence in non-transmural ischaemic lesions is inadequate. A cardiac infarction with a Q wave and a non-Q wave infarction have certain different clinical and prognostic characteristics. A non-Q infarction is characterized during hospitalization usually by more frequent episodes of angina pectoris and a lower mortality than infarction with a Q wave. The depression of the ST segment in a standard electrocardiogram in acute coronary attacks...

[Diabetic osteopathies. 6. Treatment and its pitfalls].

J Kocián, J Brunová

Vnitr Lek 1990, 36(8):799-804

The basic prerequisite of treatment of diabetic osteopenias is perfect metabolic compensation of diabetes. Insulin administration is an advantage in this respect, as it enhances calcium absorption from the gut and reduces its urinary excretion. Conversely, oral antidiabetics interfere in a negative way with vitamin D metabolism and thus also calcium metabolism and mineralization of bone. The combination of calcium, small doses of vitamin D, NaF and exercise used in the treatment of diabetic osteoporoses leads in general to a significant rise of the calcium serum level, an insignificant rise of the phosphorus level and it reduces alkaline phosphatase...

[Reflux nephropathy].

R Sabra

Vnitr Lek 1990, 36(8):805-816

A corticopapillary scar is a frequent finding on urography in patients with vesicoureteral reflux. It is considered a typical sign of so-called reflux nepropathy. It develops most frequently in children aged 5-7 years and has a negative impact on the growth of the kidney. In its development three factors participate: ureterovesical reflux, intrarenal reflux associated with so-called refluxing papillas and urinary infection. The inflammatory cicatrical process may affect the whole kidney--small shrivelled kidney--or only a portion of the kidney. The development of scars is explained by the so-called bing-bang theory according to which all refluxing...

[Use of nonparametric methods in medicine. III. Comparison of levels in 2 independent samples].

A Gerylovová, J Holcík

Vnitr Lek 1990, 36(8):817-823

The authors present two nonparametric tests which can be used to compare the level of a random variable based on two independent random samplings. Wilcoxon's (Mann-Whitney's) test is based on the sum of sequences which are appropriate for individual elements in a common group. In large samples the tested criterium is based on the normal distribution, for small samples a table of critical values is enclosed which makes rapid decision possible. The median test is based on the idea that if two independent random samples are from the same population both samples contain equal numbers of elements above and below the median for the common group of all elements...
