[Immunologic monitoring of bone marrow transplantation. Experience with 16 cases].
A Májský, J Soucek, P Herzog, P Korínková, J Prazák
Vnitr Lek 1990, 36(4):318-323
HLA-A, B antigens and the ABO group were examined in 184 patients with aplasia of bone marrow and leukaemia and in 373 of their relatives, mostly siblings. A HLA-A, B donor, identical or compatible, was found for 35.87% patients, a HLA-DR identical for 84.21% of 38 patients who had a HLA-A, B identical relative. Bone marrow was transplanted to 16 patients (10 with bone marrow aplasia, 6 with acute or chronic leukaemia), with one exception bone marrow from a sibling. The bone marrow was accepted in all patients but two where the transplantation was made despite MLC positivity. From the results ensues that it is essential for successful transplantation...
[The effect of acetylsalicylic acid and dipyridamole on glomerular filtration and proteinuria in chronic glomerulonephritis].
K Němecek, I Petrů, V Knotková, M Jáchymová
Vnitr Lek 1990, 36(4):324-329
The authors treated 14 patients with chronic proliferative glomerulonephritis for one year with acetylsalicylic acid--400 mg/day--and dipyridamole--225 mg/day. They investigated changes of glomerular filtration and proteinuria before treatment, during treatment and one year after its completion. They found that in the course of treatment proteinuria did not change and glomerular filtration declined insignificantly. During the subsequent year proteinuria and glomerular filtration declined significantly. The above findings can be partly explained by changes in the synthesis of renal prostacyclin and thromboxane. The combination of acetylsalicylic acid...
[Metabolic and acid-base changes in intensive diuretic therapy in heart failure].
V Krátký, J Bartonová, R Hrdina
Vnitr Lek 1990, 36(4):330-341
A group of 65 patients with advanced heart failure was examined with the aim to disclose changes in serum glucose, creatinine, potassium, chloride, and acid-base balance under the influence of intensive diuretic therapy. 50 patients were treated with ordinary furosemide and amiloride combination (average observation time 25 days), in 15 cases amiloride was replaced by captopril 75-150 mg/day (average observation 15.5 days). The results are as follows: 1. We found no rise of glycaemia in non-diabetics with either ordinary diuretic therapy or captopril. On the contrary: stress hyperglycaemia in the beginning of the therapy normalized in the course of...
[Relative mass and serum lipids in men with various coronary scores].
O Kusá, O Luknárová, I Riecanský
Vnitr Lek 1990, 36(4):342-348
The authors evaluated the relationship between relative body weight according to Broca and serum lipids in 120 men with stable angina pectoris with a positive coronary score and in 30 men with cardiovertebrogenic syndrome and a negative coronary score. The differences between the Broca index and serum lipid values between men with angina and men of the control group were statistically significant (p less than 0.001); the differences were independent on the value of Broca's index. Correlation coefficients, simple and partial, between Broca's index and different lipid indicators (cholesterol, triacylglycerols and HDL-cholesterol) were not statistically...
[Atherogenicity indices and coronary risk].
O Kusá, O Luknárová, I Riecanský
Vnitr Lek 1990, 36(4):349-354
The authors compared three different indices, of atherogenicity in 120 men with ischaemic heart disease, type stable angina pectoris (mean age 35.9 +/- 7.54 years) with 30 men of a control group--mean age 42.1 +/- 8.89 years). They evaluated relationship between the atherogenicity index and coronary score by the Spearman correlation coefficient. They consider the index HDL-cholesterol: total cholesterol the best index for evaluation of the coronary risk. This index differentiates men with ischaemic heart disease, type stable angina pectoris with a positive coronary score from the control group of men with a negative coronary score (r = -0.54, p less...
[Obliterative disease of the carotid and peripheral arteries in hyperlipoproteinemia].
M Scudlová, H Vaverková, O Weinbergová, V Seidlová, L Pudilová
Vnitr Lek 1990, 36(4):355-362
The authors give an account of changes of the carotid and peripheral arteries detected by ultrasonic examination in a group of 133 patients with different forms of primary hyperlipoproteinaemias. Stenoses in the extracranial carotid circulation were found in 31% of the group, stenoses of the peripheral arteries in 37%; in 18% combined changes of the carotid and peripheral system were found. The authors proved a rising trend of the number of risk factors with the rising number of arterial stenoses. The most frequent risk factors were obesity and smoking. In the entire group changes of the peripheral arterial system were found in more than half the patients...
[Etolip and Lipanthyl in the treatment of hyperlipoproteinemia].
R Ceska, J Sobra, R Procházková, M Kvasilová
Vnitr Lek 1990, 36(4):363-367
Within the framework of clinical tests of Etolip (ethophylline clofibrate cps. 125 mg, Pharmaceutical Research Institute Modra) this preparation was administered to 28 patients with different types of hyperlipoproteinaemia in a specialized clinic for disorders of the lipid metabolism. The period of administration was four weeks, the dose 2 cps. twice a day. The effect of this new hypolipidaemic drug was compared with that of Lipathyl. Treatment with Etolip did not affect significantly the cholesterol and triacylglycerol levels, and the concentrations of apolipoprotein B and LDL-cholesterol did not change significantly. All these parameters were affected...
[A Czechoslovak low-energy protein diet in the treatment of obesity].
V Hainer, M Kunesová, V Stich, J Parízková, A Zák, P Hrabák, V Wernischová, L Dědicová
Vnitr Lek 1990, 36(4):368-378
The low-energy protein diet providing 1559 kJ is the first accurately defined diet for the treatment of obesity in the CSSR. The daily amount contains 4.0 g fat, 33.0 g protein, 50.0 g carbohydrate, 5.6 g fibre and daily recommended allowances of vitamins. The diet is enriched with potassium, magnesium and iron. The low-energy protein diet was used for a period of 28 days in the treatment of 49 obese patients aged 40.49 +/- 1.39 years whose initial weight was 110.14 +/- 3.41 kg and the BMI 39.44 +/- 1.13. The therapeutic regime comprised in addition to the diet adequate physical exercise of aerobic character and training of correct eating behaviour....
[Method for the determination of ionized calcium in clinical practice].
I Porsová, J Blahos, V Chytil, J Kotas
Vnitr Lek 1990, 36(4):379-386
The authors give an account of their experience with the problem of assessment of ionized calcium (Ca2+) in clinical work. To assess Ca2+ they use an ICA 2 analyzer of Radiometer Co. Copenhagen which makes it possible to assess concurrently the Ca2+ concentration at a given pH (Ca2+/act. pH), pH and Ca2+ calculated for pH 7.4 (Ca2+/pH 7.4) in capillary and venous blood. The authors confirmed the significant difference between Ca2+/act. pH in venous and capillary blood which is due to the pH and they did not find a significant difference in Ca2+/pH 7.4 in venous and capillary blood. They found that the specimens of capillary blood can be kept in capillaries...
[Comparison of methods of cryopreservation of blood platelets].
Z Gasová
Vnitr Lek 1990, 36(4):387-390
In the submitted paper the author compares two different methods of cryopreservation of thrombocyte concentrates by means of in vitro tests with the aim to find under local conditions a suitable method of cryopreservation of platelets suited for practical use. Thrombocyte concentrates were preserved by non-programmed deep freezing at -80 degrees C with storage of platelets at the same temperature and programmed deep freezing up to -100 degrees C with subsequent storage of platelets in liquid nitrogen vapours. As a cryoprotective substance DMSO in a final 5% concentration was used. Between the two cryopreservation methods no significant differences...
[Severance of the accessory bundle of Kent in a 13-year-old boy with Ebstein's anomaly of the tricuspid valve].
B Vítek, J Nicovský, P Nĕmec, M Suchánek, A Necasová, B Uchytil, J Dominik, M Osmerová
Vnitr Lek 1990, 36(4):391-396
In patients with Ebstein's anomaly of the tricuspid valve combined with WPW syndrome, possibly other cardiac disease, in particular with the repeated occurrence of severe tachydysrhythmias we can foresee cure or at least elimination of complaints by simultaneous surgical treatment of all defects. The author presents experience with the surgical treatment, the first of its kind in this country, in a boy aged 13 years. By better diagnosis it proved possible to locate correctly and to severe by surgery the abnormal accessory bundle between the right atrium and right ventricle, simultaneously with a plastic operation of the insufficient tricuspid valve...
