[Disorders of heart rhythm in hypertrophic cardiomyopathy].

In 64 patients with hypertrophic cardiomyopathy 24-hour Holter monitoring of the ECG was made in order to detect the incidence of individual disorders of the cardiac rhythm. The examination revealed a large number of potentially malignant ventricular arrhythmias (47% of the patients). Most frequently polytopic ventricular extrasystoles were involved (31.3%), attacks of ventricular tachycardia (20.3%), less frequently bigeminy was recorded (10.9%) and early extrasystoles type R on T (3.1%). The aim of the work was a more detailed characteristic of patients with hypertrophic cardiomyopathy who are threatened by an increased risk of serious disorders of the cardiac rhythm and thus probably also by sudden death. [HLA antigens and primary prolapse of the mitral valve].

The authors present results of investigation of antigens of the HLA system in primary mitral valvular prolapse. The investigated group of patients was formed by 23 not related patients with primary mitral valvular prolapse. (15 women, 8 men, aged 18-55 years) where HLA antigens of loci A, B and C were assessed. They found a significantly increased frequency of antigen HLA-B35 in the patients (56.52%), as compared with the population (18.33%, x2 = 18.48, Pcorr less than 0.01). The measure of the observed relationship is expressed by the relative risk value--RR = 5.81. The difference in the frequency of the other HLA antigens was not statistically significant.

Indication criteria of adult patients with atrial septal defects type II for surgery are not uniform so far. In a group of 150 adult patients with atrial septal defects type II operated in 1978-1987 at the cardiosurgical department in Brno the authors report the following results: a mortality on operation of 0.7% and a very satisfactory postoperative condition in almost all patients. By assessment of corticodiaphyseal indices from the clavicle the bone mineralization was investigated in a group of 215 type II diabetics and 40 type I diabetics. In 56% of type II diabetics pathologically reduced values of bone mineralization were recorded: in 49% of the men and more than 60% of the women. Among type I diabetics 42.5% pathological values were recorded; in men only 29% but in women in more than 52%. [The effect of amiodarone on thyroid gland activity].

In order to elucidate the incidence of undesirable effects of amiodarone on thyroid activity, the authors examined a group of 11 patients who used this preparation for 8 to 44 months on account of disorders of the cardiac rhythm. In two patients clinical examination revealed obvious thyrotoxicosis, which after discontinuation of amiodarone receded spontaneously in the first patient in the course of five months, in the second patient within two months. The authors analyzed a group of 164 subjects where they collected during gastroduodenoscopy specimens of the gastric mucosa for histological and microbiological examination. Campylobacter pylori (CP) was detected in 37 of 41 subjects with gastritis type B (90.2%), in 36 of 45 patients with duodenal ulcers (80%) and in 15 of 24 patients with gastric ulcers (62.5%) and in one of 15 subjects with gastritis type A. In 8 patients with drug induced and reflux changes the presence of CP was not detected. In the control group of 31 subjects CP was found in 4 (13%). The authors submit an account of contemporary possibilities of the application of laser in the endoscopic treatment of some gastrointestinal diseases. They give an account of their own experience with the use of Nd YAG laser, particularly in the treatment of oesophageal tumours, stenotizing tumours of the rectum and with removal of broad-based polyps. [Idiopathic hemochromatosis with emphasis on its hereditary characteristics and the HLA system].

The author gives an account of idiopathic haemochromatosis and quotes a case under observation. The main attention in the submitted paper is focused on the relationship of the disease and the HLA system. The investigated patient and his two sons were subjected to HLA typing. In all three subjects antigens HLA-A3, HLA-B7 were found which is consistent with data reported in the literature.

The authors describe a case of a female patient who developed an allergic reaction to occupational allergens. The authors discuss the necessity of correct evaluation of suitable jobs in an environment causing allergization. The patient was engaged in the magistraliter preparations of medicaments in a pharmacy. The authors draw attention to possible complications, when allergic patients are referred late to comprehensive allergological occupational medical examinations. Correct enlistment into work with regard to the health status is an important preventive factor which prevents the development of occupational allergies.
