Benzodiazepiny a Z-hypnotika vyžadují zvýšenou opatrnost a zolpidem nelze obecně označit za respiračně neutrální. Antagonisté orexinových receptorů mají příznivá krátkodobá data u selektovaných pacientů, avšak jejich bezpečnost u těžké CHOPN a overlap syndromu musí být dále ověřena.
